While working on top of a Marion 8750 dragline tub an employee fell into a lower compartment injuring a rib.
When replacing the windshield on a front-end loader, the employee cut right index finger resulting in stitches.
A maintenance technician was working on the latch bar mechanism on the dipper of an electric rope shovel. A small chunk of dirt fell from the dipper door striking the individual which caused the individual to lose their balance and striking EE's mouth on the end of the crowbar. The impact caused a cut lip which required stitches.
Employee was installing a pin into the back of the dozer arm, the pin moved unexpectedly and pinched two fingers on left hand. This resulted in stitches and time away from work.
Employee was attempting to use an overhead crane to straighten a piece of metal on the bed of an overburden truck. When tension was applied to the crane hook it moved and pinched the employees thumb resulting in an injury.
While walking to EE's dozer employee tripped over EE's feet, hitting the bottom of the manlift with EE's left arm. The date of the incident the employees arm was x-rayed and no fracture was reported. On 8/31 an additional CT scan was performed, which now has reported a fracture.
While closing an access door a maintenance individual fell from a piece of mobile equipment and injured leg.
Employee was repositioning a sign and fell forward hitting/cutting EE's chin.
A Technician was working on crack repair in the revolving frame on a Marion 8750. While grinding, a small metal fragment contacted the employees right eye while wearing safety glasses and a welding hood.

Temporary employee was uncoiling a new tow-cable with a full-time employee. Cable end came up, striking temporary employee in the face. Employee received a chipped tooth and lacerations to outside and inside of lip, which did not require stitches. This occurred on the night shift and employee was outside.
An employee was cleaning mud and ice in the Shop with a Wrangler when the cutting edge got hooked on the rails that are embedded into the concrete floor. This caused the Wrangler to stop abruptly and the employee hit his lower lip on the steering wheel. The employee received a laceration that required stitches. The employee was wearing a seatbelt.
While trying to install a bushing into a right track frame on a Komatsu D475 track dozer a mechanic hit his thumb on his left hand with a soft faced hammer. This caused his thumb to smash against the bushing making a cut on the inside of his thumb.
A mechanic had climbed onto a track dozer to change fuel filters. He stepped from the dozer arm onto the track and slipped on the track. He grabbed the fender hand hold with his left arm. Momentum pulled at his left shoulder causing a strain.
An employee was walking around the front of a pickup when he slipped on ice and fell backwards. He landed on his back, left shoulder, and left elbow. Employee was taken to ER for physician evaluation.
A coal truck operator was waiting to get loaded by a loader. The loader dumped its first bucket into the truck bed which caused the truck operator to get bounced in the seat. Employee was taken in for physician evaluation.
A mechanic was installing a pin on a scraper hitch. The employee was using a sledge hammer to pound the pin into place behind him. When the hammer contacted the pin a piece of metal from the pin broke off and embedded into the mechanic's right leg. Employee was taken to ER but had to have surgery to remove the piece of metal from his leg.
One employee was holding a B&O tool in order to remove a wedge from a socket. Another employee hit the impact area on the B&O tool with a sledge hammer when a piece of metal chipped off of the B&O hit the employee, holding the tool, in the forehead. The employee was transported to ER where doctor removed the piece of metal.
An employee was getting 203 loading shovel's cable off of a cable tree. The rope between the tree and the shovel had tension on it. He stepped on the rope and then pulled on it to try and remove the clevis. This is when he heard and felt a pop in his right arm. Employee was transported to ER where he was diagnosed with a right bicep tendon tear.
I WAS UP ON LIFT UNWINDING SPRINGS TO CHANGE OUT SPROCKET ON OPENER. I WAS TAKING OUT BOLTS AND NUTS WITH IMPACT WRENCH AND BACKUP WRENCH, WHEN I TOOK OUT FIRST BOLT AND NUT SPRING BEARING PLATE SPUN AROUND SHAFT AND CAUGHT MY WRIST AND CUT IT.
The employee was standing between the tri-structure and fairlead tower. He was in the process of removing a piece of keystock from the back leg of the fairlead tower. The employee was using a 5 inch grinder and a 5 inch wheel to remove the tack welds that were holding down the keystock. During this process the grinder kicked back, and struck his right leg causing a laceration.
An employee was walking around the front of a front-end loader when he slipped on snow packed ice. He fell and landed on his right shoulder and arm. Employee was transported to ER where he was diagnosed with a rotator cuff strain.
