Showing all 16EE was doing an elevator chain and bucket inspection after the elevator had been repaired. EE was sitting on a bucket outside of the elevator, controlling the chain with an inverter and looking up when a 1in -rock became dislodged from the chain/bucket above and struck in the eye. EE was not wearing any eye protection.
EE was moving piece of metal when it slipped and made contact with EE's mouth, requiring a couple of stitches.
EE was stepping off ladder onto uneven surface, and felt a pain in foot (i.e. Pulled muscle or sprain).

A cylinder was lodged in the welding cart. In trying to move/dislodge cylinder against advice, miner injured back muscle.
Victim was securing equipment from loading dock (warehouse) to truck, when he lost his balance and fell approx 4' to the pavement. Left ankle sprain.
Victim was hammering on a bearing role to loosen it wehn a metal bit flew into his left eye. victim was wear glasses but not safety glasses.
Warehouseman placed tool on floor and then stepped on it, twising and spraining his right ankle.

WHILE REARRANGING SOME STEEL PLATES BEING TRANSPORTED BY A FORKLIFT, ONE PLATE FELL, STRIKING EEAND SLAMMING HIM INTO A CEMENT WALL RESULTING INCONTUSIONS ON BACK & ARMS & FRACTURED RIB.
