Employee was dislodging elevator buckets in #2 jig elevator shaft and inadvertently stepped into the opened elevator access repair area underneath the plant jig. Employee was able to catch himself before falling completely into area and was lowered to plant floor below by two employees. Injuries consisted of sprained right knee and lacerations on left elbow and right shin.

Showing all 14Employee was standing on flat spot the operator was pulling the pump hose over where the employee could retie the rope on the pump hose, then the rock that his left was on rolled and the employee fell. When he tried to block his fall with his left hand he hit his hand on a rock dislocating the middle joint and tearing the skin.
Area experienced a 10" to 11" rainfall event with 8" falling in the first 24 hours. This event followed 8.2" in feb. and 3.5" earlier in March. runoff overwhelled. 8" turbine pump in the sump at surface of drift mine even with the start of. 8" diesel backup. Water raised such that water entered the mine beginning at 2 pm on 3/18 and mine became inundation at 7 am on the 4th.
9/23 & 9/24 with reported 10" of rain (guage at mine went to 6") overwhelmed the sump at the drift mine pit and the 8" electric pump and power went off and the 8" diesel backup's clutch went out and water ran into the mine inundating it for the 300' it had advanced. Water roofed on left side 4th & 5th xcuts (#1 entry) and water 3000' at 4th xcut in #7 entry and face.
WHILE EE WAS WALKING TO SEDIMENT POND NO. 24 TO GET A WATER SAMPLE HIS RT FT SLIPPED AND WHEN HIS FOOT CAUGHT HE HYPEREXTENDED RIGHT KNEE.
EE INJURIED HIS BACK WHILE LIFTING A PART TO CAT 5230 BUCKET.
EE WAS GRINDING ON STEEL WHEN A PIECE OF STEEL FLEW IN HIS RT. EYE.
EE WAS DRAGGING A PULL CABLE AND PULLED A MUSCLE IN HIS BACK.
EE WAS PULLING FUEL HOSE UP ON #4 OIL WHEN HE SLIPPED ON WET TRACK HE CAUGHT HIMSELF PULLING A TENDON IN HIS RIGHT SHOULDER.
EMPLOYEE BACKED CAT 785 END DUMP TRUCK OVER BERM, THE TRUCK ROLLED OVER BACKWARD DOWN THE SPOIL.
EE WAS REMOVING RUBBER TARP STRAP. THE OTHER END CAME LOOSE AND STRUCK HIM IN THE MOUTHS.
EMPLOYEE FELL FROM REAR TIRE OF 16 G MOTOR GRADER WHILE FUELING, SURFACE WAS WET AND MUDDY.
EE LOST FOOTING AND GRABBED HOSE HILE BENT OVER CAUSING UPPER RUPTURE ON LEFT SIDE.
THE CREW WERE IN THE PROCESS OF PULLING LEFT SIDE TOP HOIST CYLINDER PIN, EE WAS IN A BASKET PULLING PIN WHICH WAS HOOKED ONTO HIS CABLE WHEN THE PIN CAME OUT IT FREE FELL ABOUT 3' THEN TH CAB LE CAUGHT THE PIN AND BASKET SHAFT BROKE. BASKET FELL TO THE GROUND AND THEN THREW EE OUT AND LANDED ON HIS BACK ABOUT 3-4" ABOVE HIS WAISTLINE.
