EE was working at the Goals mine location cutting brush when a stick struck EE in the eye. EE claims EE was wearing a hard hat with face shield. EE's supervisor transported EE to Beckley General hospital in company vehicle. Beckley General transported EE to Ruby Memorial hospital via ambulance.

He felt a pain in his back and at his beltline and left buttock. **The employee has a history of back problems & medical documentation indicates Degenerative Disc Disease (Degree 07)**

Employee was changing a suspension spring on a vibrator. While prying it away from the chute with a slate bar, the bar slipped allowing employee to fall backwards onto another vibrator. This caused an injury to the employee's knee.

EE went up a 12 foot fiberglass ladder to perform work on ceiling area. After assesing how to tie off EE started back down the ladder, he lost his balance and fell about 8 feet landing on his back. He was transported to the hospital via ambulance where it was found that he had two cracked vertebrae and was suffering from a concusion.

INJURED HAD CLIMBED UPON THE BACK OF A MECHANIC TRUCK TO CONNECT A AIR HOSE TO A GREASE GUN. HE STOOPED DOWN AND PICKED UP THE HOSE FROM THE BED FLOOR. AFTER HE RAISED UP TO MAKE THE CONNECTIO N, HE FELT A BURNING SENSATION IN THE BACK OF HIS LOWER LEFT LEG.

HE WAS HELPING TO PUT AN IMPELLER BACK ON A SLURRY PUMP. WHILE REACHING DOWN TOWARD THE LOWER SHEAVE AND PULLING THE BELTS UPWARD TO ROTATE THE SHAFT TO SPIN ON THE IMPELLER, HIS LEFT FINGER W AS CAUGHT IN BETWEEN THE BELTS AND THE MOTOR SHEAVE.

WHEN AN ATTEMPT WAS MADE TO RETRACK H, THE CHAIN CLIMBED UPON THE SPROCKET, SHEARING OFF A SMALL PIECE OF METAL WHICH STRUCK EE ON BOTH ARMS. HE REQUIRED 3 STITCHES.

EE WAS HELPING CARRY 3/8" BY 4' BY 6' PIECE OF METAL UNDER THE SIB TO MAKE REPAIRS TO A FEEDER. HE FELT A STRAIN TO HIS BACK WHEN HE LIFTED UPWARD ON A METAL. HE WORKED AS SCHEDULED UNTIL 6-10-02 WHEN HIS DOCTOR ADVISED HIM TO REMAIN AWAY FROM WORK AND TAKE THERAPY 3X A WEEK.

EE WAS HELPING REMOVE A 25 HP 284T, 150 LB MOTOR FROM THE REFUSE SCREEN. HE WAS STANDING NEAR THE BASE OF THE SCREEN TO REPOSITION THE BOTTOM AS OTHER USED COME-ALONGS TO LIFT IT UP AND AWAY. AS THE MOTOR MOVED THE HOOK PULLED THROUGH THE EYE BOLT IN THE MOTOR, ALLOWING IT TO DROP TO THE SCREEN BASE. EE'S HAND WAS CAUGHT BETWEEN THE MOTOR AND THE FRAME.

EE HAD DUMPED A LOAD OF ROCK ONTO THE #2 REFUSE BELT.HE HAD LET THE BED DOWN WHEN HE NOTICED THAT A ROCK HAD FOULED THE TAILGATE AND WOULDN'T LET IT CLOSE PROPERLY. HE TRIED TO REMOVE THE ROCK WITH HIS LEFT HAND WHEN THE TAILGATE CAME DOWN QUICKLY AND CAUGHT HIS LEFT MIDDLE AND RING FINGERS.

ER DR FOUND NOTHING APPARENTLY WRONG, (NO INJUR Y OCCURRED) GAVE HIM A "DR SLIP" FOR 1 WEEK & SET UP AN APPOINTMENT WITH AN ORTHOPEDIC DR. EE WOULD NOT TELL US WHICH SPECIALIST HE WAS SEEING & WOULD NOT SIGN RELEASE FOR CO TO GET MED RECOR
