EMPLOYEE WAS HAULING REFUSE FROM APPOLO FUELS PREP PLANT. WHEN HE GOT REFUSE DUMP HE BACKED TRUCK UP TO FAR AND GROUND GAVE AWAY WITH TRUCK CAUSING IT TO LAY OVER ON IT'S RIGHT SIDE. THERE W AS NO REASON FOR EMPLOYEE TO BACK THE TRUCK UP THIS FAR SINCE THERE WAS A DOZER WORKING THE DUMP AREA. CARELESNESS ON DRIVER'S PART: HE COMPLAINED OF HIS LEFT HIP HURTING HIM AT THE TIME.
