The fall has been timbered off and will be cleaned up. The fall effected the passage of men but not ventilation. The area was supported with 5' fully grouted roof bolts.

Employee was installing roof bolts and a piece of rock fell from the roof striking him on the left thigh/knee area. Bruised & sore.
Employee stepped on a water line and his foot buckled under him. Fractured two bones in his right ankle.
Employee was shoveling & cleaning along the beltline when he slipped and fell. Strained his lower back.
Employee was operating a shuttle car. He had a utility knife in his pocket. The knife caught on the panic bar and opened. Employee sustained a laceration to his upper left leg that required 2 stitches to close.
Employee was attempting to cross over the belt when he slipped and landed on his right shoulder. He went to doctor on 11-3-08 then worked his normal shift. Missed work on 11-4-08 due to injury, took a personal day on 11-5-08 for a court date. Returned to work on 11-6-08.

Victim was exiting the scoop and his foot slipped. He fell out of the scoop twisting his ankle. Placed on light duty.
EE worked overtime to assist with belt repairs. He had his gloves off while hooking up clamps with a wrench. The frayed end on a wire rope lacerated his right middle finger.
Victim was cutting curtain fly pads with a utility knife and it slipped. Laceration to palm of right hand. The wound was cleaned and sutured (6 stitches) at the emergency room.
Employee was operating a scoop and when he applied pressure to brake pedal he felt a pain in his left knee. Pulled ligament in left knee. Did not report the injury until 9-4-08.

Victim got a mantrip hung up and was attempting to get it loose by using a come-a-long hooked to a roof bolt. He slipped and fell onto the speed reducer in the mantrip. Has a lower back sprain.

This victim was liftng a final drive for a scoop into place by himself. This was too much weight for one person ? resulted in a hernia. Was placed on light duty until September 13, 2008, when surgery to repair hernia is scheduled. No work days missed at this time.
This victim struck the rib while rounding a curve and caused the rib to roll out into operator's deck. Alleges a separation in right shoulder. Went to one doctor and was told he could return to work, so he went to another that is keeping him off work.
The victim was walking along and stepped on a sled runner with one foot and a piece of coal with the other, the coal rolled and he fell forward. His foot was caught between sled and runner. Fractured both bones in lower left leg.
This electrician was replacing a guard on the miner after making repairs. While holding the shield in place and getting the bolts started a pain shot through his groin & testicles.
The electrician had locked out the breaker to the miner while performing tests. He gave the key to the operator and had him to reenergize the breaker. There was shielding wrapped around the cathead and this caused the catehead to be grounded. When the victim engaged the breaker it exploded. Burns to his hand and side of his face.
While installing roof bolts in #3 entry a rock fell from the top and bounced off the drillpot and struck him in the upper lip & nose area. He sustained a laceration to his upper lip area.
While pulling steel out of hole a rock fell and struck victims right hand. He sustained swelling & bruising.
Drill steel fell out of hole and struck victims left hand. He sustained bruising & swelling of his left hand.
A rock was against the side of the continuous miner. When the operator trammed the miner forward the rock rolled over onto his left foot and ankle. He sustained a fracture to his left foot
Victim was driving the mantrip. He hit a pot hole in the road and the wheel jerked around striking his hand. Two fingers were fractured. He removed the cast applied at the hospital and came back to work.
While pulling a water line the victim slipped and fell. He sustained a fractured fibula in his left leg.
Victim alledgedly was trying to pull a shield off a scoop when he strained his back. A co-worker says the victim told him he was going fishing with a buddy when he left the mine on the 7th. This report if being filed however the status of the ""injury"" is still in question as to whether it actually happened or not.
Victim was finishing bolting the #3 entry. He returned to the bolter and had his hand on top of the bolter when a piece of draw rock fell from between the plates and struck his right hand. He was transported to the hospital where surgery was performed on his hand. A pin was inserted, stitches to close and a soft cast was applied.

Became a lost time accident on 12/12/2006. Employee was moving a miner cable when he felt pain an hour later, worked until 12/01/2006 before seeking medical attention.
While the employee was hanging a miner cable, the hanger hook lacerated his nose.
EE was bolting #3 entry when he stepped on piece of rock on bottom & left knee popped - resulting in strain to knee - missed 2 shifts - returned to work on 6-10-06.
At 11:40 am a roof fall ocurred outby section approximately 1100' (outby section) in # 7 intake entry. Fall is 20' wide, 20' long and 8' thick. No one was in thia area. No injuries or equipment involved. Area will be cribbed off and left. Section will be moved to another area in this mine.
EE was loading shuttle car reached up to prevent coal from coming in deck & cut finger on a piece of metal on shuttle car. 4 stitches no lost time.
REACHING TO CUT METAL BAND ON KENNEDY STOPPING. WHILE DOING SO THERE WAS ONE ALREADY BROKE JABBING IN THE TOP OF HIS RT. HAND. REQUIRED STITCHES TO STOP BLEEDING. INJURY OCCURRED IN THE #4 ENTRY IN THE FACE.
EE is a Shuttle Car Operator was helping hang miner cable at poewer center, hurt back while lifting cable, went to hospital was treated and released for back strain missed one shift & returned to work on 3/17/06.
Employee was helping make a belt move, lifting belt with a bar & putting structure in belt when he felt pain in lower right side of back. Went to dr had strain. He came back to work on 2-20-06, worked 4 hours and left again - will be off a few days.
EE was mining in 4rt when a piece of draw rock fell out between bolts & struck him on left hand. Went to hospital and has small bone fracture on left hand. Rock was 3'x1'9"" x 2"" thick.
