While replacing a blower motor on a Komatsu D65 Dozer in the repair shop, employee was standing on the right track of the dozer. The access panel of the floor board was on the dozer deck leaning on the open cab door. The access panel slid off the deck hitting employees right outside work boot.

While installing a new piece of C Channel on the Prep Plant, employee pushed on C Channel, employees left pinky finger was caught between C Channel and wall of Prep Plant. C Channel cut through their glove to finger, resulting in 1 1/2 cut to left pinky finger, requiring 5 stitches.
Two employees working in a 125 foot man lift, replacing tin siding on Prep Plant. Injured was holding tin, man lift lowered, when stopped it bounced, causing injured employee's right hand to hit safety rails on man lift. Top of tin cut right middle finger.
Injured was using a sanding wheel on grinder to prep hand rails to be painted inside Prep Plant.

Employee was spotting new rolls or (cans) for suture installation along side BYC5 conveyor near the #2 feeder. While attempting to place a new roll or (can) on the BYC5 conveyor structure, his right arm came in contact with the top roller, pinching his right arm between the top outside roller and the conveyor belt. The employee was admitted to the hospital for observation only.
The employee was walking to his articulating truck (unit #5368) to start his shift and slipped on the snow/ice covered ground. The employee injured his left ankle in the fall.

After fueling Komatsu 155 dozer, the employee was remounting the dozer when he reached and missed the handrail grab. He slipped and fell, hitting his head on the dozer track. The employee has a laceration just above his nose on his forehead along with a couple of scratches on his nose.
The employee was seeding the bank at the refuse area. He slipped and hit his head on the rocks. He was unconscious when he was found. He had a scraped forehead and nose. He could not remember the events that led to the injury.

This incident is being reported as an occupational illness. The employee runs heavy equipment on a daily basis. The employee began experiencing pain in his neck, back, and legs. He sought medical attention for the pain, which was determined to be a degenerative change in his vertebrae. The employee began missing work on January 10, 2011.

The injured stated that he and another employee were cleaning the raw coal bin. The coal fell down, and the employee leaned back to catch his balance. He fell, striking his tailbone and lower back. He did not begin missing work until 9/16/2010.
The employee stated that he was injured when he was taking apart a water hose nozzle in the plant with two large pipe wrenches. He states he had one on the floor and was pulling on the other for about 20 or 30 minutes trying to fix a leak. The employee did not notify the employer of his injury until 3/11/10. The employee began missing work on 3/30/2010.

Injured employee was assisting in moving a 200' stacker under construction. He was holding a cribbing block against the stacker to allow the forks of an Ingersoll-Rand Telehandler to rest against. As pressure was applied to cribbing block, it broke, pinching his right middle finger between the stacker and the block.
The injured was pushing raw coal to the plant's raw coal feed bin. He drove too close to the edge in front of him. As he began to back up, the loose coal under the dozer tracks gave way, causing the machine to nose dive. The blade struck the hard packed ground, launching the injured forward into the windshield area and resulting in the injury.
The employee was standing on the top of the heavy media cyclone pump overflow box. During this time a fellow employee was tramming the overhead hoist across the plant. The frame of the overhead crane struck the victim, wedging him against a six inch plastic pipe. The six inch plastic pipe broke from its restraint, causing the victim to fall to the floor below.
Injured employee removed the quick-threader attachment from a pipe threader. As he went to place it on a work bench the swivel handle from the quick-thread attachment swung around striking injured employee in the groin. Injured employee did not experience much pain until later in the day.
While performing maintenance on #1 primary CMI, the injured employee was rolling a flight assembly on the floor. A fellow employee was attempting to separate a flight assembly from a hub. The assembly slipped, rolling into the assembly the injured employee was rolling. The injured pinched his right thumb between the two assemblies.
Employee was attempting to pry a board off of a new pump grate with a chipping hammer. Employee struck the chipping hammer with a three pound mini sledge hammer, causing a small piece of the chipping hammer to enter his left hand. Please note: Employee did not begin missing work until 4/7/2009.
The employee was replacing vee-belts on the M-5 conveyor. While attempting to roll the vee-belts on, the torque arm moved out of its bracket, causing the employee to lose his footing. He then fell from his position, striking his groin area on either the torque arm hook or torque arm bracket. He fell approximately 8 feet landing in some loose material on the ground.

Employee was pulling a 25 HP motor on a sloped catwalk. The motor started to slide backwards. The employee jerked on the motor. The motor pulled and strained his muscle in his right shoulder. Rest of shoulder was ordered.
EE was burning a jig elevator bucket that was stuck in the chute. A piece of slag entered under his safety glasses and burned his lower left eye lid. EE rubbed his eye enough to scratch them. Drops were applied to his eyes. He missed one day, 1/3/08.

Employee was looking for a suction hose ""6"". He stepped on a patch of ice and fell. He stabbed his middle finger on his right hand. He struck his left knee on a pallet. He tore ligament in his left knee when he fell.
Employee was greasing the Wasco hilift. He was using a lever action grease gun. A bucket pin was hard to take grease. He was pulling the lever towards him, with the body of the grease gun resting on his chest. Another employee assisted him pulling the lever. The force against his chest cracked a rib on his upper right chest near his sternum.
The employee stepped out of the scale room and slipped on the bar grating twisting left knee.
Was working under a WA500 hilift laying down with hat off. He started to get out and stand up and hit head on garbage dumpster cutting his head which became infected.
Employee was prying on the tail gate of the Mack truck with a slate bar. Tail gate was frozen. He experienced pain in his abdominal muscle. On 3/9/07 he informed me he had a lump in his abdomen and was scheduled for an MRI on 3/13/07. A hernia was detected. He was scheduled for surgery on 4/9/07. His 1st day missed was 4/9/07.
Employee was removing 12.00 x 24 tire from a truck. He tried to lift a tire, he strained his lower back, left side. He worked the rest of the day. On Monday he had severe pain in his back and did not report to work. On Tuesday he came to work but could not punch in. He went to the chiropractor.
Employee stepped out of the scale room onto some ice covered grating and slipped twisting his ankle as he fell. His leg was bent under his body. He tore ligaments our the left and right side of his right ankle.
EE was pulling 1 1/2"" hose around a 980G loader. He pulled it around the front of the bucket. He stepped on some mud & hose got caught under the bucket(mud was from washing the 980G). He slipped to the ground & landed on his right forearm. His head hit his shoulder and strained the muscles in his neck. He is taking therapy under Dr's care. He worked on 1/15/07 & left early.

Employee and a mechanic were in the cab of the loader trouble shooting an electrical problem. They were traveling down the road and hit a bump. The employee hit his coccyx bone on the cup holder in the cab. It bruised and fractured his coccyx.
After glancing off the second truck he collided with the rear of a tractor trailer owned by contractor that was parked immediately in front of second truck. The back of the trailer was knocked over the hill turning the tractor sideways the 350 truck continued on striking the tractor in the sleeper section& rear tandems. Truck pushed tractor on R side pushed down hill 30 feet.
Delivery person went to pick up 3 panels of bar grating by Coreco employee driving forklift. Panels slid off forklift hitting contractor in legs, knocking him off the truck with a panel falling on him.

EE WAS REPLACING B-1 SCREEN PANEL WHEN THE PANEL BECAME WEDGED IN BOTTOM DECK. EE JERKED ON SCREEN, STRAINING HIS BACK, LOWER BACK. EE CONTINUED WORKING FOR THE FOLLOWING 2 WEEKS. ON 7/25/04 EE WENT TO THE HOSPITAL - PAIN HAD INCREASED TO THE POINT OF UNBEARABLE. X-RAYS WERE TAKEN: SHOWED BULGING DISC IN L-3. FIRST DAY MISSED WAS 7/26/04.
Employee was fabricating a reinforcing mat of rebar for concrete. He was burning a piece to size when a spark went under his safety glasses and went into his eye in the pupil area, becoming lodged.
Employee was attempting to grease pins and bushings on the boom of a Hitachi 400 backhoe. He was walking on the ice-covered boom when he slipped and fell off the boom, falling to the ground. When hitting the ground his right foot landed in a rut and twisted his right knee towards his left knee, pulling tendons and cartilage in his right knee.

EE WAS PUNCHING HOLE,1/2"",IN RUBBER CONVEYOR BELTING USING AN ARCH PUNCH. EE STRUCK HIS LEFT HAND WITH A 3 LB SHOP HAMMER WHILE HOLDING THE ARCH PUNCH. HE STRUCK HIMSELF ON THE FIRST FINGER BE HIND THE FIRST KNUCKLE AND THE REAR AREA OF HIS THUMB, CAUSING SWELLING AND TENDERNESS.
