Employee stated on 5/6/25 to co-worker that EE had injured disk in EE's neck and back prior to coming to work for EDM No. 1 Mine. EE finished shift and talked to several people at the mine prior to leaving mine site. EE has not reported accident to management so time of accident and nature of injury has not been determined.

Employee was lying on employee's side changing a torque shaft on the continuous miner when a large rock slid off the miner and pinned employee between rock and frame of the miner.

Employee was on EE's knees operating the continuous miner when a piece of draw rock measuring 2'x2'x2" fell striking employee on EE's left foot

When employee released the brake on the rail mantrip, the mantrip began to roll forward. Employee panicked and jumped off. EE slipped and struck EE's left shoulder on rib/brow.

Death on mine property - A security guard (employed by Squeeze Security) suffered a fatal heart attack while working at the surface area of an underground mine.

Employee was operating a shuttle car. While being loaded, a rock fowled in the discharge end of the continuous miner conveyor chain. When the rock came loose, it was thrown toward employee, striking them on head behind right ear.

Employee was operating a shuttle car when a piece of rock was thrown from the continuous miner boom and struck employee on face and forehead.

A roof fall measuring approximately 55 inches high, 20 feet wide and 40 feet long, fell in crosscut between no. 4 and no. 5 entry near spad no. 2001. (The fall has been dangered off and will not be cleaned up)

Employee was loading roof bolt supplies onto the roof bolter when a co-worker threw a bundle of roof bolt plates and caught employee's finger between the plates and the roof bolter.

Employee stated that while walking in the No. 4 entry, EE slipped and fell causing EE to strike EE's shoulder on a rock. Employee did not return to work after scheduled 2 week vacation.

Employee was trying to remove a piece of steel that was stuck in the drill chuck. EE's hand slid up the drill steel and across a steel burr on the drill steel lacerating EE's left hand.

Employee was preparing to make a belt splice and while cutting the belt with a knife, EE slipped allowing the knife to cut EE's thumb.

A roof fall occurred while mining the No. 1 lift off the right wing of the No. 6 Pillar block on the mains section. The material caught the continuous miner on the left side and caused the continuous miner to be stuck. The continuous miner was pulled out from under the rock by using a separate machine.

Employee was operating a continuous miner when a piece of rock fell out between roof bolts striking employee.

Employee was shoveling coal onto the belt conveyor when EE felt a pain in EE's left shoulder.

Employee was operating a shuttle car and was being loaded by the continuous miner when a piece of rock being discharged by the miner struck the employee on EE's lip.

Employee was tramming a forklift when EE drove off the road into the ditch. The forklift leaned on its side against the berm. As the employee was getting out of the forklift EE slipped and rolled over the embankment approximately 15 feet.

While pushing a miner drum out of the scoop bucket, the employee stated that EE laid EE's arm on top of the back rest and as the scoop drifted back, EE's arm slipped of the back rest allowing EE's arm to get caught between the backrest and the coal rib.

Employee was removing a roof bolter tire from the rim and while striking the rim with a sledge hammer, the hammer came back and struck employee on employee's left arm.

Employee was operating a scoop and ran over a rock. This caused the employee to be jarred inside the operating deck of the scoop.

Employee was unloading timbers and one of the timbers fell on employee's foot.

Employee was using a cut off saw when the salo disk/blade broke, a piece of the fragment from the blade struck the employee on the finger.

Employee was using a hammer to remove a bearing from a shaft, when EE struck the bearing, a piece of metal flew back and struck EE on the left eye (Employee was wearing safety glasses at the time).

Employee was shoveling the belt when felt a muscle pull in upper back.

Employee was tramming the roof bolter past a parked shuttle car. The bolter slid and caught employee's leg between the bolter and the shuttle car bumper.

While moving a scoop battery charger to a new location, employee was loading cable in scoop bucket, when he was moving away from the scoop, he slipped and his foot went under the scoop bucket as the scoop was moving away

Employee was operating a roof bolting machine when he got his hand caught between the drill head and the drill steel.

Employee was hanging the continuous miner cable when he felt something start burning in his lower back.

Employee was opening airlock door to allow a scoop to travel through when the supply car being pulled by the scoop struck the door causing it to strike employees leg.

A co-worker was setting a 7 foot timber, when he loss control of the timber it fell on the left foot of this employee.

EE stated that while working on conveyor belt in the track entry, he slipped in the mud and fell on the track.

While working on track, the EE dropped a rail and the rail kicked back and struck the EE on the left foot.

Employee was operating a scoop when a small piece of draw rock fell form the roof/rib and struck employee on the upper back.

Employee was shoveling under the belt conveyor when he felt something pull in his shoulder.

While unloading cinder blocks from scoop bucket, employee dropped one of the blocks on his right knee.

EE was installing line curtain using a Poco Stick. The Poco Stick slid backwards and dislodged a piece of draw rock that fell and struck EE on the forehead and eye.

A 10' timber fell over and struck the employee in the lower back.

EE stated that while working near the #3 belt head he slipped on a piece of belt and twisted his ankle.

Employee was driving inserts into the bit lugs on the continuous miner when a piece of the hammer he was using broke off - flew back and struck employee on the lip.

EE stated that a piece of the rib rolled off and struck him on the foot.

EE was preparing to cross over the pinner cable when the cable flew up and struck him on the right side of his neck.

Employee was operating the continuous miner when a piece of draw rock fell striking employee on head and shoulder.

EE WAS INSTALLING BOLTS ON HELPER SIDE OF A DD015 FLETCHER BOLTER IN THE #2 FACE. AS HE WAS PUSHING UP THE BOLT, A LOOSE PIECE OF DRAW BROKE AROUND THE BOLT (APPROX SIZE 1' X 6" X 3"), FELL AND STRUCK EE ON HIS HAND. BROKE EE'S RING FINGER.

Rock falling hit him on top of right foot. Witness stated he was on the open side when this happened and when he got to him he had foot out under rock.

ROOF BOLTER OPERATING HELPER SIDE OF THE DOUBLE HEAD FLETCHER. WAS INSTALLING HIS SECOND BOLT IN THE FIRST ROW IN THE #7 ENTRY WHEN A LOOSE ROCK MEASURING 5' X 5' X 0 TO 5" THICK BROKE OFF AT THE EDGE OF THE ATRS AND FELL STRIKING OPERATOR ON THE RIGHT LOWER LEG.

MINE HAS BEEN IDLE FOR 2 1/2 YEARS AND IS PRESENTLY UNDER GOING REHABILITATION WORK. ROOF FALL WAS DISCOVERED DURING THIS TIME. THE FALL WAS LOCATED AT BK #22, 1BK INBY SPAD #145 BETWEEN NEUT RAL & LEFT RETURN. FALL IS APPROX. 20'LONG 0 TO 5 1/2' THICK, 18'WIDE. #3 ENTRY.

MINE HAS BEEN IDLE FOR 2 1/2 YEARS & IS PRESENTLY UNDERGOING REHABILITATION WORK. ROOF WAS DISCOVERED DURING THIS TIME. FALL LOCATED BK. #23 NEAR SPAD #163. 18'WIDE, 0-5'THICK, & 40'LONG. FELL IN BK. BETWEEN BELT AND NATURAL ENTRY.

MINE HAS BEEN IDLE FOR 2 1/2 YEARS AND IS PRESENTLY UNDERGOING REHABILITATION WORK. ROOF FALL WAS DISCOVERED DURING THIS TIME. FALL LOCATION BK #19 IN #2 ENTRY. APPROX. 60'LONG, 0-5'THICK AND 18'WIDE. APPROX. 7000'FROM FACE AREA.
