The fall was discovered at 11:20 am on 3/3/08. It measured 5' high x 20' long and 18' wide. It was timbered off. The fall did not interupt ventilation. It was called in to both Federal and State officed at 1:00 pm. It is located at spad 1801 on the old 004 section.

Replacing cover on Joy miner. Ratchet slipped off of bolt being tightened and EE's elbow (left) hit the rub rail. Elbow struck rub rail located behind EE.
The employee finished his shift (owl) and was on the haul road going home. The coal trucks were on the same road going to the mines. The two vehicles collided and mine employee's car was hit on the driver's side by the bumper of the coal truck.
Employee was moving miner cable out of roadway and stepped into a hole and felt pain in back and hip.
EE was cutting bushing for pin on backhoe, when the grinder hung up and kicked back, striking EE below right elbow. Required 2 stitches just below right elbow.
Employee was shoveling belt and bent over to pick up a rock and felt a pain in his abdomen area.
Occupational illness - Pneumoconiosis.
Employee was in scoop bucket getting ready to ride to face area. He had hand on top of ram scoop. Operator got in scoop and activated ram lever not knowing injured had hand in pinch point location.
Crew were taking a belt drive apart. The boom was being held up with come-along while pins were removed. Chain broke letting boom fall down against drive frame catching employee's hand.
Tightening wheel on truck with lug wrench, strained abdominal muscle.
The employee was lifting a CLA for a joy miner, with the help of another employee when he felt a sharp pain in lower back.
Employee was taking air reading on top overcast. Coming down steps on overcast he midjudged last step causing him to fall backward hitting on his lower back and tailbone.
EE was loading miner bits on flat car to take into the mine. He twisted around and felt pain in right arm, shoulder & neck.
Was lifting a rub-rail on a joy miner, felt pain in lower back.
Employee walked between line to hang halo spray bar. He started to rehang halo spray. When entering spot to reach chain, caught right boot toe on toe plate. Felt some knee pain but no medical attention needed at that time.
Employee was helping change batteries on scoop. He reached around back of battery to release chain that holds the battery to the stand. His left shoulder hit left rib, causing coal and rock to roll from rib, striking left side of his body, leg, ribs and shoulder.
The employee was positioned at the rear of the section feeder observing a stalled feeder chain. A parked coal hauler rolled down a grade, catching the EE between feeder & hauler resulting in injuries requiring amputation of right foot & ankle & fracturing the left leg. The accident is still under investigation.
Operator was spot bolting roof bolts, installed bolt while waiting on other drill man. He reached to get an 8 oz of drinking water. Pulled his glove off to pull tab, small pieces rock fell, striking the top of his right hand. Laceration causing cut (8 stitches in top hand).
Fall of roof at spad# 1028, extending 50'outby. The fall was 50'long, 18'wide & 6'high (horse-back). The fall did not interfear with ventilation. The area will be timbered & cribbed off.
Employee was supervising removal of slide plate. Was attempting to hook 2nd come-a-long to "I" Beam when hooked come-a-long slid from "I" Beam falling approx. 4' hitting employee on right hand pointer finger causing possible fracture of finger.
Roof fall existing in old 1st left panel of northwest mains SS #347. Fell additional 30' past SS #847 thru intersection. This is a gob area with no personnel required to walk thru.
Was installing roof bolts when roof fractured thin scale of draw rock causing to break free, hit employee's left hand and cutting left ring finger causing to have 2 stitches in finger.
Roof fall on 1st left off NW mains. 2 breaks inby SS 844. Length-120 X 20 wide- 10' to 8' high. This is a gob area, no main airways were affected.
Was pulling miner cable out of roadway strain lower back.
Was riding rail runner to section. Got cramp in leg. Raised up, too high. Hit mine roof. Knocked him back down in rail runner, he hit frame of rail runner with left hip.
Was lifting car tounge up to move into position to couple up. Pain middle back.
Was moving a section of track 30' long when moved it slipped rail fell back striking employee on top of right foot.
The rock fall was found by forman at 2:15 am. Fall is in an entry to the right of no. 2 belt on the main line. The fall is 20' long, 20' wide & 6' high. Does not block any airways or escapeways. Fall was timbered off.
Employee was unloading metal roof support jacks from roof drill when jack pinched finger cutting top of finger at second joint.
Employee was starting to drill hole to install roof bolt when stab jack pin broke, letting canopy down and hit employee in head. (top)
Employee was going to set additional supports at ad fall that had occurred an 7-11-06 when he found this fall. the fall is 20' wide, 50' long. area was belt with 4' resin bolts.
Discovered roof fall outby bep #2 around 1:40 am. Fall was 1 break inby spad no. 887. Fall has been timbered and dangered off. Fall was approx. 20' wide , 50' long and 8' high.
Employee had changed batteries and put old batteries on charger. While raising lid on battery, slipped from his hand and struck left hand between lid and frame.
Discovered roof fall in primary escapeway measuring 50' long x 18' wide x 7' high. Area was timbered and dangered off.
Ee was climbing up stairs of forklift when his left foot slipped off of top step his left leg went under forklift. He was holding onto both handles when slipped. Right knee ligament was strained.
Miner foreman crawled out into intersection to look, swag was in area, ram car came over swag. Foremen did not see car. The car artickiled down, bucket end hit foreman's right leg.
EE was on his knee's in front of scoop unloading mega block from scoop bucket. EE twisted around to pick up block from scoop bucket and felt some- thing pop his knee.
EE was pulling a torgue shaft out of Conv. motor on Joy miner when pulled outer cover hot oil had built up behind the cap & expelled on right hand
Operating roof bolter, drill steel got stuck in hole. Operator reached out to hold steel and was going to hit the reverse rotation on drill. Instead he hit forward causing steel to come out and strike the back of the operator's hand causing a break in the left wrist bone.
Was loading supplies on drill or roof bolter. Small rock fell from mine roof approx 18" - rock 6" x 8" x 1" striking left hand on top. Simple fracture #3 finger bone on top hand.
While lifting rub rail to put back on Joy miner, he felt pain in left knee. First day out 3/7/06.
Was reaching out to get remote box that was on top joy miner when got hand on it a peice of rock 6"x8"x2" fell out mine roof striking R. hand causing oper to jerk box handle cause cut or tear to fingers.
Was making splice miner cable that had shielding. Pulled on shielding to cut. Utility knife slipped. Cut left thumb and web of left hand.
Drilling a hole to install roof bolts using a 6' steel, he used his wrench to torque bolts to drill a deeper hole and steel bent. He reversed the drill rotation and was holding the steel in his hand when he hit the reverse lever. Wrench came off steel, hit his left arm causing a simple fracture of the forearm.
EE WAS WALKING DOWN FROM LIGHT HOUSE TO MINE PORTALS AND SLIPPED ON FROZEN HAUL ROAD. HIS LEG WENT BACK UNDER HIS BODY - BROKE RIGHT ANKLE.
A roof fall in return on top end of 5th right panel near ss #880 approx. 100' x 20' x 6'.
EE WAS TAKING BOLTS OUT OF PLATE ON CRUSHER. HE WENT TO MOVE OUT THE THE WAY TO POSITION HIMSELF TO PULL THE PLATE. THE BOTTOM OF HIS SHOES CAUGHT ON THE BASE OF THE CRUSHER, RESULTING IN TWISTING OF LEFT KNEE. FIRST DAY OFF WORK 1/31/06.
Employee was duck walking around the drill when his left shoulder hit a bent roof bolt plate cutting ee's back near shoulder
During weekly exam of return a fall was encouted in old s. right panel on job side. Fall inby spad #868 40'x 30' 7' intersection.
