EE was changing belt for motor on elevator return for baggers. He cut the belt with a knife while having his left hand on the pulley. When the belt was cut the weight on the elevator cups caused the pulley to turn which caught his finger between the belt and the pulley.

Showing all 4EE was removing a lump of kaolin clay from 12" screw after the lump tripped the screw. EE got on top of an 8' ladder and the ladder slid causing his finger to bump against the screw blade. EE used ladder instead of bucket lift.
Employee was opening end loader door to grease two fittings. The door did not come open immediately and he pulled hard on the door causing it to open quickly witht he edge of the door hitting him above the right eye in the temple area.
EE WAS OPERATING BAGGER FILLING 50# PAPER BAGS WITH KAOLIN CLAY. BAG BURST GETTING CLAY IN EYES. WHEN EMPLOYEE ATTEMPTED TO REMOVE BAG FROM MACHINE HE ACCIDENTLY ACTIVATED THE MACHINE CAUSING IT TO DISCHARGE CLAY ON HIM. HE LOST BALANCE AND FELL BACKWARDS HITTING HEAD ON WALL & LANDING ON BACK. HE SAID HE WAS OK BUT SORE & WANTED TO BE CHECKED. HE WENT TO HOSP, X-RAYED & GIVEN RX.
