SAND STONE AND SHALE ROCK STRUCTURE DID NOT BOND. 3 ROOF ABNORMALITIES ALSO EXISTED 2 KETTLE BOTTOM AND ONE SLIP. THESE ABNORMALITIES WEAKENED ANCHORAGE ZONE GREATLY CONTRIBUTING TO THE FALL A LSO A FACTOR WAS THE PRESENCE OF SOME WATER IN THE STRATA.
