Employee suffered minor hip and arm pain, due to rolling down from the top of a railcar while wearing fall protection. Employee came into contact with railcar.

The accident occurred at the rotary feeder that had stopped rotating. When attempting to get the feeder rotating again, the employee pulled on the drive chain and subsequently got hand caught between the drive sprocket and the chain.

Employee was carbon arcing inside the materials dryer. A piece of rolled off the welding jacket to EE's groin area causing burns to EE's genitals.

Struck by falling object

EE was doing an elevator chain and bucket inspection after the elevator had been repaired. EE was sitting on a bucket outside of the elevator, controlling the chain with an inverter and looking up when a 1in -rock became dislodged from the chain/bucket above and struck in the eye. EE was not wearing any eye protection.

Warehouse worker was filling a bag with product at a sacking station. He bent down to adjust the bottom of the bag when it made contact with his eye, causing a superficial abrasion to the cornea.
