Employee felt something in the right eye and went a mirror to find a foreign debris in the right eye. Employee asked for assistance from another employee to flush eye. The employee was taken to a local optometrist and prescribed antibiotics

While checking the tightness of the belt on the skid steer, IE had ahold of the belt when other employee tried starting the skid steer, it pulled their right index finger into the pully and pinched it between the pully and belt.

IE stated that IE was trying to unplug a chute in the prep plant screen building using a scaling bar. IE was attempting to bust up material from the chute working upwards. The debris came loose falling below. The weight of the material sloughed down the chute catching the bar. The bar springboarded upward striking the IE on the left jaw, causing a laceration requiring 13 sutures.

IE was installing a roof bolt. While holding the bolt in the drilled hole, the bolt inadvertently moved, bending the bolt causing EE's hand to get caught between the drill chuck and the drill guide. This caused a laceration that required sutures.

IE went to the tailgate side of shear, opened the cabinet door to run the tailgate cowl off the valve bank While operating the cowl, the cabinet door closed striking their right forearm. IE was diagnosed with a hairline fracture. They returned to work their next shift

IE installed a washdown pump in the wash plant. IE stated they had opened the valve for the incoming water to the pump when impeller housing broke apart. The 4" inlet pipe came towards the employee striking the employee in the knee. On the morning of 10/6/2024, Employee began missing work on 11/12/2024.

Employee was unthreading a water hose from the wet dust truck while the hose was stretched. When the hose was undone the water hose sprung back striking the employee in the cheekbone causing a contusion just under the right eye. After further evaluation from an ophthalmology specialist, IE was diagnosed with a closed fracture of the right orbital floor on 7/9/2024.

Two bolter operators had finished bolting the mine roof, backed the roof bolter up and started to hang a section of exhaust tubing. A large piece of rock fell out from the rib striking one employee on the leg causing a deep tissue contusion. IE was unable to return to EE's normal work duties. EE was placed on restricted duty restrictions due to weight bearing on the calf muscle.

IE was moving steel pipe on a metal pallet that needed to be rearranged to a smaller length pipe. The stack gave way. Subsequently, the IE's hand was caught in a pinch point resulting in their left thumb being injured. The employee was wearing impact gloves at the time of the incident and was taken to the Hospital for further evaluation.

A non-injury accident occurred. An unplanned roof fall occurred at the 6 East belt 26+01 crosscut, #1 entry intersection. A breaker row of timbers were set inby and outby the roof fall in # 1 entry. A breaker row was set in the intersection of the fall on the # 2 entry side of crosscut 26+01. A rehabilitation plan was submitted to MSHA on 08/05/2023 and approved.

While bolting in 3 entry on 406 bolter the bolter helper was setting the ATRS when the left side bolter operator boomed in and smashed the helper's finger between the frame over the levers and the bolter boom tray. Left middle finger is the injury.

IE was bolting on the right hand station. IE was bolting in the roof on a corner at an angle. IE had the drill box all the way up. On the way back down IE had the left foot below the drill box. Once IE got the drill box toward the bottom IE smashed the foot. IE has a broken left, big toe. IE is in a boot and on crutches. At this point IE is seeking orthopedic care.

Crew was loading a techseal bag (4000lb) on to the teachseal platform. Two of the upper straps broke. The crew was repositioning to use the lower straps. A strap pinched the right pinky finger of the IE, causing a crushing laceration to the finger. IE received 5 stitches.

Grating had been removed from a floor drain. The IE could not see the drain that was full of water and had stepped into it causing a sprain/strain to their left knee.

Responsible Person (RP) was conducting a pre-shift examination. RP was traveling into 7E - 2E- XC 39+75. When the RP noticed a substantial roof fall above anchorage. RP immediately called management. Management immediately notified MSHA through the 800# and called the local MSHA field office.

Employee was walking on the second floor of the train load out to trace out a strip of heat tape for the load out waterline. The employee tripped over a used motor and pump that was out of service sitting on the floor. The employee landed on elbows causing pain to left elbow and shoulder.

Injured Employee (I.E.) was tramming a 12CM12 continuous miner (company #317) into the face and positioned on the right side. I.E. was found pinned between the continuous miner tail and the rib. Current known injuries: 7 broken ribs (right) 4 broken ribs (left) and a collapsed lung. I.E. is stable.

EE was walking through Surface Weld Shop when they slipped in a puddle, catching themselves with right arm. EE did not experience any pain as a result of the fall until next morning when they began to notice pain in shoulder. EE was referred to an Orthopedic and continued to work until 08-30-19 when they had surgery to repair a torn rotator cuff.

Employee rolled EE's ankle on a piece of coal but finished EE's shift. Sought treatment on 5/8 after walking on it for a week, was released but was referred to orthopedic for closed fracture of fibula, saw ortho on 5/14 and was put in walking boot and taken off work.

IW was assisting in installing a draw bar on the pump monorail. IW was helping hold the drawbar in place as the trolley was pushed into position. IW had hand on the monorail for balance when the trolley wheel pinched IW's finger.

IE was setting up to do an alignment on a motor to pump coupling on 5mn xc 25. as ee was walking around the pump skid ee stepped off the pump skid rail, rolling foot outward. IE sustained a fracture to the 5th metatarsal bone and missed next scheduled shift on 11/14/18 making this reportable at that time.

Employee was diagnosed with bilateral carpal tunnel and had surgery on 11/12/18. Surgeons work related determination was not received by the company until January 7th, 2019. It was at this time that we accepted the claim and are filing the report for the occupational illness.

Fire Boss bumped IE's head on a pipe; later in the shift began to experience vision issues, i.e. spots/floaters. IE had a broken blood vessel in the eye which in turn caused a torn retina. It appeared to be a result of the bump to IE's head. IE had laser surgery to repair the retina on 11-2. IE has unrelated existing retina pathology in other eye.

Employee reported ongoing shoulder pain in August, but has no specific event or incident. IE had surgery on 10/25/18 for repair of the bicep tendon, rotator cuff and labrum. Clinical findings were bicep tendonitis, AC joint arthrosis, fraying of the distal supraspinatus tendon, etc. Surgical report received 11/8/18.

Injured worker was involved with a slip, trip and fall incident in which the IW fell striking EE's head on the mine floor. IW was taken to the emergency room and immediately released to full duty. IW remained working, but continued to experience symptoms related to the injury. IW's first loss time date was on 10/25/2018.

IE was cutting a hole in belt to put rope through to pull belt. EE was cutting right to left. Left hand was on the belt, knife was in the right. The left hand was cut on top of the hand when right hand continued it's direction of travel after knife blade broke. IE was wearing EE's cut-proof gloves at the time of the incident. This was submitted in Oct., but it's not on the DRS.

A roof fall occurred in the #1 entry, 8 East section, at crosscut 31+25 extending approximately 70 feet outby. Approximate dimensions were 70 ft long by 22 feet wide by 6 feet high. The area was bolted with 6 foot long resin encapsulated rebar bolts.

Contract miner was injured while cutting rib mesh away from a bolt. The employee was outby the stageloader on the off-walkway side. Miner was struck in the arm and hand by a piece of rock that had fallen from behind the mesh. The miner received two broken bones in the hand requiring medical treatment.

IE was climbing down from belt structure during the section belt move. IE had both hands on the structure chain & stepped off the structure. The IE contacted 2nd chain & hook caught on the inner seam of pant leg. IE stepped off the structure the hook caught IE. IE transported to the ER for treatment and received sutures to close the wound.

IE was bolting in face. As IE was drilling IE's drill bit would not make a hold due to angle and was just spinning off of desired mark in a downward direction. As IE did IE's drill wrench came loose out of drill chuck and due to angle of drill steel caused the wrench to be propelled at IE's station, striking IE in the face on lower jaw.

IW was roof bolting. IW was holding the bolt with the plate above hand and just above the stationary portion of the jaw assembly. As the IW fed the bolt into the hole, the bolt caught on a ledge in the hole causing the bolt to bend smashing the IW's right little finger between the bolt and stationary portion of the jaw assembly.

A plant operator was helping lift a piece of HDPE pipe to reconnect the bent sieve circuit. Another employee was lifting the pipe but was having difficulty getting it into place. The IE employee stepped onto the top of the bent sieve bin and at this time, the pipe shifted, knocking the IE off balance causing IE to fall.

IE was installing roll pins in the flights in the feeder. IE was driving them in with a hammer and missed striking IE's left thumb, which was holding the pin with the hammer. The blow caused the thumbnail to be removed from the cuticle and exposed. The IE received sutures.

IE attempted to catch the drill steel with right hand and the drill steel struck 4th digit finger. IE informed supervisor. Once IE arrived home after shift, IE removed first aid gauze and sought medical treatment since laceration would not stop bleeding. The emergency room diagnosed IE with a laceration and fracture to the tip of IE's finger.
