A roof fall occured at spad 0+00 inby to 30' outby spad 1+40. The fall is located on #1 unit-1F panel in the #6 entry (belt entry). The fall is 110' long X 20' wide X 40"+ high. This was in the 2nd room set-up. Unit moved to the 3rd room set-up.

Employee was in the process of helping another ram car out of underpass. The underpass was very steep. The batteries on the ram cars became interlocked. The batteries came apart dropping suddenly approx. 12" to 18" causing the ram car that EE was operating to jar him injuring his upper back. This area is not normally traveled by ram cars. The regular travelway was blocked.

EE was in the process of working on batteries for a battery hauler. He was stepping from the ground onto the batteries when he twisted his right knee. He didn't start missing work until March 16, 2007 to have surgery on his knee.

Working on trying to keep water from going into the mine due to a storm. Helping pump water in the pit area when called to go to #1 unit due to a continuous miner being down, took off his extremely wet gloves while traveling into the mine. He hit the antenna switch for the air lock doors that had the end of the wire hooked up ward causing it to cut his middle left finger.

Employee was pulling into the battery shack on his golf cart when he came into contact with the two wheel utility trailer parked on the back side of the curtain. The curtain was triple layered. He hit the right rear end of the trailer and it rolled upon his golf cart and on top of his left foot.

Employee was riding in Alpha 14 man diesel mantrip facing to the side (perpendicular) to the direction of travel. He had his feet propped upon the brace. When the mantrip made a 90 deg turn to the rt, the trip was hitting the coal rib. His lt foot hit the coal rib first, he was trying to move his lt foot when toe of his rt foot caught between frame of mantrip and coal rib.

EE was bolting in 2 right on #1 unit. He had drilled his starter hole, He had put his top section on the finish steel into the hole in the roof. He then put the bottom section into the chuck. The steel was not seated properly and when applied upward pressure on the boom, the steel dislodged and struck him in the arm.54

Employee was riding in scoop bucket in the direction of the battery end on #1 unit's supply. While the employee was watching the miner cable, the scoop operator raised the bucket because the tire went down into a hole. Scoop came out of hole; the bucket went up against the roof pinning the employee between the floor of bucket and mine roof.

The rock caused 2 fractures (1 6 inches above ankle on right side of leg & 1 3 inches above ankle on left side of leg.) The rock was 2.9 feet wide x 4.8 feet long x 6 inches to 1 inch thick.

STARTER STEEL WAS NOT ALL THE WAY IN THE CHUCK OF THE PINNER POT. EE HAD ALREADY SWUNG HIS BOOM OUT AND WAS REACHING FOR HIS FOOT JACK LEVER WHEN HE ACCIDENTLY HIT ROTATION LEVER CAUSING THE STEEL TO FLY OUT STRIKING HIM ON HIS SAFETY GLASSES, WHICH IN TURN CUT HIS NOSE AND CHEEK.

EE STATED HE WAS WORKING ON 1C PONY HEADER IN THE PROCESS OF TIGHTENING THE JACK PIPE. A PIECE OF ROCK FELL OUT OF THE TOP HITTING HIM ON THE BACK. HE SHOOK THE ROCK OFF HIS BACK & THAT CAUSED THE ROCK TO FALL HITTING HIS RIGHT FOOT AGAINST THE WATER LINE PIPE. IT WAS A ZINC WATER LINE. ROCK WAS 3 FT X 3 FT X 3 1/2 INCHES.

Ee was changing bits in the miner head. He had the bit wrench in his left hand and small sledge hammer in his right hand. He put the bit wrench on the bit close to the bottom of the head. While attempting to strike the wrench, he struck his right index finger against the bit wrench or bit block.

EE and other were trying to tighten a nut on the stacker belt take-up. They had a pipe wrench on the nut a cheater bar on the pipe wrench and both ot them were pushing on the cheater bar. The wrench slipped ee mashed his finger between the cheater bar and frame of stacker belt.

EE was splicing a blown waterhose when the band broke off in the banding machine. He got another bander and finished fixing the waterhose. He took out his channelocks and tried to pull the broken piece of band out of the machine. When the piece came out it struck his right thumb causing a laceration that needed 8 sutures. He had removed his gloves because they were wet & slippery.
