Two miners were lifting a rock together to stack on a pallet, and one let go to set the rock down and the other miner did not, and the rock smashed the miner's finger.
Working on the chopper, a large rock was being moved with the rollers, and the miner was caught in between the rock and the chopper, causing a back injury.
Employee was pushing a rock on the rollers of the chopper when another rock rolled forward smashing their fingers between the two rocks.
The miner was using a polisher/grinder to repair the sandvik crusher and something unknown got in EE's eye irritating it where EE needed to see a doctor.

WORKER WAS CLEANING OUT DEBRIS CATCH BETWEEN THE FRAME PLATE AND THE CHOPPER RIG LOWER PLATE ITSELF. WHEN DEBRIS WAS REMOVED THE CHOPPER CYCLED AND THE FLEXING ACTION OF THE TWO PLATES CAUGHT THE WORKERS LEFT HAND FINGERS BETWEEM THEM BEFORE IT COULD BE WITHDRAWN. THE MIDDLE FINGER AND THE RING FINGER WERE BOTH MASHED NEAR THE NAIL.
WORKER WAS MOVING A CHUNK OF ROCK ON THE CHOPPER TABLE WHEN THE ROCK ROLLED TO ONE SIDE AND CAUGHT THE WORKERS FINGERS IN BETWEEN ANOTHER ROCK. RESULTING IN (3) CUT FINGERS.
SMALL PIECE OF STEEL FROM A CHIPPING HEAD STUCK THE WORKER IN THE CHEST. THE OBJECT WAS REMOVED BY A DOCTOR.
WORKER WAS STACKING CHOPPED ROCK ON A PALLET WHEN RIGHT HAND PINKY FINGER GOT CAUGHT BETWEEN TWO ROCKS. RESULTING IN A SMASHED AND CUT FINGER.
WORKER WAS STACKING ROCK ONTO THE CHOPPER DECK WHEN EE DROPPED A ROCK ON THE RING FINGER OF LEFT HAND. EE WAS TAKEN TO MERCY HOSPITAL ER IN FORT SMITH, AR. AND RECEIVED (5)FIVE STITCHES TO THE TIP OF FINGER AND RETURNED TO WORK.!
Employee began to complain of chest pains and difficulty breathing. Area Supervisor was called and arrived in 10 minutes on site. They put EE in their truck and took EE to the local emergency room.
WORKER WAS CHIPPING AWAY A PIECE OF STONE WHEN A PIECE OF THE HAMMER BROKE OFF AND RICOCHETED BACK OFF THE ROCK FACE STRIKING SUBJECT WORKER IN THE NOSE.
WORKER DROPPED A CHOPPED PIECE OF ROCK ON RIGHT FOOT. THE ROCK MADE CONTACT WITH THE FOOT JUST ABOVE THE STEEL TOE PROTECTED AREA RESULTING IN A CLOSED FRACTURE OF MEDIAL PHALANX OF THE RIGHT LESSER TOE.
WORKER WAS STEPPING BACKWARDS WHEN EE CAUGHT THE EDGE OF A SKID WITH SHOE. EE LOST BALANCE AND FELL BACKWARDS ONTO THE CONCRETE FLOOR. EE STRUCK THE BACK OF HEAD ON THE FLOOR RESULTING IN A MILD CONCUSSION.
Worker was moving a rock on chopper table into chopping position. But the operator did not make sure it was clear before engaging the chopper action. This resulted in the loss of a finger tip on the right hand.

WORKER WAS LIFTING A 5-GALLON BUCKET OF HYDRAULIC FLUID INTO TRUCK WHEN IT SNAGGED THE TAILGATE OF TRUCK AS IT WAS BEING LIFTED. RESULTING IN A RIGHT SHOULDER STRAIN. WORKER WAS TREATED AND RELEASED BY DOCTOR AT COOPER CLINIC IN FORT SMITH, AR.
Worker was cleaning the waste material out of the chopper. When ee accidentally stepped on the operating lever. The workers hand and wrist was caught between the chopper teeth.
STEPPED ON A ROCK AND TWISTED EE'S RIGHT KNEE.
EMPLOYEE WAS WORKING AND BECAME OVERHEATED AND BEGAN VOMITING. EE TOLD SUPERVISOR AT HAND AND WAS TOLD TO GO TO VEHICLE AND COOL OFF. EE SAID THE SEVERE VOMITING CAUSED INJURY TO RIGHT EYE AND SOUGHT MEDICAL CARE. HOWEVER, THE EMPLOYER WAS NOT NOTIFIED OF THIS POSSIBLE INJURY UNTIL 6/23/2016.
WORKER WAS CLEANING UP WORK AREA WHEN EE's RIGHT KNEE CAME IN CONTACT WITH A SHARP ROCK STACKED ON A PALLET. THIS RESULTED IN A LACERATION THAT REQUIRED SEVERAL SUTURES.
Worker was feeding rock into a chopper when the rock began convex. When chopper was activated the rock rotated quickly to the right and cut off the end of the workers fourth finger of EE's right hand which was under the rock at the time of activation.
SUBJECT WORKER AND TWO OTHER WORKERS WERE MOVING A SKID OF ROCK WHEN THE LOAD SHIFTED. A PIECE OF FALLING ROCK STRUCK THE SUBJECT WORKER ON THE FOURTH FINGER OF THE RIGHT HAND RESULTING IN A LACERATION.
EMPLOYEE WAS CATCHING ROCK FROM CHOPPER WHEN A PIECE OF ROCK STRUCK EE IN THE EYE (RIGHT OR LEFT WAS NOT SPECIFIED). WORKER WAS NOT WEARING SAFETY GLASSES AS REQUIRED. EE RECEIVED A SLIGHT CORNEAL ABRASION. ACCIDENT NOT REPORTED UNTIL 2/29/2016.
Veneer saw blade cover was being reinstalled after routine blade change. When the cover snapped into place the worker was caught off guard by the sound. EE fell back onto the non-operational conveyor system of said machine and suffered back strain and contusions as a result.
Load of excess metal was being moved by a fork truck. As the load began to rise, it shifted and came into contact with employee's lower back. This caused EE to fall forward into the shop wall where EE cut two of EE's fingers on a piece of metal as EE tried to regain EE's balance.
Worker was stacking a piece of chopped rock onto a pallet when his finger was caught between another piece of rock as he was setting the rock down. This resulted in a smashed and broken pinky finger on the left hand.

Worker was chopping rock at the chopper when the rock being chopped broke at a seam and fell off the chopper table. It struck the floor and then came to rest up against the workers right ankle resulting in a sprained ankle.
Worker was attempting to connect a bucket to a skid steer loader when he failed to secure the bucket properly. The bucket dislodged and fell on the workers left ankle. This resulted in a fracture of the of the workers left ankle.
A rock was being moved by another worker on the chopper table when the subject's middle finger of his left hand was caught between two rocks and his finger was broken as a result. His ring finger suffered only a minor contusion.
Worker was removing rock from the chopper and stacking it on a pallet when the pinkie finger on his right hand got caught between two rocks. This resulted in a mashed finger tip on his right hand pinkie finger.
WORKER CLAIMS HE CAME OUT OF THE PORTA-POTTY AND A LOAD OF ROCK DUST WAS DUMPED ON HIM BY HIS FRIEND FROM A SKID STEER. THERE HAVE BEEN NO WITNESSES, AS OF TODAY, TO COME FORWARD WITH ANY INFORMATION TO HELP SORT OUT WHAT DID HAPPENED. THIS ACCIDENT WAS NOT REPORTED TO HIS SUPERVISOR OR SUPERINTENDENT UNTIL 3/2/15.
Worker was picking up a chopped rock with a sharp edge to put on a pallet. It slipped out of his hand and landed on the end of his ring finger on the left hand severing the tip of finger off.
Worker was removing chopped rock from conveyor when the middle finger of his right hand got jammed between two rocks he was removing from the conveyor in order to stack on pallet. Injury resulted in a broken finger just below the first joint.
Worker was chopping a rock on the chopper when the rock shattered into several pieces failing to make a clean cut. One of the pieces fell from the chopper table striking the worker on the right leg as it fell. He was checked over by the ER doctor in Poteau and told to put no weight on the bruised and swollen leg for seven (7) days.
Worker was removing chopped stone from the chopper via conveyor discharge belt. He removed the next stone from the conveyor and stacked it onto pallet in the floor. He then turned his head (Still Bent Over)and this action placed his head into the discharge area. The next stone from conveyor belt struck him on top of head resulting in a cut.
