Fell off the rock truck while changing air filters.

HE SLIPPED OFF BACK BUMPER OF MECHANIC'S TRUCK WHILE REMOVING MATERIALS. HE LANDED FLAT ON BOTH FEET AND PAIN SHOT DOWN BOTH LEGS.

EE WAS CHANGING DRILL BIT ON RIG WITH WRENCH WHEN WRENCH FLEW BACK AND HIT HIM ON THE RIGHT WRIST.

EE WAS GETTING OFF FRONT END LOADER & EITHER SLIPPED OR PARTLY MISSED THE STEP. THE COAL SURFACE WAS DAMP. EE FELL & SPRAINED HIS BACK.

EE WAS WALKING IN PIT & DECIDED TO ROLL A LUMP OF COAL OFF THE 3' HIGH EDGE OF THE MINED AREA. IT ROLLED AGAINST HIS ANKLE CAUSING SOME DISCOMFORT IMMEDIATELY. EE TRIED TO WORK BUT ENDED UP MISSING TIME DURING THE COURSE OF THE NEXT MONTH DUE TO SWELLING.

EMPLOYEE WAS CHANGING A DRILL BIT AND LACERATED HIS LEFT INDEX AND MIDDLE FINGER.

EE STEPPED OUT ONTO LEFT FRONT FENDER OF CAT 992C LOADER WHICH HAS A FLAT WALK AREA. AREA WAS DRY, NO GREASE OR LOOSE DIRT WAS PRESENT. AS HE WAS RETURNING TO CAB, HE SIMPLY MISPLACED OR MISJU DGED HIS STEP AND FELL TO THE GROUND.

WHIOLE WALKING UP EMBANKMENT TO PULL WINCH CABLE, EMPLOYEE SLIPPED AND FELL CAUSING INJURY TO HIS LOWER BACK.
