EE was working at the Goals mine location cutting brush when a stick struck EE in the eye. EE claims EE was wearing a hard hat with face shield. EE's supervisor transported EE to Beckley General hospital in company vehicle. Beckley General transported EE to Ruby Memorial hospital via ambulance.
Employee has applied for Occupational Pneumoconiosis.

Employee was diagnosed occupational pneumoconiosis without pulmonary function impairment.
Employee had just repositioned a space heater in the bucket of an IT loader. When he turned to walk away, he slipped on coal covered ice. He fell on his right side over extending his shoulder.
employee was in front of a 800 amp breaker box when the box exploded. Employee was moving a mat and had his hand on the box, when the box exploded
Employee was cleaning some spillage on the second floor of the plant with a shovel. He kicked the back of the shovel to get it through the material. He felt a pain in his back and at his beltline and left buttock. **The employee has a history of back problems & medical documentation indicates Degenerative Disc Disease (Degree 07)**
Employee was walking in the maintenance shop and while reaching for a box of rags the employee tripped over a wooden pallet causing the employee to fall to the ground. Employee fell on wrist causing an injury.
Employee was using an electric drill and the handle slipped causing the employee to cut his hand on the drill bit. Causing the employee to receive 5 sutures in his hand.
Employee was getting on the plant elevator when his right foot slipped, causing him to lose his balance. He twisted his left knee which caused him to buckle and fall to the ground.
Employee was changing a suspension spring on a vibrator. While prying it away from the chute with a slate bar, the bar slipped allowing employee to fall backwards onto another vibrator. This caused an injury to the employee's knee.
Member cut his finger on a screen, causing him to receive stitches to his finger.
Member was hammering on the clean coal silo load-out chute trying to unplug the chute. He felt a pain in his right arm above his elbow. Member did not miss any days of work until surgery was required on 10/8/2010.
Member smashed his finger with a hammer causing him to receive stitches to his left ring finger.
Member was at the raw coal silo helping get feeders running. Water had been utilized to wash raw coal. When water and mud broke loose and knocked membert to floor causing a laceration to his left upper eye lid which required 12 stitches,
Injured employee was moving screen deck across floor, strained lower back while stabilizing.
He was cutting weeds on the side of a hill and his right foot slipped on the wet mud and he twisted the lower part of his back causing injury to his lower back. He has had a history of back problems due to a past accident.
EE was helping to lift the hopper out of the CMI and it slipped, cutting EE's finger. EE received 6 stitches.
Removing radiator guard from D8L dozer. Removing pins drove pin out guard moved crushed finger between guard & frame, hammer & punch.
EE WAS ON ELEVATOR. WHEN CLOSING THE DOOR HIS RIGHT HAND BECAME CAUGHT BETWEEN THE DOORS SMASHING HIS MIDDLE AND PINKY FINGERS.
EE WAS MOUNTING THE D-10 DOZER (MAINTAINED 3 POINT CONTACT)WHEN HIS LEFT FOOT SLIPPED ON THE MUDDY TRACK & WENT TOWARD THE DOZER BODY BETWEEN TRACK & B0DY. HE TRIED TO CATCH HIMSELF WITH HIS LEFT HAND & BENT BACK HIS MIDDLE FINGER. THE SAFETY DEPT WAS NOT MADE AWARE OF THIS ACCIDENT UNTIL 8/19/05.
EE went up a 12 foot fiberglass ladder to perform work on ceiling area. After assesing how to tie off EE started back down the ladder, he lost his balance and fell about 8 feet landing on his back. He was transported to the hospital via ambulance where it was found that he had two cracked vertebrae and was suffering from a concusion.
INJURED HAD CLIMBED UPON THE BACK OF A MECHANIC TRUCK TO CONNECT A AIR HOSE TO A GREASE GUN. HE STOOPED DOWN AND PICKED UP THE HOSE FROM THE BED FLOOR. AFTER HE RAISED UP TO MAKE THE CONNECTIO N, HE FELT A BURNING SENSATION IN THE BACK OF HIS LOWER LEFT LEG.
HE WAS HELPING TO PUT AN IMPELLER BACK ON A SLURRY PUMP. WHILE REACHING DOWN TOWARD THE LOWER SHEAVE AND PULLING THE BELTS UPWARD TO ROTATE THE SHAFT TO SPIN ON THE IMPELLER, HIS LEFT FINGER W AS CAUGHT IN BETWEEN THE BELTS AND THE MOTOR SHEAVE.
EE WAS SUPERVISING THE CONTRUCTION CRESO AS THEY ATTEMPTED NEED OVER OF THE D10 DOZER FROM THE EMBANKMENT THE DOZER CAME OFF IT STRACKSON THE RIGHT SIDE. WHEN AN ATTEMPT WAS MADE TO RETRACK H, THE CHAIN CLIMBED UPON THE SPROCKET, SHEARING OFF A SMALL PIECE OF METAL WHICH STRUCK EE ON BOTH ARMS. HE REQUIRED 3 STITCHES.
EE WAS HELPING CARRY 3/8" BY 4' BY 6' PIECE OF METAL UNDER THE SIB TO MAKE REPAIRS TO A FEEDER. HE FELT A STRAIN TO HIS BACK WHEN HE LIFTED UPWARD ON A METAL. HE WORKED AS SCHEDULED UNTIL 6-10-02 WHEN HIS DOCTOR ADVISED HIM TO REMAIN AWAY FROM WORK AND TAKE THERAPY 3X A WEEK.
EE WAS HELPING REMOVE A 25 HP 284T, 150 LB MOTOR FROM THE REFUSE SCREEN. HE WAS STANDING NEAR THE BASE OF THE SCREEN TO REPOSITION THE BOTTOM AS OTHER USED COME-ALONGS TO LIFT IT UP AND AWAY. AS THE MOTOR MOVED THE HOOK PULLED THROUGH THE EYE BOLT IN THE MOTOR, ALLOWING IT TO DROP TO THE SCREEN BASE. EE'S HAND WAS CAUGHT BETWEEN THE MOTOR AND THE FRAME.
PUTTING IN STAINLESS STEEL PAN. HE HAD HOLD OF IT WITH HIS HAND BETWEEN PAN AND CHUTE. THE PAN SLIPPED AND MASHED HIS FINGER.
EE WAS WALKING UP THE STAIRS TO THE ROTARY BREAKER. HE FINISHED THE SHIFT AND CONTINUED TO WORK AS SCHEDULED. ON MARCH 6, 2002, THE SAFETY DEPARTMENT WAS NOTIFIED THAT EE HAD SEEN A NEUROSURGE ON IN BECKLEY. HE WAS ADVISED TO REMAIN AWAY FROM WORK UNTIL HIS CONDITION IMPROVED.
EE HAD DUMPED A LOAD OF ROCK ONTO THE #2 REFUSE BELT.HE HAD LET THE BED DOWN WHEN HE NOTICED THAT A ROCK HAD FOULED THE TAILGATE AND WOULDN'T LET IT CLOSE PROPERLY. HE TRIED TO REMOVE THE ROCK WITH HIS LEFT HAND WHEN THE TAILGATE CAME DOWN QUICKLY AND CAUGHT HIS LEFT MIDDLE AND RING FINGERS.
EMPLOYEE WAS GOING OVER A BANK TO THE FOREIGN COAL BELT TUNNEL. HE LOST HIS FOOTING AND FELL LANDING ON HIS WRIST.
EE STATED THAT VALVE ON CYCOLNE WAS STUCK. HE REACHED OVER HANDRAIL & ATTEMPTED TO LIFT HANDLE ON THE VALVE. HE SAID HIS RT ELBOW BEGAN TO HURT. ER DR FOUND NOTHING APPARENTLY WRONG, (NO INJUR Y OCCURRED) GAVE HIM A "DR SLIP" FOR 1 WEEK & SET UP AN APPOINTMENT WITH AN ORTHOPEDIC DR. EE WOULD NOT TELL US WHICH SPECIALIST HE WAS SEEING & WOULD NOT SIGN RELEASE FOR CO TO GET MED RECOR
