A Miner was taking down a portable plant. The leg on CR11 was being lifted with skid loader and got stuck so the miner was trying to get it free. Pinched hands between the fork and leg of CR11. Miner had proper gloves and PPE. Resulted in 2 fractured fingers on left hand and stitches on finger on right hand.

Greasing and cleaning frozen gravel between the head pulley and guard on bottom side of the conveyor with shovel. Conveyor grabbed shovel and pulled the shovel and right arm into the conveyor. There were no broken bones, treated for a burn on right arm. Was First aid treatment to bandaging on the first visit, follow-up visits to change bandages and observation.

Removing guard under 60" belt. EE was laying under the guard holding it with one hand. Could not hold it and dropped the guard on thigh.

Stepped out of the bulldozer to urinate, the sidewall caved in, burying EE waist deep in gravel, pinning left leg against the track. Hit head on the ripper arm of the dozer, split head open.

Employee had finished operating the dozer and was walking away when he stepped on a dirt clump and broke his ankle.

Ee was welding on paddles of dozer when a piece of hot metal got into his ear and burned it.
