Struck by flying object
Changing teeth on loader, one of the teeth would not go on all the way so I tapped on it and a piece of the tooth broke off and hit my neck which started to bleed. I went to get a rag and called a co-worker over to help.

Was setting up crusher and got dirt in eye. Developed an infection
