Showing all 40Employee was pulling a water hose down a set of steps in the sample building when he slipped and fell hitting the steps with his butt. Due to complications he started missing work on August 14, 2007.
Employee was exiting the thickener site and five steps from the ground he slipped and fell on his rear and rolled to the ground causing a deep bruise to his tail bone.
Emp. was removing a pin from the rock vibrater by pulling with pipe wrench. The wrench slipped and caught finger against metal.
Employee was going to clean his back-up lights on his truck when mud fell from the bed and hit him in the face, going into his left eye.

Employee was using a chain hoist to lift a facing from a water pump and magnetite fell from the chain and into his eyes. First day of missing work was 9/9/05. Employee notified us on 9/16/05 that he had employment with another employer and that he had quit his job with us.
Employee was pulling check valve out of a hydraulic control valve on a 313 Excavator that had high pressure oil in it. When the valve was pulled, the pressure cut left index finger, which allowed oil to enter his finger. Went to ER but only was given an Rx, no stitches and was released back to work.
Employee was dumping a load of refuge, using a Cat. swivel truck, when his load shifted and the cab of the truck turned over hitting his head against the window. Seat belts were in use.
Employee was clearing an area for a new refuse site when he stepped on a rock that was covered with leaves and slipped and fell.
Employee was lifting an air compressor frame by means of a hoist truck and the cloth strap slipped and the frame caught middle finger on right hand between frame and side of locomotive.
Employee was in the rotor breaker coal chute prying on a piece of frozen coal material when it broke loose and fouled him against the side of the chute. Coal material was approximately 1 to 1.4 tons.
Employee stepped on a piece of grating that was over a drain and it flipped up and he fell approximately 16 inches into the drain channel.*The reason the form was late was the time involved from the injury date (3/27/2004) & first day missed (5/12/2004) was questionable and we waited on a doctor report to confirm the injury*.
EE WAS HELPING INSTALL A NEW SLIEVE ON A CHUTE WHEN THE PIECE OF METAL HE WAS HOLDING SLIPPED AND CAUGHT HIS FINGER BETWEEN ANOTHER PIECE OF METAL.
EMPLOYEE HAD SET HIS DINNER BUCKET ON THE REMAINS OF A SCRAPED ROCK TRUCK. HE GOT HIS BUCKET AND WHEN HE TURNED AROUND TO WALK HE RAN INTO THE MIRROR FRAME ON THE OFF SIDE DOOR. EMPLOYEE WAS W EARING HIS HARD SHELL CAP.
EE WAS CROSSING THE THICKENER RETURN LINE. INSTEAD OF CROSSING UNDER THE LINE HE WENT OVER THE TOP OF IT AND WHEN HE CAME OFF OF THE LINE HE TRUNED HIS RIGHT ANKLE. THE PIPE WAS APPROX. 40 INC HES HIGH.
EE WAS INSIDE OF THE ROTARY BREAKER PATCHING A HOLE AND LOST HIS BALANCE AND FELL.
EE WAS CLOSING THE ELEVATOR DOORS AND GOT HIS THUMB ON HIS LT. HAND CAUGHT BETWEEN THE DOORS.
EE WAS CLEANING OUT A SUMP TO SET A PUMP WHEN HE SLIPPED AND HIS RIGHT LEG WENT INTO THE SUMP HOLE.
EE WAS PRYING ON A COAL CHUTE WITH A SLATE BAR. THE SLATE BAR SLIPPED AND HE HIT HIS RIGHT ELBOW ON A PIECE OF METAL.
EE WAS ON A LADDER TAPEING A WATER LEAK WHEN THE LADDER SLIPPED AND THE EE FELL TO THE PLANT FLOOR. THIS TOOK PLACE ON THE FOURTH FLOOR NEAR THE DIESTER TABLE. HE FELL APPROX. EIGHT FEET.
ON THE AFTERNOON ON APRIL 21, 2003, EE INFORMED ME THAT HIS BACK WAS HURTING. HE WAS WORKING ON THE BRAKES OF A 350C PAYHOULDER DUMP TRUCK. WHEN ASKED IF HE WANTED TO FILE AN ACCIDENT REPORT, HE STATED THAT HE COULDN'T REMEMBER DOING ANYTHING SPECIFIC TO HAVE HURT HIS BACK & THAT IT WOULD BE OK. HE MISSED THE NEXT DAY OF WORK & WENT TO DR. MRI SHOWED RUPTERED & HERNIATED DISC IN BA
EE WAS CLEANING UP SPILLAGE AROUND A ROTARY BREAKER CHUTE WHEN HE GOT A PIECE OF COAL IN HIS RT.EYE. EE WAS NOT WEARING SAFETY GLASSES.
EE WAS HELPING ANOTHER EE SHUT THE ELEVATOR DOORAS THE TWO DOORS CAME TOGETHER THE EE GOT HIS HAND CAUGHT BETWEEN THE TWO DOORS.
EE WAS CHANGING SCREENS OUT ON A CLEAN COAL VIBRATOR. AS HE WAS HANDLING A SCREEN IT SLIPPED & CAUGHT HIS HAND BETWEEN THE SCREEN & THE FRAME OF THE VIBRATOR.
EE WAS LINING A CHUTE AT THE ROTOR BREAKER. AS HE WAS STANDING ON A BOARD THAT WAS ACROSS THE CHUTE THE BOARD SLIPPED OUT AND THE EE FELL AND SLID DOWN THE CHUTE APPROX. 6-7 FEET.
EE WAS PUTTING PACKING IN A CLEAN COAL PUMP ON THE BASEMENT FLOOR WHEN HE STEPPED IN A DRAINAGE HOLE THAT WAS APPROX. 8-10" DEEP AND 12-14" SQUARE.
THE ROTOR BREAKER HAD A BLOCKAGE AND WAS SPILLING OIL. EE WAS TRYING TO UNSTOP THE BLOCKAGE AND AS HE WAS GOUGING AT THE BLOCKAGE WITH A SHOVEL HE HIT HIS RIGHT KNEE ON A PIECE OF METAL.
EE WAS OPERATING A TRUCK WHICH WAS LOADED WITH SLUDGE. HE ROUNDED A SHARP TURN AND AS HE WAS PARTIALLY ON A SET OF SCALES, THE TRUCK STARTED SPINNING AND SLID SIDEWAYS. THE REAR WHEELS SLID OV ER A 3' DROP AND THE TRUCK TURNED OVER. THE WEATHER WAS SLEET, RAIN AND SNOW MIXED AT THE TIME OF THE ACCIDENT.
EE HAD A DRYER BASKET HANGING FROM A CHAIN HOIST AND AS HE WAS PUSHING IT INTO PLACE THE BASKET TWISTED AND FELL HITTING HIM ON THE SIDE OF HIS LEFT FOOT.
EE WAS MOVING A SET OF TORCHES THAT WERE MOUNTEDON A WHEELED DOLLY. AS HE BACKED UP HE STEPPED INTO A DRAIN THAT IS 16" SQUARE & ~1/2" DEEP AND FELL WITH THE TORCHES FALLING ON HIM.
EE STEPPED OUT OF HIS TRUCK ONTO A PLATFORM AT THE #1 MUD HOPPER. IT HAD BEEN RAINING & THE PLATFORM WAS MUDDY. AS HE STEPPED ONTO THE PLATFORM HIS FOOTING SLIPPED & HE TWISTED HIS RIGHT KNEE.
EMPLOYEE WAS HELPING CHANGE AN IMPELLER OUT ON A DELUTE PUMP. HE WAS HOLDING THE IMPELLER ON THE PUMP SHAFT AND IT STARTED FALLING OFF. HE PUSHED HARDER ON THE IMPELLER AS IT WAS COMING OFF AN D FELT A PAIN IN HIS LEFT SHOULDER AND ALSO HEARD A POPPING NOISE IN HIS SHOULDER.
EE HAD REPORTED FOR WORK AT THE LAB ANBD WHEN HE STEPPED OUT OF HIS VEHICLE HE TURNED HIS ANKLE.
EMPLOYEE HAD PLACED A LADDER IN THE VIBRATOR CHUTE TO DO SOME REPAIR. THE LADDER SLIPPED AND FELL LETTING THE EMPLOYEE FALL DOWN THE CHUTE.
EMPLOYEE WAS COMING OUT OF THE STOCKPILE TUNNEL AND WHEN HE STEPPED ONTO THE CATWALK, HE TURNED HIS LEFT ANKLE.
EMPLOYEE HAD CHANGED THE REFUGE FILTER OUT AND WAS CLIMBING DOWN WHEN HIS FEET SLIPPED AND HE PULLED OR JERKED HIS LOWER BACK. HE FELL APPROX 3-4 FT.
EMPLOYEE HAD CHANGED A TIRE OUT ON A AUTOCAR TRUCK. WHEN HE WENT TO PICK THE OLD TIRE UP, HE FELT A SHARP PAIN IN HIS LOWER BACK LEFT SIDE.
EMPLOYEE WAS ADJUSTING A SIEVE SCREEN AND AS HE PULLED IT DOWN HE DID NOT GET OUT OF THE WAY AND IT PUSHED HIM TO THE FLOOR.
THE EE WAS PUSHING SLUDGE BACK TOWARD THE MUD HOPPER TO KEEP IT FROM ENTERING A POND THAT WAS LOCATED TO HIS RIGHT WHEN HE BACKED UP TOO FAR & WENT INTO THE POND THE LOADER TURNED OVER ON I TS SIDE
