EE WAS RIDING 15USU PICKUP OUT OF MINE AT END OF SHIFT THE TRUCK HIT A BUMP AND EE HIT HIS HEAD ON ROOF HE WENT HOME AT END OF SHIFT BUT LATER THAT MIGHT HIS NECK STARTED TO ACHE. HE CAME TO W ORK NEXT MORNING AND DECIDED TO HAVE THIS NECK LOOKED AT BY A DR RETURN TO WORK WITH NO RELEASE AND WAS SENT HOME TO GET RELEASE FOR FRIDAY DEAN CAME BACK TO WORK.

EE STATED THAT HE WAS WELDING ON THE HEAD PULLEY OF THE PLANT FEED BELT WHEN A HOT SPARK FELL ON HIS RIGHT AM BETWEEN WRIST AND ELBOW, CAUSING A SMALL BURN.
