Employee was on the radio stacker belt when the plant was turned on. He was shaken off and fell from the stacker belt. Employee sustained a broken femur.
Employee was opening the crusher door. The door fell and he tried to catch it. In doing so he suffered a laceration on his left middle finger requiring 21 stitiches and a broken finger tip.
Employee was doing repairs on cone crusher. It has 2 motors and one of the box lids fell off the motor landing on the catwalk were he was standing. The lid hit his left mid foot above the steel toe.
EE was positioning a tail pulley into place when his hand was caught underneath the pulley. The pulley crushed his left hand severing his left hand ring finger from the upper knuckle to the tip of his finger.
EE was preparing to service transmission of the 988G loader. He had jack stand in place and was in the process of placing the bottle jack. The bottle jack slipped out of his hand before it was fully on the jack stand and fell on EE's thumb. The impact broke the thumb in three places. This was found on 12/22; it was thought to be a sprain when it first happened.
