Employee was operating a DeWalt 4 1/2" grinder when it hit something and bounced back and caught EE's left index finger. ER report states "X-ray did not show any evidence of bone injury. EE did sustain a partial tendon laceration."

Showing all 6Employee was cleaning the tailgate area of his dump trailer when his second and third finger of his right hand got caught resulting in two fractured fingers.
The tailgate of the truck did not latch. Driver lifted the tailgate by hand when it slipped and caught his finger. This resulted in an open fracture of the fourth finger on his left hand.
Employee was hoeing out a chute on the crusher and hurt his back.
Employee suffered a back strain while running our 980G loader.
While the employee was powerwashing our front end loader, the nozzle got stuck inside his boot causing a burn to his left foot.
