Fall onto or against objects

While getting off dozer, stepped onto the ground but an unseen stump caused EE to slip and fall and bump left leg.

Struck against a moving object

Driver was backed up to lime pile to dump. As the bed started up, one side stuck and caused the unit to go on its side. Driver got out through windshield. Was transported to hospital but was released. His side is sore.

While carrying antifreeze to his 95Z Kawaski loader, he slipped on a patch of ice and fell on his left shoulder, twisting his back.

Scrapping loose rock from highwall with front-end loader. The bucket slipped off a rock, hit another rock and slammed employee back into the seat causing the injury.

While changing a hose on the dozer, the bolt broke loose, causing him to hit his left elbow on the dozer hood.

While shoveling wet fly ash out of the bed of the truck, ee twisted the wrong way and pulled his back out.

While unloading an idler from the bed of Ford F-350 pick up truck the load shifted cutting his right thumb, resulting in stitches.

Walking on snow covered area, he slipped on an ice patch, as he tried to catch himself, he dislocated his ring finger on his right hand.

EE was operating the dozer when it caught fire. He tried to put out the fire with the fire extinguisher that was in the dozer. He breathed in smoke and dust from the extinguisher. He went to the hospital for x-ray as a precaution.

While lifting discharge hose he strained his lower back.

While lifting a concrete barrier with crane the injured reported that he may have hit the wrong control then over-corrected resulting in his right hand being crushed.

EE WAS WALKING IN PIT & DECIDED TO ROLL A LUMP OF COAL OFF THE 3' HIGH EDGE OF THE MINED AREA. IT ROLLED AGAINST HIS ANKLE CAUSING SOME DISCOMFORT IMMEDIATELY. EE TRIED TO WORK BUT ENDED UP MISSING TIME DURING THE COURSE OF THE NEXT MONTH DUE TO SWELLING.

EE STEPPED OUT ONTO LEFT FRONT FENDER OF CAT 992C LOADER WHICH HAS A FLAT WALK AREA. AREA WAS DRY, NO GREASE OR LOOSE DIRT WAS PRESENT. AS HE WAS RETURNING TO CAB, HE SIMPLY MISPLACED OR MISJU DGED HIS STEP AND FELL TO THE GROUND.
