A roof fall occured at spad 0+00 inby to 30' outby spad 1+40. The fall is located on #1 unit-1F panel in the #6 entry (belt entry). The fall is 110' long X 20' wide X 40"+ high. This was in the 2nd room set-up. Unit moved to the 3rd room set-up.
A roof fall occurred in #5 room, in 1-F panel; of #1 unit at spad #3+50 to 4+20. Approximately 80' long x 20' wide x 4' high.
Employee was in the process of helping another ram car out of underpass. The underpass was very steep. The batteries on the ram cars became interlocked. The batteries came apart dropping suddenly approx. 12" to 18" causing the ram car that EE was operating to jar him injuring his upper back. This area is not normally traveled by ram cars. The regular travelway was blocked.
A rock fall occurred on #2 unit at 5+10 in the 1st south panel. The fall was approximately 80 x 19 x 6. Unit was moved to next set of intake rooms.
EE was in the process of working on batteries for a battery hauler. He was stepping from the ground onto the batteries when he twisted his right knee. He didn't start missing work until March 16, 2007 to have surgery on his knee.
A rock fall occurred at 12+60 in the 3rd east submain in entries 5 & 6. The fall was 90 x 19 x 6 and ran north & south. The fall will be cleaned up.
Working on trying to keep water from going into the mine due to a storm. Helping pump water in the pit area when called to go to #1 unit due to a continuous miner being down, took off his extremely wet gloves while traveling into the mine. He hit the antenna switch for the air lock doors that had the end of the wire hooked up ward causing it to cut his middle left finger.
A rockfall occurred @ 1pm on the #2 Unit in the 3rd West Panel. The fall blocked our secondary escapeway. The fall was between #3 & #4 entries, extending to #5 entry. The fall was 10x19x9. Msha was notified within 15 minutes.
A rockfall occurred on the 2C belt. The fall was 40 x 19 x 5. Five foot glue bolts were used in this area. The area was advanced on 70 x 50s. There was a hidden slip - no water in area.
Employee stated pinner steels were stuck and he was trying to pull apart. When they came apart struck him in lower lip.
Employee was standing & starting complaining of knee pain. He's a brittle diabetic with many aches & pains in his joints. After medical exam it was determined he was suffering from diabetic neuropathy.
Employee was getting ready to move miner when a piece of rock fell striking left shoulder and arm. He was on #2 unit in #1 entry cutting 1 left.
#2 Unit had a rockfall inby the dumping point in the #5 room at 2+85 to 3+20. The face was 120 feet inby the crosscut. There was a slip. The fall was in the pillar line.
#2 unit had a rockfall inby the dumping point in the intersection of #8 room at 2+75. The face was 15 feet inby the crosscut. There was a slip.
Employee was pulling into the battery shack on his golf cart when he came into contact with the two wheel utility trailer parked on the back side of the curtain. The curtain was triple layered. He hit the right rear end of the trailer and it rolled upon his golf cart and on top of his left foot.
Employee was riding in Alpha 14 man diesel mantrip facing to the side (perpendicular) to the direction of travel. He had his feet propped upon the brace. When the mantrip made a 90 deg turn to the rt, the trip was hitting the coal rib. His lt foot hit the coal rib first, he was trying to move his lt foot when toe of his rt foot caught between frame of mantrip and coal rib.
He had bone spurs upon exam.
A roof fall occurred at 15+60 in the 4th south panel. The fall was 80 x 19 x 10. The area had 5' glue bolts & 12' cable bolts in it. The belt only had 5' glue bolts. The fall was 3800' outby the working faces. Fall will be cleaned up.
A rock fall occurred ap[prox. at 5:00 am on #1 unit in the intake from #8 entry to #6 entry blocking the intake. Some intake was open in #8 entry from rib approx 5 feet. Rockfall was 180 X 6 X 19 feet, 40 feet outby working faces.
EE was bolting in 2 right on #1 unit. He had drilled his starter hole, He had put his top section on the finish steel into the hole in the roof. He then put the bottom section into the chuck. The steel was not seated properly and when applied upward pressure on the boom, the steel dislodged and struck him in the arm.54
EE WAS GOING TO PUT SOME WIRE FOR THE POWER MOVE. THE EE STATED HE WALKED THROUGH THE AIR LOCK CURTAIN ON THE SUPPLY ROAD. WHEN HE STEPPED INTO A SMALL HOLE ON THE OTHER SIDE OF THE CURTAIN CAUSING HIM TO TRIP/STUMBLE. HE WAS TRYING TO CATCH HIMSELF & FELL TO THE MINE FLOOR.
A roof fall occured in the 1st north main in the #4 entry (supply road) at spad # 65+10 inby to spad #66+50. The fall was 120'long X 18 1/2' wide X 6'high.
EE HAD MOVED MINER CABLE OVER IN #4 RIGHT AT THE START OF THE SHIFT WHEN HE EXPERIENCED A SMALL PAIN IN HIS LOWER BACK. HE THOUGHT NOTHING OF IT AT THAT TIME. HE CON'T TO CUT COAL, MOVED THE MINER TO Z RIGHT, BENT OVER TO THROW ONE END OF LOOP OVER WHEN HE FELT ANOTHER PAIN IN LOWER BACK.
Employee was riding in scoop bucket in the direction of the battery end on #1 unit's supply. While the employee was watching the miner cable, the scoop operator raised the bucket because the tire went down into a hole. Scoop came out of hole; the bucket went up against the roof pinning the employee between the floor of bucket and mine roof.
A rock fall occurred on #1 unti -#8 entry in the intake. Fall was from 17+55-18+65 a total of 110 ft. fall started outby feeder and extended 55 ft inby-110 ft from face.
EE was pinning in #3 right when a piece of very thin rock and coal spun down steel hitting him on the lower part of his lip.
Employee stated he was coming down the steps from the bath house (it was drizzling rain, steps were wet, handrail wet also) when he slipped on the third step from the bottom causing him to fall to the second step from the bottom.
Employee was working on a ram car, he turned and was walking to get wrench when he stepped on a piece of rock 12" x 12" x 4". This caused him to fall to the mine floor, he experienced immediate pain and swelling in his right ankle.
Employee stated he was pulling the miner cable back with one hand. He stated, he fell down while holding onto the cable. He stated, he felt a pain in his left shoulder.
EMPLOYEE WAS WALKING FROM THE UNIT MAN TRIP. WHEN HE ENTERED THE BELT ENTRY HE STEPPED IN A HOLE CAUSING HIM TO SPRAIN HIS ANKLE.
Employee stated he was beside pinner in the #6 entry on #2 unit when a piece of rock fell striking rt leg between knee & ankle. The rock caused 2 fractures (1 6 inches above ankle on right side of leg & 1 3 inches above ankle on left side of leg.) The rock was 2.9 feet wide x 4.8 feet long x 6 inches to 1 inch thick.
EE WAS TRAVELING IN AT START OF HIS SHIFT, OPERATING A GOLF CART ON THE #5 SUPPLY ROAD. WHEN HE TURNED THE CORNER AT THE #5 HEADER, AT CROSSCUT #2, A PIECE OF ROCK FELL OUT, HITTING HIM ON THE NOSE. CAUSED A LACERATION ON THE LEFT SIDE OF HIS NOSE REQUIRING 8 - 10 STITCHES.
STARTER STEEL WAS NOT ALL THE WAY IN THE CHUCK OF THE PINNER POT. EE HAD ALREADY SWUNG HIS BOOM OUT AND WAS REACHING FOR HIS FOOT JACK LEVER WHEN HE ACCIDENTLY HIT ROTATION LEVER CAUSING THE STEEL TO FLY OUT STRIKING HIM ON HIS SAFETY GLASSES, WHICH IN TURN CUT HIS NOSE AND CHEEK.
A rock fall occurred at #6 entry #1 unit fro 8+50 to 10+00. The unit has moved out of the panel. MSHA district office was notified.
A ROOF FALL WAS OBSERVED IN THE MAIN NORTH ENTRY (#9) STARTING AT SPAD #51+80 INBY TO SPAD #56+30. APPROX 45O LONG. FALL IS INBY 4E PANEL SEALS.
An unintentional rock fall occurred on #1 unit #3 entry inby 3+50 to inby 4+50. This is a neutral entry - the face is at 11+00. The fall will not be cleaned up at this time.
EE STATED HE WAS WORKING ON 1C PONY HEADER IN THE PROCESS OF TIGHTENING THE JACK PIPE. A PIECE OF ROCK FELL OUT OF THE TOP HITTING HIM ON THE BACK. HE SHOOK THE ROCK OFF HIS BACK & THAT CAUSED THE ROCK TO FALL HITTING HIS RIGHT FOOT AGAINST THE WATER LINE PIPE. IT WAS A ZINC WATER LINE. ROCK WAS 3 FT X 3 FT X 3 1/2 INCHES.
A roof fall was observed at spad #15+40 along the supply road, belt and secondary road of #1 unit in panel Z-E. 170' x 18.5' x 5'
A rock fall occurred at 13+50 on the #2 unit. This was 1800' outby working section. Fall was from edge #5 entry across panel into old rooms - Return side rooms - the unit was idled until fall was cleaned up in supply road and return.
EE STATED HE WAS KNEELING IN CROSS-CUT WHILE SCOOP WAS PASSNG BY WHEN ROCKFELL OUT BETWEEN PINS ON 2-B SUPPLY ROAD CROSS-CUT 48+30 STRIKING HIM ON THE UPPED BACK, NECK AND LEG. HE MISSED STARTING TUESDAY'S SHIFT (OCT. 18, 2005)
Examiner found rock fall in return main south-# entry @ 66+40. Must have benn hidden slip-no water. Fall was 6350 ft outby dumping pt. on unit #1.
Ee was changing bits in the miner head. He had the bit wrench in his left hand and small sledge hammer in his right hand. He put the bit wrench on the bit close to the bottom of the head. While attempting to strike the wrench, he struck his right index finger against the bit wrench or bit block.
EE and other were trying to tighten a nut on the stacker belt take-up. They had a pipe wrench on the nut a cheater bar on the pipe wrench and both ot them were pushing on the cheater bar. The wrench slipped ee mashed his finger between the cheater bar and frame of stacker belt.
A roof fall occurred on #1 unit-#8 entry in the intake at plus 32 & 20. The fall was in an section and extended both direction 15 ft. the fall was outby the working section 850 ft. the fall was 18x 51/2 x 50.
A ROOF FALL WAS OBSERVED BY MINE EXAMINER DURING HIS PRE-SHIFT OF THE MINE ON SUNDAY AFTERNOON, FOR MEN TO ENTER THE MINE. THE FALL IS LOCATED BETWEEN INTERSECTION OF SPAD #25 + 90 AND EXTENDS INBY TO THE INTERSECTION OF 26+60 ON #1 UNIT SUPPLY ROAD IN Z-E PANEL.
EE was splicing a blown waterhose when the band broke off in the banding machine. He got another bander and finished fixing the waterhose. He took out his channelocks and tried to pull the broken piece of band out of the machine. When the piece came out it struck his right thumb causing a laceration that needed 8 sutures. He had removed his gloves because they were wet & slippery.
EE WAS IN THE PROCESS OF MOVING MINER CABLE OUT OF THE HAULAGE WAY WHEN HE EXPERIENCED BACK PAIN.

A ROCKFALL OCCURRED SOMETIME OVER THE WEEKEND ON #1 UNIT. THE FALL WAS IN #3 ENTRY-NEUTRAL BETWEEN 28+30=29+10. 80X18X51/2
Examiner found rockfall in #8 entry - #1 unit in the intake - The fall was 140 x 18 x 5 1/2 - outby the face about 1300 ft.
