Showing all 34The employee stated that he was working on a motor that was suspended above a walkway. The motor fell, striking the employee's foot and collapsing the walkway.
EMPLOYEE WAS PICKING UP A BAG CONTAINING COAL USING HIS LEFT ARM WHEN HE STRAINED A MUSCLE IN HIS LEFT ARM.
The employee stated that he was walking on a barge when the barge cable recoiled and hit him under the right arm, knocking him into the loaded barge. The employee did not begin missing work for this injury until 1/14/2010.
While trying to dislodge the frozen pulling lashing from the loaded barge, something ripped loose and popped in my right shoulder socket.
Employee was pulling a barge down river when he felt a pain in his lower back.
Employee was pulling a 25 HP motor on a sloped catwalk. The motor started to slide backwards. The employee jerked on the motor. The motor pulled and strained his muscle in his right shoulder. Rest of shoulder was ordered.
EE was burning a jig elevator bucket that was stuck in the chute. A piece of slag entered under his safety glasses and burned his lower left eye lid. EE rubbed his eye enough to scratch them. Drops were applied to his eyes. He missed one day, 1/3/08.
Employee was looking for a suction hose "6". He stepped on a patch of ice and fell. He stabbed his middle finger on his right hand. He struck his left knee on a pallet. He tore ligament in his left knee when he fell.
Employee was greasing the Wasco hilift. He was using a lever action grease gun. A bucket pin was hard to take grease. He was pulling the lever towards him, with the body of the grease gun resting on his chest. Another employee assisted him pulling the lever. The force against his chest cracked a rib on his upper right chest near his sternum.
The employee stepped out of the scale room and slipped on the bar grating twisting left knee.
Was working under a WA500 hilift laying down with hat off. He started to get out and stand up and hit head on garbage dumpster cutting his head which became infected.
Employee was prying on the tail gate of the Mack truck with a slate bar. Tail gate was frozen. He experienced pain in his abdominal muscle. On 3/9/07 he informed me he had a lump in his abdomen and was scheduled for an MRI on 3/13/07. A hernia was detected. He was scheduled for surgery on 4/9/07. His 1st day missed was 4/9/07.
Employee was removing 12.00 x 24 tire from a truck. He tried to lift a tire, he strained his lower back, left side. He worked the rest of the day. On Monday he had severe pain in his back and did not report to work. On Tuesday he came to work but could not punch in. He went to the chiropractor.
Employee stepped out of the scale room onto some ice covered grating and slipped twisting his ankle as he fell. His leg was bent under his body. He tore ligaments our the left and right side of his right ankle.
EE was pulling 1 1/2" hose around a 980G loader. He pulled it around the front of the bucket. He stepped on some mud & hose got caught under the bucket(mud was from washing the 980G). He slipped to the ground & landed on his right forearm. His head hit his shoulder and strained the muscles in his neck. He is taking therapy under Dr's care. He worked on 1/15/07 & left early.
Employee and a mechanic were in the cab of the loader trouble shooting an electrical problem. They were traveling down the road and hit a bump. The employee hit his coccyx bone on the cup holder in the cab. It bruised and fractured his coccyx.
After glancing off the second truck he collided with the rear of a tractor trailer owned by contractor that was parked immediately in front of second truck. The back of the trailer was knocked over the hill turning the tractor sideways the 350 truck continued on striking the tractor in the sleeper section& rear tandems. Truck pushed tractor on R side pushed down hill 30 feet.
Delivery person went to pick up 3 panels of bar grating by Coreco employee driving forklift. Panels slid off forklift hitting contractor in legs, knocking him off the truck with a panel falling on him.
Employee picked up a 4 ft section of 6" schedule 80 pipe, twisted his back and pulled a muscle in his lower back.
EE WAS REPLACING B-1 SCREEN PANEL WHEN THE PANEL BECAME WEDGED IN BOTTOM DECK. EE JERKED ON SCREEN, STRAINING HIS BACK, LOWER BACK. EE CONTINUED WORKING FOR THE FOLLOWING 2 WEEKS. ON 7/25/04 EE WENT TO THE HOSPITAL - PAIN HAD INCREASED TO THE POINT OF UNBEARABLE. X-RAYS WERE TAKEN: SHOWED BULGING DISC IN L-3. FIRST DAY MISSED WAS 7/26/04.
Employee was fabricating a reinforcing mat of rebar for concrete. He was burning a piece to size when a spark went under his safety glasses and went into his eye in the pupil area, becoming lodged.
Employee was attempting to grease pins and bushings on the boom of a Hitachi 400 backhoe. He was walking on the ice-covered boom when he slipped and fell off the boom, falling to the ground. When hitting the ground his right foot landed in a rut and twisted his right knee towards his left knee, pulling tendons and cartilage in his right knee.
CLIMBING OUT OF FRONT DRILL CAB, STEPPED ON A FROZEN TIRE RUT. ANKLE TURNED SIDEWAYS.
EE SLIPPED OFF OF SIDE OF 350 PAYHAULER WHILE ATTEMPTING TO REPLENISH BRAKE FLUID. EE FELL BACKWARDS TWISTING RIGHT KNEE IN TAILGATE OPENING STRUCTURE EE PULLED LIAGMENTS IN KNEE AND LOWER RIG HT LEG WAS X RAYED AT HOSPITAL THE KNEE WAS HYPEREXTENDED.
EMPLOYEE LOST HIS BALANCE AND FELL OFF TRACK FRAME.
EE WAS PUNCHING HOLE,1/2",IN RUBBER CONVEYOR BELTING USING AN ARCH PUNCH. EE STRUCK HIS LEFT HAND WITH A 3 LB SHOP HAMMER WHILE HOLDING THE ARCH PUNCH. HE STRUCK HIMSELF ON THE FIRST FINGER BE HIND THE FIRST KNUCKLE AND THE REAR AREA OF HIS THUMB, CAUSING SWELLING AND TENDERNESS.
FELL OUT OF BOBCAT LOADER WHEN FOOTING WAS LOST.
EE WAS CUTTING METAL WITH CUTTING TORCHES. HOSE OF TORCH BECAME TWISTED. WHEN IT QUICKLY AND SUDDENTLY BECAME LOOSE, IT BURNED THE LEFT SIDE OF THE EE'S FACE.
INJURED EMPLOYEE WAS REPLACING AN END ON AN ELECTRIC HEATER. WHILE USING AN ELECTRICIAN'S KNIFE,THE KNIFE BLADE DID NOT LOCK AND CLOSED ON INDEXFINGER OF LEFT HAND.
EE STOOD UP AND HIT HEAD ON PIPE.
WHILE REMOVING BOLTS FROM A DEWATERING SCREEN, THE WRENCH BEING USED SLIPPED AND CAUSED EMPLOYEE TO FALL BACKWARDS AND INJURED HIS BACK AND/OR NECK.
THE ENGINE STALLED IN A LOADED ROCK TRUCK DRIVEN BY EE. IN THE INTERSECTION OF THE MAIN HAUL ROAD. EE IMMEDIATELY RESTARTED THE ENGINE BUT DID NOT HAVE ENOUGH TIME TO AVOID HITTING ANOTHER VEH ICLE ON THE HAUL ROAD.
VICTIM WAS REMOVING PIN FROM A CHAIN WHILE STRIKING PIN WITH HAMMER A PIECE OF HAMMER BROKE OFF AND PIERCED THE BACK OF HIS HAND.
