Changing teeth on loader, one of the teeth would not go on all the way so I tapped on it and a piece of the tooth broke off and hit my neck which started to bleed. I went to get a rag and called a co-worker over to help.

Was moving rocks off grizzly into crusher jaw manually. Jaw threw 4" rock out that bounced off a guard and into lower abdomen of employee.
