Caught in, under or between a moving and a stationary object

Employee tried to maneuver bearing around the frame of tailpulley. His right ring finger got caught and cut underneath bearing on tailpulley.

Accident type, without injuries

Employee was operating the CASE Excavator when he noticed what he thought was dust coming from the engine compartment. Upon inspection it was discovered that a fire had started in the compartment. He exited the excavator and called for assistance. He was not physically injured in the incident.

Employee was doing repairs on cone crusher. It has 2 motors and one of the box lids fell off the motor landing on the catwalk were he was standing. The lid hit his left mid foot above the steel toe.

EE was preparing to service transmission of the 988G loader. He had jack stand in place and was in the process of placing the bottle jack. The bottle jack slipped out of his hand before it was fully on the jack stand and fell on EE's thumb. The impact broke the thumb in three places. This was found on 12/22; it was thought to be a sprain when it first happened.
