Employee was unloading supplies from scoop to roof bolter. While loading roof plates, stack fell over & sharp edge cut middle finger & ring finger on left hand. Went to doctor & cleaned up cut - no sutures.
EE was unloading scoop outside that had water line on it, when he stepped on a piece of hiline cable laying on ground-twisted his right ankle. Went to dr an dmissed 1 shift.
EE, general labor. Foot slipped on gob or rock & he twisted his right knee. Went to Dr & he is going to run an MRI to check knee. Doesn't know extent of injury or when he will return to work.
An unintentional roof fall occured in # 6 Entry at break 18, spad # 66. No people or equipment involved. Fall is approximately 12' wide X 5' thick X 25' long. Fall is in intake entry but did not occur in escapeway. Fall will be timbered off and not cleaned up. Fall is 2500' from section.
Employee, general labor, was outside in supply yard loading timbers in scoop bucket when he slipped and fell and hit his tailbone on scoop bucket. Went to doctor - bruised - will be off couple of shifts.
An unintentional roof fall above anchorage occurred in #3 entry BK#16 Spad #87. Personnell or equipment were not involved. Fall did not block air course or escapeway. Nor does it block the passage of employees. Fall has been breakered off. Fall was approx 6' thick, 20'wide 40' long
Employee was helping wind up belt rubber in roadway, hit his arm against guard and cut his forearm - small cut to lower forearm. He went to hospital, received first aid treatment (no sutures); missed one shift.
Beltman was cleaning gob out at tail area, had piece of drill steel prying on rock - hit bar against eye - resulting in laceration to head over right eye - went to hospital & got 5 sutures. Returned to work 6-12-06.
EE was bolting #3 entry when he stepped on piece of rock on bottom & left knee popped - resulting in strain to knee - missed 2 shifts - returned to work on 6-10-06.
Employee was helping set feeder, picked up a crib block that had a nail in it and punctured his right wrist. Got 2 sutures in wrist but missed no work.
At 11:40 am a roof fall ocurred outby section approximately 1100' (outby section) in # 7 intake entry. Fall is 20' wide, 20' long and 8' thick. No one was in thia area. No injuries or equipment involved. Area will be cribbed off and left. Section will be moved to another area in this mine.
EE was loading shuttle car reached up to prevent coal from coming in deck & cut finger on a piece of metal on shuttle car. 4 stitches no lost time.
REACHING TO CUT METAL BAND ON KENNEDY STOPPING. WHILE DOING SO THERE WAS ONE ALREADY BROKE JABBING IN THE TOP OF HIS RT. HAND. REQUIRED STITCHES TO STOP BLEEDING. INJURY OCCURRED IN THE #4 ENTRY IN THE FACE.
EMPLOYEE WAS USING TORCH TO CUT LINK OUT OF CONVEYOR ON FEEDER WHERE THE HOSE CONNECTS TO THE HANDLE. THE HOSE RUPTURED & BURNT HIS LEFT ARM. EE WENT TO THE HOSPITAL & WAS TREATED & RELEASED. EE WORKED HIS NEXT SHIFTS ON FRIDAY & SUNDAY & THEN WAS OFF DUE TO THE INJURY, HE PLANS TO RETURN TO WORK ON 4/3/06.
EE is a Shuttle Car Operator was helping hang miner cable at poewer center, hurt back while lifting cable, went to hospital was treated and released for back strain missed one shift & returned to work on 3/17/06.
Employee was helping make a belt move, lifting belt with a bar & putting structure in belt when he felt pain in lower right side of back. Went to dr had strain. He came back to work on 2-20-06, worked 4 hours and left again - will be off a few days.
Employee was helping rock dust, standing by scoop when operator went to move scoop and backed over his foot. He went to the hospital; big toe was broken.
Roof fall occurred in #1 return entry at spad #89 approximately 350' outby section. Fall is 3-5' thick, 16' wide & 23' long. No injuries/equipment involved. Area was fully bolted with 4' resin bolts. Area that fell resembles a horse-back slip. Fall will not be cleaned up - has been breakered off.
EE was mining in 4rt when a piece of draw rock fell out between bolts & struck him on left hand. Went to hospital and has small bone fracture on left hand. Rock was 3'x1'9" x 2" thick.
Bolter operator was loading bolt supplies on bolter when other bolter operator trammed bolter forward - ran wheel up on metatarsal boot on left foot, bruising foot.
Ee electrican was helping install belt head outside, when he slipped on a metal rail & fell striking left forearm against a piece of expanded metal-causing laceration to left forearm recieved 12 sutures.
EE was plastering stopping by hand, when he splashed a little in right eye-causing irritation to eyes. Went to dr and got first aid treatment-he was off 2 days.
A ROOF FALL OCCURRED IN THIS MINE AT APPROX. 5:10 PM IN THE #3 ENTRY (MAINS), ABOUT 10'INBY SPAD #19. THIS FALL TRAPPED THE ROOF BOLT MACHINE BUT NO ONE WAS INJURED. FALL IS APPROX. 90'L X 10-15'W X 6'THICK. PLAN HAS BEEN SUBMITTED TO CLEAN UP THIS FALL.
Employee was loading headers on roof bolter, slipped and fell and hurt his right wrist. He went to doctor; had wrist sprain. Was off two days.
Employee was using a utility knife to cut plastic tape off a belt chain when he cut his right index finger. He went to the doctor & got 4 sutures. Returned to work next scheduled shift - no lost time.
EE was helping load an old piece of hi-line to get it out of way. Hurt lower back while handling cable - was loading it in an empty car. Went to doctor and was off work 2 days. Returned to work on 2-27-05.
EE WAS HELPING SET TIMBERS ON PILLAR LINE. WHILE LIFTING 10' TIMBERS, EE FELT PAIN IN LOWER BACK, WENT TO DOCTOR OVER WEEKEND, WAS TOLD TO TAKE COUPLE OF WEEKS OFF WORK
Employee had turned scoop on charge and was walking a couple of breaks to help with belt reduction when he stepped in a hole and twisted his ankle. Missed one shift; returned to work 2/13/05.
EMPLOYEE WAS TRAMMING FEEDER TO SET ON BELT TAIL, AFTER MAKING A BELT REDUCTION. WHILE TRAMMING FEEDER ON A SMALL GRADE HE PINNED HIMSELF AGAINST THE RIB, HITTING HIS RT. FOOT AGAINST THE FEEDER FRAME. HE WENT TO THE DR., CONTUSION, & EE WAS OFF FOR 3 DAYS.
EE was helping set breaker timbers on north mains when a rib fell and hit him on right knee, he worked until 3:00 pm and walked outside, reported swelling in right knee, undetermined at this time as to extent of injury.
Employee scooping gob in order to get to pillars to mine. While scooping, a piece of 3" plastic water line sprang & hit him in face resulting in cuts & bruises to nose & lips. He was taken to hospital, treated & released. Also knocked out 1 tooth.
EE WAS HELPING REPLACE A 20' PIECE OF BELT IN #2 MAIN LINE BELT. WHILE WORKING, HE SLIPPED & FELL AGAINST BELT ROPE HURTING HIS RIBS. WENT TO DOCTOR & TOLD TO TAKE A COUPLE OF DAYS OFF - HE EXPECTS TO RETURN IN COUPLE OF DAYS.
EE had just gotten to section, went over to belt entry, slipped and fell against section belt tail piece, hitting his ribs against a piece of metal causing a fracture to ribs, finished shift before going to Dr, was off total of 6 days.
EE WAS HELPING LOAD HI-LINE CABLE IN A FLATCAR, TO BE MOVED, WHEN HE GOT HIS FOOT CAUGHT BETWEEN AN OLD PIECE OF BELT RUBBER LAYING IN ROADWAY & SCOOP BUCKET, CAUSING A BRUISE TO TOP OF LEFT ANKLE. HE WAS TREATED WITH A COLD PACK & DRIVEN HOME.
EE ELECTRICIAN TRAINEE, WAS HELPING DO MAINTAINENCE WORK ON CONTINOUS MINER. WHIEL PRYING WITH SLATE BAR TO REMOVE POT ON MINER, HE PULLED MUSCLE IN LOWER BACK. WENT TO HOSPITAL AND GIVEN MUSC LE RELAXER MEDICATION AND TOLD TO REST FOR A FEW DAYS.
EE WAS WALKING ALONG BELT, WAS GOING TO BELT SWITCH TO TURN BELT OFF - SLIPPED & FELL & FELL BACKWARDS ON RIGHT ANKLE. BROKE BONE IN ANKLE, WILL BE OFF 6 TO 8 WEEKS.
A ROOF FALL OCCURRED ON SAT 10-18-03 AT APPROX 2:30 AM. NO EQUIPMENT OR PERSONS INVOLVED, FALL WAS IN #45 ENTRY OUT BY SPAD 1754. AREA WAS DEVELOPED 01-99, FALL WAS 65' LONG BY 25; WIDE BY 6' THICK. AREA WAS CLEARED UP AND REBOLTED AND ADDITIONAL SUPPORTS INSTALLED. REPORTED TO STATE AND FEDERAL AGENCIES.

THIS AREA DEVELOPED IN JAN. 99 - APPROXIMATELY 1 1/2 MILES FROM ACTIVE SECTIONS. FALL AREA WILL NOT BE CLEANED UP. FALL 45' X 20' X 6'.
EE WAS LOADING RESIN ON TO THE ROOF BOLTING MACHINE WHEN A SMALL PIECE OF DRAWROCK FELL OUT & STUCK HIM ON THE LEFT HAND, MASHING IT AGAINST THEMACHINE. HE TOOK HIMSELF TO THE HOSPITAL-NOTHING BROKEN-GIVEN 1ST AID TREATMENT & WAS OFF WORK 1 SHIFT
EE WAS BOLTING ROOF IN #5 RT BT -HAD PULLED SOME DRAW ROCK DOWN & MASHED SUCTION HOSE ON BOLTER. WHILE REPAIRING HOSE, ANOTHER PIECE OF DRAW ROCK 16" X 8" X 1 1/2" THICK FELL & STRUCK HIM ON H EAD & NECK. HE WAS TAKEN TO HOSPITAL -CHECKED & RELEASED - NOTE - THIS DID NOT SEEM TO BE A SERIOUS ACCIDENT AT TIME OF OCCURRANCE.
EE WAS HELPING UNLOAD BELT STRUCTURE FOR A BELT MOVE - STRUCTURE GOT TANGLED UP & ROLLED BACK ON HIS LEFT INDEX FINGER CAUSING SPRAIN - WENT TO DOCTOR & MISSED 1 SHIFT DUE TO DOCTORS APPOINTME NT.
BOLTER OPERATOR WAS BOLTING ROOF IN #2 ENTRY ON 1ST RT SECTION. TOP WAS REAL HARD AND BIT TORE UP. HE WAS KNOCKING THE BIT OFF THE DRILL STEEL TO CHANGE IT WHEN HE STRUCK HIS LEFT INDEX FINGE R BETWEEN BOLTER AND DRILL STEEL BRUISING FINGER. HE WAS OFF 2 DAYS.
SHUTTLE CAR OPERATOR WAS CLEANING SCRUBBER FILTERS ON MINER. A PIECE OF DRAW ROCK WAS LAYING ON TOP OF MINER. HE ROSE AND STRUCK ROCK WITH HIS HAT, JAMMING NECK. WENT TO HOSPITAL. FOUND NOTHIN G BUT WAS TOLD TO TAKE A WEEK OFF FOR STIFFNESS.
FOREMAN WAS TAKING MINER CABLE DOWN FROM NYLON ROPE HANGER, WHEN HE DROPPED CABLE HOOK IN HANGER, SNAGGED HIS HAND AND CUT IT OPEN. DID NOT MISS ANY WORK, BUT HAD TO GET 8 SUTURES IN LEFT HAND .
