Caught in, under or between a moving and a stationary object

Crew were taking a belt drive apart. The boom was being held up with come-along while pins were removed. Chain broke letting boom fall down against drive frame catching employee's hand.

Over-exertion in lifting objects

The employee was lifting a CLA for a joy miner, with the help of another employee when he felt a sharp pain in lower back.

Fall onto or against objects

Employee was taking air reading on top overcast. Coming down steps on overcast he midjudged last step causing him to fall backward hitting on his lower back and tailbone.

EE was loading miner bits on flat car to take into the mine. He twisted around and felt pain in right arm, shoulder & neck.

Over-exertion in lifting objects

Was lifting a rub-rail on a joy miner, felt pain in lower back.

Caught in, under or between a moving and a stationary object

The employee was positioned at the rear of the section feeder observing a stalled feeder chain. A parked coal hauler rolled down a grade, catching the EE between feeder & hauler resulting in injuries requiring amputation of right foot & ankle & fracturing the left leg. The accident is still under investigation.

The rock fall was found by forman at 2:15 am. Fall is in an entry to the right of no. 2 belt on the main line. The fall is 20' long, 20' wide & 6' high. Does not block any airways or escapeways. Fall was timbered off.

Employee was unloading metal roof support jacks from roof drill when jack pinched finger cutting top of finger at second joint.

Employee was tramming the roof bolter up an incline and had his arm around the canopy post of the deck of the roof bolter. The roof bolter ran into the rib and cut his right arm.

FALL WAS APPROXIMATELY 70' LONG, 18'-20' WIDE, 8' HIGH. AREA WAS BOLTED WITH 4' BOLTS AND INTERSECTION 6' POINT ANCHOR.

EE HAD BEEN BUILDING AND PLASTERING BRATTICES WHEN HE BROKE FOR LUNCH. HE TOOK OFF HIS GLOVES, SLAPPING THEM AGAINST BLOCK. WHILE SLAPPING HIS GLOVES, AN OBJECT STRUCK HIM IN THE EYE.
