Employee was terminated on November 17, 2012 and filed a hearing loss claim. He was diagnosed on 2/26/14 with work related noise exposure. Company Safety Department was not notified until April 10, 2014 of diagnosis.

777 Cat truck was loaded in pit, ground was soft and when truck started to pull out, front of truck raised up then came down hard and jarred operator, hurting neck and back.

EE was repairing a rear spring on a F250 pick up truck when the spring fell & struck his left index finger causing a laceration. He rec'd 8 stitches.
Employee was cutting out a blade liner on a D-11R dozer. The liner broke loose pushing him to the ground. His foot was under the blade and his foot was fouled when he began to move causing a break to his lower left leg (blade was on the ground; the ground was uneven where the work was being done).
Mechanic was working under piece of equipment when another mechanic tossed him a bolt that ricocheted and hit him in the mouth, knocking several teeth loose, breaking one tooth and causing damage to his lip and outer mouth.
Employee was pushing in a shot. When he started to back up a rock came from the spoil and he backed up on it. When he started to tram off, the dozer slipped off causing a jolting action resulting in lower back pain. He continued to work until 10/21/06 at which time he bagan to miss work.

EE NOTIFIED THIS OFFICE THAT HE WAS SEEKING MEDICAL TREATMENT DUE TO HIS EXPERIENCE ON 10/30/02.HE EXPERIENCED ABDOMINAL PAIN THAT ROTATED TO HIS BACK. EE STARTED MISSING TIME ON 11/25/02, HE WAS TO RTW ON 12/3/02. EE DID NOT RETURN TO WORK& HIS CLAIM IS STILL UNDER INVESTIGATION TO DETERMINE IF WORK RELATED. EE WAS DIAGNOSED WITH A MILD BULGING DISC & DEG. DISC DISEASE.
