Employee was removing a block wall that was used to store mud behind it in order to be dried and later removed from the mine via Scoop. As he started to remove blocks some mud came loose thus pushing employee's leg against the blade of the Scoop.

Employee was installing pin on a bridge when he caught finger under the pin. Employee did not miss any work however he did go to a medical facility to get an x-ray done and it was determined that it was fractured therefore making this accident a reportable.

Employee was drilling hole on a motor base when the drill bit jammed in hole. He reversed the drill and while pulling back out on the drill the drill struck employee's mouth thus breaking his front teeth. Employee is getting necessary dental work done.

Employee had lowered the head of the roof bolter to remove drill steels. The finisher steel had stuck in the roof. As the employee was putting the pusher steel away, the finisher steel fell out of the roof, striking the employee on the ring finger of his left hand.

While employee was attempting to use the drill head of the roof bolter to move coal laying in the bottom in order to obtain clearance for the second set of drill steels, the canopy swung, thus catching the webbing of his left hand between the canopy and drill steels that he was holding on to.

Over the road truck driver pulled in to get loaded with coal. Driver was doing his truck inspection. He bent over to check air hoses. When he stood back up he got dizzy, lost balance, fell and hurt his ankle.

Employee pinched hand between the head of roof bolter and roof bolt plate. He missed one (1) day due to this injury. This occurred over the Thanksgiving/deer season holiday of which we were idle. We returned back to work on November 28, 2007. His day missed was on Friday, November 23, 2007 only.

Employee continued working until February 28, 2008 at which time surgery was performed to repair a meniscus tear of the right knee now making this a reportable.

Employee was checking breaker in Motor Control Center Building. As he went to step up on a bucket with screwdriver in right hand he apparently lost his balance and as he went to catch himself he accidently contacted the top side of breaker with screwdriver thus receiving flash burns.

Employee was riding on a personnel carrier directly behind the operators compartment when apparently his cap lamp cord caught a roof bolt thus pulling his hard hat off his head. As he went to duck down his left shoulder struck the roof and rolled his body back over the fender of the personnel carrier. Employee did not miss any work however he had been placed on restricted duty.

The working supervisor was observing the Continuous Miner Operator in the No. 2 entry, when a piece of rock dislodged between the roof bolt and rib line, striking employee on the head forcing his hard hat off thus causing a head laceration and jamming neck. Employee required staples to close laceration.

While performing his normal duties as a maintenance supervisor over a period of time the employee experienced pain/numbness in both hands. Employee was later diagnosed with carpal tunnel syndrome. He continued to work until 09/21/2006 at which time he went off to have surgery performed for the right hand and will have the left hand done at a later date.
