HE WAS PUTTING UP HOPPERS ON R/R CARS & PULLED TENDONS IN HIS RT ARM.
HE WAS STEPPING ON THE TRACKS OF HIS DOZER TO GET INTO THE CAB AND HE SLIPPED AND HIT HIS BACK ON THE DOZER
HE WAS CLEANING COAL WITH HIS HIG LIFT AND HIS HAND SLIPPED OF OFF THE STERAING WHEEL AND HIS ARM WENT INTO THE STEARING WHEEL AND THE SPOKES TWISTED HIS ARM.
USING SLEDGE HAMMER REMOVING TOOTH FROM DRAGLINE BUCKET AND SLEDGE SLIPPED AND HIT HIM ON RIGHT RING FINGER.

AS HE WAS GETTING OUT OF HIS DOZER FOR BREAK THE WIND BLEW THE DOZER DOOR SHUT ON HIS RT HAND.
HE WAS OILING PARTS ON HIS DRAGLING WHEN SOME DIRT FROM THE DRAGLINE FELL INTO HIS LEFT EYE.
HE WAS WELDING ON DRILL TRUCK AND WHILE HE WAS WELDING ON THE DRILL SOME SPARKS WENT DOWN AND CAUGHT THE DRILL FUEL TANK ON FIRE.

I.NAME TAKEN TO AREA HOSPITAL AND ADMITTED. .. TRIGGER ON SPRAY WAND NOT WORKING PROPERLY AND FAILED TO SHUT OFF WHEN RELEASED
CLIMBING OFF ROCK TRUCK AT QUITTING TIME WHEN HE STEPPED FROM LADDER ONTO UNSEEN ROCK AND TWISTED LEFT ANKLE.

KEENER WAS RIDING ON BACK OF FLAT BED TRUCK WHILE PATCHING POT HOLES WHEN HE LOST HIS GRIP AND FELL OFF TRUCK TRUCK WAS MOVING VERY SLOWLY FROM POTHOLE TO POTHOLE AT THE TIME
