Crew were taking a belt drive apart. The boom was being held up with come-along while pins were removed. Chain broke letting boom fall down against drive frame catching employee's hand.

The employee was lifting a CLA for a joy miner, with the help of another employee when he felt a sharp pain in lower back.

Employee was taking air reading on top overcast. Coming down steps on overcast he midjudged last step causing him to fall backward hitting on his lower back and tailbone.

EE was loading miner bits on flat car to take into the mine. He twisted around and felt pain in right arm, shoulder & neck.

Was lifting a rub-rail on a joy miner, felt pain in lower back.

The employee was positioned at the rear of the section feeder observing a stalled feeder chain. A parked coal hauler rolled down a grade, catching the EE between feeder & hauler resulting in injuries requiring amputation of right foot & ankle & fracturing the left leg. The accident is still under investigation.

Fall of roof at spad# 1028, extending 50'outby. The fall was 50'long, 18'wide & 6'high (horse-back). The fall did not interfear with ventilation. The area will be timbered & cribbed off.

Roof fall existing in old 1st left panel of northwest mains SS #347. Fell additional 30' past SS #847 thru intersection. This is a gob area with no personnel required to walk thru.

Roof fall on 1st left off NW mains. 2 breaks inby SS 844. Length-120 X 20 wide- 10' to 8' high. This is a gob area, no main airways were affected.

Was pulling miner cable out of roadway strain lower back.

Was lifting car tounge up to move into position to couple up. Pain middle back.

Was moving a section of track 30' long when moved it slipped rail fell back striking employee on top of right foot.

The rock fall was found by forman at 2:15 am. Fall is in an entry to the right of no. 2 belt on the main line. The fall is 20' long, 20' wide & 6' high. Does not block any airways or escapeways. Fall was timbered off.

Employee was unloading metal roof support jacks from roof drill when jack pinched finger cutting top of finger at second joint.

Employee was starting to drill hole to install roof bolt when stab jack pin broke, letting canopy down and hit employee in head. (top)

Employee was going to set additional supports at ad fall that had occurred an 7-11-06 when he found this fall. the fall is 20' wide, 50' long. area was belt with 4' resin bolts.

Discovered roof fall outby bep #2 around 1:40 am. Fall was 1 break inby spad no. 887. Fall has been timbered and dangered off. Fall was approx. 20' wide , 50' long and 8' high.

Discovered roof fall in primary escapeway measuring 50' long x 18' wide x 7' high. Area was timbered and dangered off.

Was reaching out to get remote box that was on top joy miner when got hand on it a peice of rock 6""x8""x2"" fell out mine roof striking R. hand causing oper to jerk box handle cause cut or tear to fingers.

EE WAS WALKING DOWN FROM LIGHT HOUSE TO MINE PORTALS AND SLIPPED ON FROZEN HAUL ROAD. HIS LEG WENT BACK UNDER HIS BODY - BROKE RIGHT ANKLE.

A roof fall in return on top end of 5th right panel near ss #880 approx. 100' x 20' x 6'.

During weekly exam of return a fall was encouted in old s. right panel on job side. Fall inby spad #868 40'x 30' 7' intersection.

A fall was discovered on 4 Right Panel Northwest Mains near SS 767 at top of old panel in gob area near BEP-1. 20' x 30' x 8'+. Area cribbed off.

A ROOF FALL WAS DISCOVERED ON 8/29/05 ON 2ND SHIFT FIRE BOSS RUN ON #2 BELT ON NORTHWEST MAIN. 30'X20'X6' NEAR SS#96 IN A SECTION ENTRY

Employee was tramming the roof bolter up an incline and had his arm around the canopy post of the deck of the roof bolter. The roof bolter ran into the rib and cut his right arm.

A ROOF FALL WAS DISCOVERED WHILE MAKING RETURN ON WEEKLY EXAM. THE AREA IS ON 4TH RIGHT OFF NORTH ENTRY MAINS. 40' X 20' X 6'. NEAR SS#768 156' INBY THIS SS#. AREA WAS TIMBERED OFF.

A fall was discovered during preshift exam on 5 right off main line in # 6 entry at face of old panel. Fall was approx 20' by 20' by 7' and was dangered off.

A fall was found on the 1st left section off northwest main at ss# 847 - fall was 30' long by 20' w by 5' high.

A ROOF FALL WAS DISCOVERED WHILE FIREBOSSING MAIN RETURN OF THIS MINE AT 4 RIGHT OF NORTHWEST MAIN. THIS IS A CONTINUATION OF A FALL THAT OCCURRED 3/21/05. 20' WIDE, 60' LONG, 6-10' THICK. THIS FALL STARTED AT SS #773. FELL TO FACE.

A roof fall was discovered at 10:30 sunday morning during preshift exam 5 right of northwest mains 50' inby SS # 870. Fall approx 20' by 10' by 50' length.

A roof fall was found in the 4th right panel in the #4 entry approx. 3 break inby ss# 757 this is a continuous of the fall that was reported. 03-21-05 to msha 20'x20'x6' fall was discovered by ee pmce ee msha out 5. area has been timber off. this is in a gob area.

Roof fall of 2nd right northeast mains. at location area is in gob line.

Making airway, found fall in #4 entry at spad no. 772. This is area in gob area. 50' length, 20' width, 6' thick. Area is being timbered off.

WEEKLY EXAM WAS MADE 03/14/05, THE FALL WAS FOUND 03/17/05, 4TH RIGHT NORTHEAST MAIN APPROX 50' INBY SS 774. FALL WAS 50' LONG X 20' WIDE X 6' THICK. AREA WAS DANGERED OFF, DOES NOT BLOCK ANY AIRWAY NEEDED TO VENT GOBB AREA

A roof fall in 3 different location in 3 entenstion fell over a period of 7 clays between weekly, inspection ss# 631 ss# 616 ss #611 fall # 611 20' x 40' x 8' ss# fall #2 ss# 616 20'x40'8' fall #3 intersection 631 breadkline.

A ROOF FALL WAS FOUND IN THE NO. 7 ENTRY ON 3RD NORTHWEST MAINS AT BREAK #44 IN THE INTAKE ENTRY. FALL WAS 1 BREAK INBY SPAD #540. FELL FROM EDGE OF #61 ENTRY TO WALL IN THE #7 ENTRY. FALL WAS APPROXIMATELY 70' LONG, 18'-20' WIDE, 8' HIGH. AREA WAS BOLTED WITH 4' BOLTS AND INTERSECTION 6' POINT ANCHOR.

WHILE TRAVELING AIR COURSE ON WEEKLY EXAM, FOUND FALL AT #6 ENTRY, BR #50, 100' OUTBY SS 565. 30' LONG X 18' WIDE X 5' HIGH. APPROX. 1600 OUTBY SECTION.

Employee was walking across section and slipped and fell on loose material. (Started missing work 10/18/2004).

ROOF FALL, 2-BREAKS OUTBY FACE 001 MMU. SS#642, 10'INBY. FALL 11'X 18'X 10'LONG.

A second fall occurred this date on 10-01-4 out by break #26 110 in by spad. 5' by 20' by 30'. Additional support put in location around area and beam and jack put in belt tract entry.

Employee was running roof bolter and started to drill a hole and a piece of draw rock fell from top cutting his face. (Required 6 stitches)

ROOF FALL OUTBY SS #72 APPROXIMATELY 2100' FOOT PORTAL, 3000' FROM FACE #3 ENTRY.

A ROOF FALL 150' INBY NO. 1 PORTAL 6'-8' X 16' X 60' WAS FOUND 7-31-04 9:30 pm AREA DANGERED OFF.

Injured had completed setting survey station (#555) in the face area when a piece of draw rock fell and hit him on his right shoulder.

Employee was running roof bolter and rock fell from top striking him on the back of right hand. Started missing work 6/28/04.

Employee was setting timbers and a rib roll came out knocking one of the timbers striking employee on left leg.

Employee was walking down belt line entry when a rib roll fell striking employee on left leg and foot. Broke both bones in leg and foot.

A ROOF FALL OCCURRED APPROX 6:30 AM IN THE #3 ENTRY WHICH IS A NEUTRAL LOCATION AT SS #76. THERE WERE DISRUPTION OF VENT CONTROLS. FALL SIZE 20'W X 30'L X 5'THICK.

#2 ENTRY NETRAL AT SS #133 5' X 16' W X 15' L DISCOVERED AT 12:40 PM.

#1 ENTRY LEFT RETURN SS #104 10' OUTBY. 6' X 20' W X 30'L.

A ROOF FALL MEASURING APPROX. 100'L X 20'W & 40-48'THICK. IN THE #6 ENTRY, 40'INBY SURVEY STATION 063, APPROX. 1400'FROM SURFACE, 4500'FROM ACTIVE FACE.

EE WAS WORKING ON A JOY SHUTTLE CAR WHEN THE CHAIN ON COME-A-LONG BROKE STRIKING EMPLOYEE'S LEFT HAND.

A ROOF FALL LOCATED IN #3 ENTRY (NUETRAL AT 55# 102 WAS DISCOVERED APPROXIMATELY 11:15 PM MEASURING 50'LX 20'W X 6'T NO EQUIPMENT OR INJURIES AFFECTED AND NO DISTURB ANCE OF VENTILATION.

EE HAD BEEN BUILDING AND PLASTERING BRATTICES WHEN HE BROKE FOR LUNCH. HE TOOK OFF HIS GLOVES, SLAPPING THEM AGAINST BLOCK. WHILE SLAPPING HIS GLOVES, AN OBJECT STRUCK HIM IN THE EYE.
