While using a pry bar to perform repairs on a bed of a pickup truck, the pry bar slipped and struck the employees lip resulting in an injury.

EMPLOYEE WAS STEPPING DOWN A STAIRS NOT FACING THE STAIRS AND WITHOUT THREE POINTS OF CONTACT WHEN EE LOST EE'S FOOTING AND SLID DOWN THE LAST THREE STEPS LANDING AWKWARDLY ON EE'S RIGHT ANKLE CAUSING A FRACTURE TO EE'S TIBIA ABOVE THE ANKLE.

While working on top of a Marion 8750 dragline tub an employee fell into a lower compartment injuring a rib.

A maintenance technician was working on the latch bar mechanism on the dipper of an electric rope shovel. A small chunk of dirt fell from the dipper door striking the individual which caused the individual to lose their balance and striking EE's mouth on the end of the crowbar. The impact caused a cut lip which required stitches.

Employee was installing a pin into the back of the dozer arm, the pin moved unexpectedly and pinched two fingers on left hand. This resulted in stitches and time away from work.

Employee was attempting to use an overhead crane to straighten a piece of metal on the bed of an overburden truck. When tension was applied to the crane hook it moved and pinched the employees thumb resulting in an injury.

While walking to EE's dozer employee tripped over EE's feet, hitting the bottom of the manlift with EE's left arm. The date of the incident the employees arm was x-rayed and no fracture was reported. On 8/31 an additional CT scan was performed, which now has reported a fracture.

While closing an access door a maintenance individual fell from a piece of mobile equipment and injured leg.

A Technician was working on crack repair in the revolving frame on a Marion 8750. While grinding, a small metal fragment contacted the employees right eye while wearing safety glasses and a welding hood.

Employee was kneeling down to inspect the latch on the shovel dipper. As employee was getting up employee felt pain in right leg. The employee has a fractured right fibula.

Employee was attempting to set a tumbler into position. The employee placed right hand in a pinch point between the tumbler and the brass sleeve. The contact from the tumbler being placed into position resulted in a laceration to EE's right hand middle finger that required sutures. The employee received a regular duty work release.

While an employee was conducting a walk around inspection of a 693 Overburden Truck, the employee slipped on packed snow injuring ankle.

Temporary employee was uncoiling a new tow-cable with a full-time employee. Cable end came up, striking temporary employee in the face. Employee received a chipped tooth and lacerations to outside and inside of lip, which did not require stitches. This occurred on the night shift and employee was outside.

An employee was cleaning mud and ice in the Shop with a Wrangler when the cutting edge got hooked on the rails that are embedded into the concrete floor. This caused the Wrangler to stop abruptly and the employee hit his lower lip on the steering wheel. The employee received a laceration that required stitches. The employee was wearing a seatbelt.

While trying to install a bushing into a right track frame on a Komatsu D475 track dozer a mechanic hit his thumb on his left hand with a soft faced hammer. This caused his thumb to smash against the bushing making a cut on the inside of his thumb.

A mechanic had climbed onto a track dozer to change fuel filters. He stepped from the dozer arm onto the track and slipped on the track. He grabbed the fender hand hold with his left arm. Momentum pulled at his left shoulder causing a strain.

An employee was walking around the front of a pickup when he slipped on ice and fell backwards. He landed on his back, left shoulder, and left elbow. Employee was taken to ER for physician evaluation.

A coal truck operator was waiting to get loaded by a loader. The loader dumped its first bucket into the truck bed which caused the truck operator to get bounced in the seat. Employee was taken in for physician evaluation.

A mechanic was installing a pin on a scraper hitch. The employee was using a sledge hammer to pound the pin into place behind him. When the hammer contacted the pin a piece of metal from the pin broke off and embedded into the mechanic's right leg. Employee was taken to ER but had to have surgery to remove the piece of metal from his leg.

One employee was holding a B&O tool in order to remove a wedge from a socket. Another employee hit the impact area on the B&O tool with a sledge hammer when a piece of metal chipped off of the B&O hit the employee, holding the tool, in the forehead. The employee was transported to ER where doctor removed the piece of metal.

An employee was getting 203 loading shovel's cable off of a cable tree. The rope between the tree and the shovel had tension on it. He stepped on the rope and then pulled on it to try and remove the clevis. This is when he heard and felt a pop in his right arm. Employee was transported to ER where he was diagnosed with a right bicep tendon tear.

I WAS UP ON LIFT UNWINDING SPRINGS TO CHANGE OUT SPROCKET ON OPENER. I WAS TAKING OUT BOLTS AND NUTS WITH IMPACT WRENCH AND BACKUP WRENCH, WHEN I TOOK OUT FIRST BOLT AND NUT SPRING BEARING PLATE SPUN AROUND SHAFT AND CAUGHT MY WRIST AND CUT IT.

The employee was standing between the tri-structure and fairlead tower. He was in the process of removing a piece of keystock from the back leg of the fairlead tower. The employee was using a 5 inch grinder and a 5 inch wheel to remove the tack welds that were holding down the keystock. During this process the grinder kicked back, and struck his right leg causing a laceration.

An employee was walking around the front of a front-end loader when he slipped on snow packed ice. He fell and landed on his right shoulder and arm. Employee was transported to ER where he was diagnosed with a rotator cuff strain.

An employee was on break when he felt something in his right eye and it started watering. The employee was previously putting culverts together but is unsure of when the metal particle entered his eye. The eye was examined by a mine site EMT but they could not get the metal particle out. The employee was taken to the Emergency Room to have his eye examined and particle removed.

Was exiting a Loading Shovel pinched fingers between the ladder and the tracks of the equipment. Was placing the ladder into its original position

Struck by flying object

Emp operating a 992 front-end loader was struck from behind by mud chunks which flew out of a sump. A 8750 dragline was filling in the sump located 450 feet away. A chunk flew thru the back window of the loader hit the head rest deflecting the chunk towards the emp's right shoulder/back area. The chunk then flew out the front window. The employee was transported to a hospital.

Employee was grinding welds between 2 safety posts that come together. The grinder jammed between the post and kicked back hitting the employee in the welding shield breaking the hood. The clear lens and plastic on the hood broke causing lacerations and scratches on the employee's nose. The employee was taken into a clinic where he received 6 stitches.

Emp noticed an eye irritation in his right eye on the evening of 2/3/12. The emp was fitting the top rim angle on a fuel tank earlier that day on a mine site. The emp did not seek medical attention until 2/6/12. An optometrist diagnosed that he had developed a scratch on his eye but was released to come back to work that day with prescription antibiotic eye drops.

Employee was fixing the filter on the coffee pot when the filter with the grounds in it and hot water spilled onto the employee's left hand causing a burn. Employee did not seek medical treatment until 10/27/11 when a large blister started forming on the top of his hand. Employee was seen by a doctor on 10/27/11 and diagnosed with a second degree burn.

Emp tripped while going thru a revolving frame of an 8750 drag line injuring his right bicep muscle. Emp was taken to ER where the doctor diagnosed him with a bicep rupture. Emp did receive a full return to work and was referred by the ER doctor to a specialist on 9/15. The specialist required a MRI on 9/20. On 9/26, emp received results of MRI indicating that surgery was needed.

An employee was pounding an 8 inch pin with a sledge hammer on an 8750 dragline when a piece of metal from the hammer broke off and hit another employee standing 10 feet away. The piece of metal lacerated the left chest/shoulder area on the employee.

Two mechanics were changing the axles in a boom truck when the differential slipped off of the jack landing on the steel toed portion of a mechanic's left foot. The mechanic was transported to the hospital where he was diagnosed with a toe fracture.

An employee was operating a scraper when he hit a frozen chunk of dirt and hit his head on the radio bracket above him causing an incision that needed stitches.

The operator of a 657E scraper experienced neck pain while push pulling with another scraper. The mine was informed by Workers Compensation on 10/28/2010 that employee was receiving chiropractic care.

A chain hook released causing a poly pipe to contact the emp's left leg. Minesite EMS examined to find a mild scrape-no immediate concern. 08/10 the emp asked to be seen by a DR. Mine was only informed from DR that the emp was taking antibiotics as precautionary. 09/10 mine was informed emp had seen DR multiple times for an infection.

Employee was pulling a new steel bearing with a redi-rod into the stick of a backhoe. The bearing was already into the bore of the stick..was into 3 inch of the stick with about 10 ton of force. A piece a thread flew off of the rod hitting the employee in the left check. Laceration which had to be removed.

Loosening a radiator cap, when it popped off, resulting in antifreeze going over his left hand.

Burn on left hand and face area. Employee was working with one other employee. They were trouble shooting disconnects when an arc flash occurred.

While Security officer was walking down the coal silo's catwalk his left foot slipped on the frosted grating. He caught himself from falling but twisted his right knee while doing so.

Security guard had come into the guard shack, tripped and missed a step and fell onto her knees. She extended her arms to break her fall. Her left arm got jammed in between the desk and a blue water cooler crate. Afterwards she felt no mobility in her arm and shoulder.

Employee was replacing a hose on an overburden truck when dirt from above started to fall onto his face. He turned his head and a plastic tie wrap on the hose slipped behind his safety glasses causing a cornea abrasion on his right eye.

Security officer tripped while descending stairs, loosing her footing and spraining her left ankle.

Emp was moving a loading shovel from one ramp to another. The shovel spun out on the slope causing the cable to tighten. He cut the rope that held the cable that was tied off to the shovel. When he cut the rope the cable whipped, knocking him to the ground. When he fell to the ground he landed on his right shoulder.

Employee got stung by a bee and had an allergic reaction. He administered an Epi-pen to himself and called his Supervisor and Dragline Maintenance Supervisor who activated the emergency response team on the minesite.

Employee was assisting with setup of a work platform. When climbing down, he missed the step or slipped backwards approximately 3 feet contacting a retangular guard on an 8750 dragline. This contact resulted in a multiple rib fracture to the employee.

Employee was working underneath a pickup when the transmission shifted rolling into the right shoulder and neck area pinning the employee.

Employee was filling the boiler system in the B Building at the Coal Handling Facility with a mix of antifreeze using a pump in a barrel. The system had just reached the proper pressure and he was going to disconnect the air hose from the pump when the antifreeze suddenly shot out of the pump and onto the left side of the employee's face and entered his left eye.

EE was working for a construction company that was remodeling offices in the office building at the Falkirk Mine. EE was lifting a bundle of 10' metal studs weighing approx 60lbs, when he felt a strain in his back, re-injuring a prior back injury.

A particle entered a contractor employee's right eye while he was on 104 dragline carbody located on Falkirk Mine. Employee was taken to clinic after minesite EMT irrigated the eye.

Employee was climbing down the ladder off of 101 dragline when she lost her footing and fell to her left, hitting her head on the floor of the dragline shoe. She received a laceration to her head. She was carrying a bucket of solvent in her left hand as she was going down, however she still contained three points of contact but lost her footing on the ladder.

Security Officer was opening door to Coal Handling Fire Suppression Pump House to conduct a routine inspection. Upon grabbing the door the Security Officer was stung three times on her left index finger by wasps. The Security Officer was aware she was allergic to bee/wasp stings and started to have an allergic reaction. The Security Officer was wearing cloth gloves.

The employee was climbing a ladder so that we could finish a welding project on a coal truck, when the bottom of the ladder slipped. The employee fell to the floor landing on his back and buttocks. The employee was treated at the minesite and taken to the hospital for an evaluation.

AFTER MEDICAL CONSULTATION IT WAS CONCLUDED THAT THE EE HAS A RUPTURE OFTHE BICEPS TENDON IN HIS LT. FOREARM WHICH WILL REQUIRE SURGERY TO REPAIR.
