A ROOF FALL OCCURRED ON THE 002 SECTION-OUTBY FACE AREA IN THE #5 ENTRY. THE FALL MEASURED 200'WX 12'H. AREA HAS BEEN CRIBBED IN ALL LOCATIONS AND HAS BEEN CORDONED OFF.
THE EMPLOYEE WAS ROOF BOLTING TOP WHEN A PIECE OF DRAW ROCK FELL AND STRUCK HIM IN THE FACE AND LEFT SHOULDER.
THE EE WAS PUTTING UP A BOLT, AND THE BOLT BENT WENT HE BEGAN TO ROTATE IT, CAUSING THE BOLT TO STICK HIM ON THE LEFT ARM.
A ROCK FALL OCCURRED ON THE TRACK AT XCUT #100 OUTBY BOTH SECTION. THE ROOF FALL IS APPROX. 20' WIDE 80' LONG AND 4-8' THICK. EXTRA CRIBS HAVE BEEN INSTALLED IN THE AREAS AROUND THE FALL AND T HE FALL HAS BEEN REBOLTED AND CLEANED UP.
EMPLOYEE WAS BOLTING TOP WHENA PIECE OF DRAWROCKFELL AND STRUCK HIM IN THE NECK.
EMPLOYEE WAS DIPPING WATER OUT FROM BEHIND A TAILPIECE WITH A BUCKET AND HE STRAINED HIS BACK.
THE EE WAS WALKING ACROSS THE TRACK TO ANSWER THE MINE PHONE, AND HE SLIPPED AND FELL ON THE TRACK.
EMPLOYEE WAS PULLING ON A ROOF BOLT MACHINE TIREAND HIS FEET SLIPPED OUT FROM UNDER HIM. HE LANDED ON HIS BACK AND SOME CRIB BLOCKS.
THE EMPLOYEE STATED THAT HE WAS HANGING A MINER CABLE WHEN HE STRAINED HIS LOWER BACK.
THE EE SAID HE WAS WALKING DOWN #5 ENTRY TO PULLUP SOME SLACK IN THE WATERLINE WHEN A PIECE OF DRAW ROCK FELL &HIT HIM ON THE BACK &NECK
EE STATED THAT HE HAD HURT HIS ARM BACK IN JANUARY OF 199 AND DID NOT SAY ANYTHING ABOUT IT, BUT HAD WORKED WITH THE PAIN. AND HE SAID THAT HE REINJURED THE SHOULDER ON 3-30-99 WHILE TRYING TO HOLD ONTO A ROOF BOLT THAT WAS BEGINNING TO BEND AS HE TURNED ON THE ROTATION LEVER AND THE BOLT JERKED HIS ARM CAUSING INTENSE PAIN IN HIS SHOULDER.
THE ACCIDENT OCCURRED WHEN 2 RAILRUNNERS COLLIDED AT XCUT #42 WHERE THE TRACK SLOPED DOWN FOR APPROX. 10-15' THEN TURNS BACK UP. ON WAS ON THE WAY OUT, THE DAY SHIFT HAD WORKED LATE & 2 OF THE 2ND SHIFT EES HAD COME IN LATE. THE OTHER WAS COMING OUT & STARTED DOWN THE HILL ABOUT THE SAME TINME THE ONE GOING IN DID ON THE OTHER SIDE & BH THE TIME THE OPERATORS COULD SEE EACH OTHER,

A ROOF FALL WAS FOUND AT XCUT #27 IN #1 AND 2 ENTRY GOING TO 002 SECTION IN THE RETURN ENTRIES. IT IS APPROX. 80' LONG, 20' WIDE, AND 7' HIGH. THE RETURN AND THE ESCAPEWAY HAS BEEN REROUTED AR OUND THE FALL AND NO PLANS ARE MADE TO CLEAN IT UP.
A ROCK FALL WAS FOUND AT CROSS CUT 185 IN TEH NUTRAL ENTRY INBY 10 HEAD DRIVE. THE FALL IS IN AN AREA NOT BEING USED SO WE WILL NOT NEED TO CLEAN IT UP. THE FALL IS APPROX 8'TX18'LX18'W.
THE HIGHWALL FELL OFF OUTSIDE HAZ-4-WEST MINE BETWEEN THE #3 AND 5 ENTRY. IT DAMAGED THE ROCK DUST BUILDING AND BLOWED THREE TRANSFORMERS AND THE PHONE SYSTEM. THE FALL IS APPROX.100' LONG.
A ROOF FALL OCCURRED ON #6 BELTLINE AT CROSS CUT 142 OUTBY 001 SECTION. THE ROOF FALL IS APPROX 19'WX22'LX8'T. EXTRA CRIBS HAVE BEEN INSTALLED IN THE AREAS AROUND THE FALL AND THE FALL HAS B EEN REBOLTED AND CLEANED UP.
THE EMPLOYEE WAS RIDING IN THE FRONT OF THE RAILRUNNER AND WAS SETTING ON HIS LEG. THE EMPLOYEE PUT HIS HAND ON THE BUMPER AND RAISED HIMSELF UP TO MOVE HIS LEG FROM UNDER HIM WHEN THE RAIL RU NNER HIT A BENT PLACE IN THE TRACK CAUSING THE RAIL RUNNER TO SHIFT TO ONE SIDE. THE EMPLOYEE'S HAND SLIPPED OFF THE BUMPER & HE FELL OVER INTO THE FRONT OF THE RAIL RUNNER & IT RAN OVER HIM.
A ROOF FALL OCCURRED IN THE #5 ENTRY AT CROSS CUT 145. THE FALL IS APPROX 18'WIDE X 20'LONG X 10'HIGH. THE AREA HAS BEEN CRIBED AROUND THE AFFECTED ARE AND NO PLANS ARE BEING MADE AT THIS TIME TO CLEAN UP THE FALL.
SEVERAL AREAS IN THE MINE THE TOP HAS CONTINUED TO FALL OUT AROUND THE BOLTS. AND ARE NEW ROCK FALLS. THE AREAS DOWN THE RETURN ARE XCUTS 109 AND 111 IN #2 ENTRY AND 111 AND 112 IN #1 ENTRY. A N INTAKE SIDE IS 55, 59, 61, 62, 67 #5 ENTRY AND 87, 91 IN #6 ENTRY. THE AREAS AROUND THE FALLS ARE NOW BEING TIMBERED AND CRIBBED.
EE WAS BUILDING A BRATTICE AND HAD IT ABOUT 6' HIGH WHEN A CONCRETE BLOCK FELL OFF THE TOP AND HIT HIM ON THE LEFT FOOT.
A ROOF FALL OCCURRED IN THE 4 BELT ENTRY AT APPROX 147 CROSS CUT OUTBY 001 SECTION. THE ROOF FALL IS APPROX. 20'W40'L12'H. THE ROOF IN THE AREA IS LAMINATED SLATE AND SANDROCK IT ALSO HAS TWO OR THREE SMALL COAL SEAM APPROX. 6 TO 8 FEET UP.THE FALL IS NOW CLEANED UP AND CRIBS AND TIMBERS HAVE BEEN INSTALLED IN THE OTHER INTERSECTION IN THE AREA.
EMPLOYEE WAS HELPING REPAIR ON THE DOUBLE HEAD ROOF BOLTING MACHINE AND WAS BENT OVER HELPING REPLACE A BLOWER MOTOR BELT WHEN A PIECE OF DRAW ROCK FELL AND HIT HIM IN THE LOWER BACK. (ROCK SI ZE 1/2 TO 2" THICK AND 6 TO 12" LONG.)
ON MONDAY AFTERNOON THE TOP ON 001 SECTION BEGIN TO RIDE IN STARTING IN 31 ENTRY COMING ACROSS THE SECTION. WE WERE UNABLE TO STOP IT AND BEGIN TO MOVE THE EQUIPMENT OFF OF THE PANEL. WE GOT A LL THE EQUIPMENT OUT EXCEPT THE FEEDER AND THE SECTION TAIL PIECE. THE PANEL NOW IS COMPLETELY FELL IN. WE DO NOT PLANT ON TRYING TO RECOVER THE FEEDER AND ARE GOING TO SEAL THE 2ND LEFT SUBM
A ROOF FALL WAS FOUND IN THE TRACK ENTRY AT CROSS CUT #81 IN #6 ENTRY BY THE PRE SHIFT EXAM - IN THE HAZ-4-WEST MINE. IT IS APPROX 20'WIDE, 10'HIGH, 50'LONG. THE ROOF FALL WAS FOUND AT APPROX 8:50 PM SUNDAY NIGHT.
THE EE WAS STANDING ON A 5 GAL BUCKET TRYING TO HAND A WIND CURTAIN WHEN THE BUCKET FLIPPED OUT FROM UNDER HIM AND HE LANDED ON HIS BACK AND HEAD.
THE EE WAS STANDING BESIDE THE TAIL PIECE WATCHING FOR BAD SPLICES IN THE BELT. HE HAD HIS HAND LAYING ON TOP OF THE SPILL BOARD WHEN A SPLICE CAME THREW AND HIT THE SPILL BOARD CAUSING IT TO JAM INTO THE TOP. IT CUT THE PINKY FINGER OFF ONHIS LEFT HAND JUST BEHIND THE FINGERNAIL AND THERING FINGER 1/2 WAY BETWEEN THE 1ST & 2ND JOINT.
EMPLOYEE WAS WALKING DOWN TO THE END OF THE TRACK WHEN HE STEPPED ON SOME LOOSE ROCK AND TWISTEDHIS ANKLE.
A ROOF FALL WAS FOUND AT THE 53RD BREAK IN #2 ENTRY OF THEMAINS. IT IS IN THE RETURN ESCAPEWAY. THE ESCAPEWAY WILL BE ROUTED AROUND THE FALL DOWN #1 ENTRY. WE DO NOT PLAN TO CLEAN UP THE FALL. THE AREA IS BEING CRIBBED AND DANGERED OFF. THE FALL IS APPROX 20' WIDE AND 20' LONG.
EMPLOYEE WAS BOLTING TOP AND HAD DRILLED A HOLE AND LEFT THE DRILL STEEL HANGING IN THE HOLE. THE EMPLOYEE LOWERED THE DRILL BOOM AND LAYED HIS HAND OVER THE SUCTION HOLE TO TALK TO HIS BUDDY WHEN THE DRILL STEEL FELL OUT OF THE HOLE AND MASHED HIS LEFT RING FINGER.
EE WAS BOLTING IN IN #1 HEADING AND WAS DRILLINGA HOLE WHEN HE TURNED TOGET ANOTHER PIECE OF DRILL STEEL AND FELT SOMETHING POP IN HIS LOWER BACK. (TWISTED BACK)
EE WAS TRYING TO PUT A BENT ROOF BOLT IN THE TOP AND WHEN HE TURNED ON THE ROTATION THE BENT PART OF THE BOLT HIT HIM ON THE LEFT ARM AND BROKE IT.
LIFTED A BOX OF 4' GLUE TO PUT IT ON THE ROOF BOLTING MACHINE AND STRAINED A MUSCLE IN HIS LOWER BACK.
THE CONTINOUS MINER CUT INTO A CAVITY ( OR INUNDATION) IN THE #7 ENTRY ON 001 SECTION OF THE HAZ-4-WEST MINE. THE CAVITY MEASURED 2 FT WIDE AND THE FULL HEIGHT OF THE COAL SEAM (6') THICK AND 20 FT WIDE. THERE WAS 4 TO 5% CH4 DETECTED AT THE TIME. THE POWER WAS CUT OFF AND THE AREA VENTILATED. NO ONE WAS INJURED.
CONTINUOUS MINER OPERATOR WAS TURNING A RIGHT BREAK IN THE #5 ENTRY WHEN A HORSEBACK FELL OUT OFTOP ON CONTINUOUS MINER. HORSEBACK WAS APPROX 33'L, 4 1/2' THICK, 12'W. NO ONE WAS INJURED IN TH IS FALL.
A ROOF FALL OCCURRED ON 002 SECTION IN #2 ENTRY IN THE LAST OPEN X-CUT AND EXTENDED IN BY TOWARDTHE FACE APPROX. 10 FT. A DUAL HEAD FLETCHER ROOF BOLTER WAS COVERED UP IN THE FALL. NO ONE WAS INJURED IN THE FALL. (SECTION HAS BEEN MOVED).
EE WAS STANDING IN FRONT OF THE SCOOP WHEN A PIECE OF DRAW ROCK FELL OUT BETWEEN THE BOLTS AND HIT HIM ON THE LEFT FOOT.
THE SECTION FOREMAN WAS WALKING THE RETURN ENTRY OUT WHEN HE FOUND A ROCK FALL AT THE 106 BREAK, #1 ENTRY IN THE RETURN. THE FALL IS IN AN AREA WHERE IT DID NOT AFFECT THE VENTILATION OR MININ G DEVELOPEMENT AND WILL NOT BE CLEANED UP. PROPSETTERS AND CRIBS HAVE BEEN INSTALLED AROUND THE FALL.
A ROCK FALL OCCURED AT THE 8TH BREAK ON 2ND LEFT SUB MAINS GOING TOWARD 002 SECTION. THE FALL IS APRPOX. 50' LONG 18' WIDE AND 6 FT HIGH. NO ONE WAS INJURED OR VENTILATION WAS NOT INTERUPED.
EMP WAS DRIVING A 10SC SHUTTLE CAR WHEN HE STARTED HAVING BAD CHEST PAINS. WAS TAKEN TO HOSPITAL AND WAS TOLD HE HAD A BAD HEART ATTACK.
MANTRIP WAS ON ITS WAY TO THE SECTION WHEN THEY STOPPED TO FIX A BROKEN WATER LINE. WHEN MEN RELOADED TO CONTINUE ON ONE MNA GOT ON TOP OF THE BATTERIES CAUSING SEVERAL BROKEN BONES.
