Fall from machine

Employee was found outside of the mine laying on the backside of the Slope Belt. His last known work location was around the Main South Belt Tailpiece area. His only memory of the accident is being on top of the Main South Belt and striking the top of his head on a roof bolt. The cause of the accident is unknown.

Caught in, under or between a moving and a stationary object

Injured employee was standing in between two (2) mobile rides, when one (1) of the rides was struck by a scoop, knocking the rides into each other, pinning the employee causing a fracture of the right lower leg.

Caught in, under or between running or meshing objects

Employee attempted to catch pin steels as they fell from the drilled hole. In an attempt to regain his balance, his right hand landed on top of the roof bolter pod and his left hand accidentally grabbed the raise lever causing the boom to lift, pinching his little finger between the pod and extension arm.

Injured employee (rb operator) was inserting his extention steel, a piece of draw slate (10"" L X 8"" W X 2"" T) fell striking the employee's Right Hand, (little finger) causing a laceration, requiring stitches.

A rock measuring 45"" W x48"" L x2"" Thick fell (approximately 46"") from between roof bolts. Upon impacting the mine floor, the rock rolled inward with the outer edge striking the employee's right ankle.

Employee was in the process of installing a roof bolt and was using the boom of the machine to push the pin into the hole when he got his left hand caught between the boom and the pin causing a laceration to his little finger requiring six (6) stitches.

A rock (4' x 3'x 3"" thick) slid from atop the loading section of a ram car landing on top of the injured employee's left foot.

Employee was in the process of splicing a cable when the twisted cable slipped in his hand as he was splitting the jacket of the cable. The knife blade struck the palm of his hand cutting it open in a slicing motion.

A roof fall occurred in the 1st SW Submain in the #8 entry intake escapeway at XC 9+30. This was in a slip area. The fall measured 30' X 30' X 8'. Some 9' cable bolts were in the intersection, but were pulled out or came out with the rock. Visible slips and wet soft bottoms that is present will require more support, such as cribs, timbers and prop-setters.

At approx. 3:40 am, a roof fall occurred in an intersection on No. 2B supply road in the No. 6 entry. The fall was approx 5,800 feet outby the working face. In addition, the fall was approx 30 feet long, 15 feet wide & 9 feet thick. It did not affect ventilation or passage of men. It did not fall above the anchorage of the bolt being used. Fall to the walkway or working surface

The EE was assigned to service the mantrip located outside the mine shop.As he was walking out of the shop, he tripped over the corner of a wooden pallet that was laying on the ground next to the shop. While he was walking out of the shop, he was carrying a quart of oil that he was going to put in the mantrip. He fell forward & landed on his right knee & right shoulder.

EE was in the process of changing a flat tire on a 3-wheeler transport ride underground on #2 unit. He had hooked a chain to the front of the 3-wheeler and was in the process of raising the ride about a foot and a half, when the chain came off, causing the ride to hit him on the left knee. Because of low coal, he was on both knees. He was taken to a local hospital for cut to knee.

Employee was in the process of reclaiming belt structure and was loading the structure in a scoop bucket. While doing so, he accidentally placed his left foot underneath the bucket as it was being lowered to the floor. As the bucket came down, it pressed down on top of the foot and fractured the tarsal bone. NOTE: He was wearing metatarsal boots.

Starting in intersection in last open xc outby 70' to 90', an unplanned roof fall occurred in #1 entry on #1 unit starting at 23+80 to 24+50. There is a slip running through this area. The area has been timbered out and cribbed off and flagged off to keep anyone from entering. Fall was 14' to 18' thick and 8' high.

EE stated that he was in the process of roof bolting the face of the #2 entry on the #1 unit. While pulling the drill steels back out of the roof, one of the drill steel sections stayed in the mine roof. While the ee was placing drill steel sections in the roof bolter's tray, the steel that had stayed in the roof fell out striking the ee's finger on his left hand.

Employee was in the process of reclaiming a water line at the old #2 beltline. As he was lifting the water line up to a trailer, his left ear scrapped against a belt chain that was hanging down from the mine roof causing a small cut to the inner portion of the ear. Later that evening, he went to the medical clinic and received 5 stitches.

The employee stated that while he was roof bolting, a piece of roof rock fell out striking the head of the roof bolting machine and bounced off hitting him in the mouth cutting his lower lip. First aid was administered and he completed his shift. Later on, he decided to go to the emergency room. While at the ER, he received 5 stitches to the lower lip.

Employee had just completed greasing fittings on the back of the roof bolting machine. When he got up and turned around to walk away from the bolter, a piece of loose rock fell striking him on the left lower arm and hand. The ee was using a walking stick and rock hit his hand driving in down on the walking stick. He has a small fracture behind the 2nd knuckle on his left hand.

EE stated that while operating a ramcar on the section, he dipped down in a hole & then bounced up throwing him against the roof of the canopy. The impact caused him to hit his head against the canopy. He was semi-unconscious immediately afterwards & then responsive. Taken to the hospital, treated & released. On 7/28/08 he went to a chiropractor, & now has begun missing work.

The EE was sitting down at the NO.1 unit battery shack, preparing to change a 4-man ride tire. While doing so, he started having shortness of breath and then said that he was experienching chest pains. The pain was in the center of his chest, with tingling in his left hand. He told 2 co-workers he wanted to sit down. A few minutes later he said he was having a heart attack.

Contact with electrical current

The injured employee was in the process of hanging the left side roof bolter cable that was lying on the mine floor to make sure that no one would run over it. As he picked it up, he received an electrical shock to the right hand. He was examined and placed under observation at the hospital. He returned to work the following Monday.

Struck by falling object

While moving across the unit, a piece of rock (3'x1'x1""thick) fell out between pins and struck the EE on the back and left hand. He returned to work the next 2 days and then began missing work.

A roof fall occurred in the morning of 5/6/08 on #1 unit in the #4 entry approx 70' long, 18' wide and 5' high. During investigation it was found that a kidney bed had been encountered in the one entry only.

While moving belt structure underground, the employee stated that he twisted his left knee. He has not missed any work up to 5/28/2008. According to a doctor who recently examined him, the employee has a torn meniscus in his knee. He is scheduled to have outpatient surgery on 5/29/2008.

A roof fall occurred between the afternoon of 10/8/07 and the morning 10/9/07 (mine on idle status) one shift per day five miners keeping the mine rehab. 1st sourth panel-no. 1 entry 4 x cuts from face of no. 1 unit 120 ft long 19 ft wide and 5 ft high. spad no 6+08.

A rock fall occurred on #2 unit at 5+10 in the 1st south panel. The fall was approximately 80 x 19 x 6. Unit was moved to next set of intake rooms.

A rockfall occurred @ 1pm on the #2 Unit in the 3rd West Panel. The fall blocked our secondary escapeway. The fall was between #3 & #4 entries, extending to #5 entry. The fall was 10x19x9. Msha was notified within 15 minutes.

#2 Unit had a rockfall inby the dumping point in the #5 room at 2+85 to 3+20. The face was 120 feet inby the crosscut. There was a slip. The fall was in the pillar line.

Employee stated he was beside pinner in the #6 entry on #2 unit when a piece of rock fell striking rt leg between knee & ankle. The rock caused 2 fractures (1 6 inches above ankle on right side of leg & 1 3 inches above ankle on left side of leg.) The rock was 2.9 feet wide x 4.8 feet long x 6 inches to 1 inch thick.

EE was splicing a blown waterhose when the band broke off in the banding machine. He got another bander and finished fixing the waterhose. He took out his channelocks and tried to pull the broken piece of band out of the machine. When the piece came out it struck his right thumb causing a laceration that needed 8 sutures. He had removed his gloves because they were wet & slippery.

EE twisted knee 1-10-05 after going to Dr and not missing any work it was decided to do some cleaning of knee. Diagnosis was osteoarthritis of knee, but an MRI showed tear which was repaired on 3-3-05 while knee was being checked. EE's first day to miss was 3-3-05 original injury was 1-10-05.

Ee stated back had been hurting for about a week after rock fhit in shoulder. took to hospital having muscle spasm did not miss any work with back until 7-13-04-went back to dr with muscle spams and taking physical therapy.
