Employee stated that he was stacking tires when as he was rolling a tire, it rolled on a bearing and began to fall. As he tried to catch it, grabbing it with his left arm, he felt something pop in his arm. Employee began missing work on 01/12/2011.

EE steam cleaned equip. until 3:30 pm fueled equip. and was returning to maint. area, EE stated he started down grade when truck jumped out of gear he applied brakes no brakes. The truck continued down the hill struck the berm and overturned coming to a stop on its top. EE was taken to hospital with injuries to right arm and several bruises. Released 06-21-10
Employee alleges that he sustained multiple physical injuries from minor motor vehicl accident. Employee was driving truck and had left the truck dump station. He was traveling approximately 5-mph going down hill and 'rear-ended' another truck that had stopped. SUBMISSION BE FILED UNDER PROTEST
Employee was driving off-road coal truck and had stopped; before proceeding after stop, the truck was'rear-ended' by an approaching truck with minimal damage to both trucks. Both trucks had just dumped in loading bin before incident occurred.

Employee stated he was cleaning windows on truck - stepped on lower step mounted to the fuel tank causing it to break. He alleges his right leg hit the ground while his left foot remained on step.

EE was performing maintenance on a hyd shovel. Wind was gusting at times. Around 3:00 a piece of grit was felt in the eye. It was washed out with water and felt OK. That night the grit was felt again. All attempts to remove didn't work. Went to hospital and had it taken out. Eye protection being worn.

EE was handling a clevis for 2" steel cable when the clevis dropped catching the tip of his ring finger on the right hand between the clevis and a rock resulting in a fracture to the tip of the finger.

The bulldozer slid out of the road. Employee put blade down to stop. The bulldozer rolled 3 times down hill. He fractured neck in two places and had 9 staples in his head.
He says he was walking to truck and slipped and fell. We had a witness that says he saw him walk to truck and get in and didn't fall. WE ARE FILING UNDER PROTEST

He said he was checking his tires at the end of his shift to make sure they was not flat. He did this by kicking them He was kicking with right foot when his left foot slipped. He tried to prevent fall by bringing his right foot down.
EE stated that he raised the hood to check the fluids, when hsi shoulder popped, resulting in a "fractured" collar bone. This hood weighed approx 25's maximum.

EE was using a come-a-long to secure a steel frame to the forks of a forklift to move to another location. While pulling on the come-a-long, his feet were muddy and he slipped. While catching himself,to keep from falling in the mud, he grabbed the steel beam and felt like he may have sprained his right arm.
Employee was installing input shaft in rotary head, using hammer to install bearing on pinion shaft. A piece of metal chipped from bearing and went into the left eye. He was taken by ambulance to medical center, then to hospital for surgery. He has been instructed to remain out of work for a least 14 days to recover, unsure of prognosis from that point.

While exiting truck, he slipped on middle step. He stated that he grabbed handrail, swung around and hit his left shoulder on bed of truck, then fell to the ground.

As the employee went to the fill to dump Cat 785 rock truck company number unit #334, the truck went through the berm and over the fill. The employee received a laceration to back of the head, which required 9 staples in addition has contusions to waist and left leg. Medical reportable only - no lost work days.

Employee stated that he was picking up a crib block when he hurt his back. Stated that it was an old injury from a non-occupational motor vehicle accident.
EE was exiting 'cut A' coal pit. Because other adjacent vehicles/equipment, he drove on left side of roadway which is inclined; vehicle turned over onto its drivers side.
Employee stated that he was driving up hill from 'Y' cut when he missed a gear. The truck rolled back against the berm and turned over onto it's driver side.
EE stated that he was backing truck onto the stockpile ramp to dump coal, when the hoist nest broke, causing the bed to shift, laying truck partically on its side.
Employee stated that he was traveling in a curve when the power steering hose blew. This apparently caused the steering wheel to center & jerk his right arm.

Doing service work on Dozer. Oil filter on extremely tight. Was using 1/2" ratchet. Would not budge. Hit ratchet with 4 lb. hammer. On the 5th swing, he missed the handle and hit his thumb.

Injured was replacing a leaf spring on a school bus. While positioning the spring on the front axle, the spring slipped off the axle and caught his finger betwen the spring and a crib block lacerating the little finger which required stitches and medical treatment.

While opening the door on a rail car, used slate bar to open door. Stuck slate bar in door release to turn open and release, skipped a tooth causing employee to slip down to knees and bar hitting track rail with right thumb between rail and bar mashing right thumb.
