Employee was moving a drum when EE felt a pain is EE's stomach. The pain ended quickly and EE resumed work. On 12/22/2025 EE felt the pain again doing light work. EE later visited a doctor and was diagnosed with a hernia. It was reported on 01/06/2025. After investigating the incident it was determined to be work related on 01/29/2025.

An employee was opening the cargo doors on a van trailer to unload new empty drums. As the door was opening a drum fell from the trailer and hit the employee on the hard hat and caused a laceration on EE's forehead. There were 12 stitches required to seal the cut.

Was loosening bolts to replace cutter blades on a process filter. Employee was putting extra force on a bolt to break it loose when it released quickly and pushed EE's finger against the edge of the blade. It cut through EE's A6 gloves and left a 1" laceration on the outside of the index finger. It required 5 stitches to close the cut.

Employee was putting a driveline into a scraper; they had a chain come-along around the end to help push it into place and a 4X4 piece of wood between the come-along and the shaft. As the employee was cranking on the come-along, the wood slipped off and hit the employee in the nose, resulting in a laceration to EE's nostril.

Employee was diagnosed with CBD by Doctor at National Jewish Health in Denver, Colorado in conjunction with Doctor with Materion.

Employee was diagnosed with CBD by the Corporate Medical Director.

Heart Attack

When the ammonia line was charged, during the plant start-up process, the compression fitting holding the flow meter in the pipe failed releasing ammonia into the building. As the operator was evacuating the building the operator dropped from a mezzanine deck to the floor below. This drop caused lower-back pain.

Subject has worked in a beryllium mill for 39+ years. As of 8/7/18, employee was diagnosed with Chronic Beryllium Disease. Currently, employee is medically able to perform all of employee's regular work duties, without restrictions.

The operator was pulling drums off the rollers in Beryllium Hydrolysis when the operator twisted, overexerted, and strained back. Operator was seen by operator's PCP on 3/23/18 where operator was referred to Physical Therapy and prescribed an anti-inflammatory. Operator will report to work 3/23/18 on light duty.

Individual descended a flight of stairs, stepping on a hose at the bottom and lost footing. EE landed on back side. On 9/14/16 the individual reported discomfort in buttock. On 9/28/16 ee was examined by PCP and prescribed an order for an exam and treatment by a Physical Therapist. EE attended 1st PT session the following week.

Changing a broken strut on a Cat 775 haul truck, keeper bolts had sheared off and strut slid out striking employee on the right side and left leg.

Was cutting a piece of plastic off his drill's battery post with a pocket knife. His knife slipped and cut his left hand/thumb.

Employee received laceration above eyebrow while removing CV housing from front axle of vehicle. CV housing was difficult to remove, and suddenly broke free, striking above the individual's left eye. He was treated and released from the ER. Wound required 4 stitches.

Transport driver received concentration of sulfuric acid

Employee stepped on stainless steel lid covering an inspection port. The lid slipped off the opening, causing the right leg to fall into the opening resulting in a laceration above the knee on the back of the thigh, and a contusion on the upper shin. Treatment required stitches.

Employee was removing metal filings after performing lathe work that resulted in a laceration to the top 1/3 of the middle finger on the left hand. Employee was wearing leather work gloves at the time of injury.

Employee was descending from D9 Dozer when foot slipped on icy track. The employee's leg hit a bar and sustained a laceration to the inside right shin area (L=3cm; W=.5cm; required 5 stitches).Cold slick conditions caused a slipping hazard.

EE and some other employees were moving steel to relocate them in the process of cleaning up the area. He hadn't noticed that he cut his finger until a few moments had lapsed. He looked down and saw blood. Upon investigation, he discovered that he had cut his Right pinky finger.

EE was descending the ladder, one foot already on the ground and the other on the lowest rung, had just unhooked his fall protection harness and lost his footing and slipped.

Employee was connecting an air hose to the ball valve on the steam line. While making the connection, foreign material on the connector fitting flipped into the employee's eye. Employee went to the Dr on 10/03/12. Treatment indicated irrigation, antibiotics/steroids-topical antibiotic. No follow-up requested.

Water truck driver began to experience symptoms of heat stress. Was removed from truck and put into air conditioned environment and told to hydrate. Began to feel better and returned to work for remainder of day. After work drove for 45 minutes in personal vehicle without AC and began to feel ill. Went to hospital, where she was hydrated and released that evening.

Employee was grinding a 1/2" piece of metal. The metal was clamped in a vice. The grinder bound up or caught on something causing it to jump back, hitting the employee on the left wrist resulting in a laceration.

The employee was on the lower cat walk at the thickener area to repair an auto valve. When stepping across the pipes to the valve, his foot slipped between the pipes causing his right knee to hyperextend, spraining the knee. Incident went unreported until 12/23/2011

Upon completion of a pipe replacement the EE slid from the pipe rack to the cat walk. Once standing he twisted to move forward and felt a pop in the knee.

When changing the teeth on the 330 BL excavator bucket, using a small sledge hammer to loosen the teeth, a small metal shard broke loose from the tooth penetrating the assisting employee's shoulder.

While closing a 3" steam valve with a pipe wrench, in a sitting position using a pulling motion, strained the left shoulder.

Cutting 1" channel off of the top of the portable impact crusher hood.

Subject has worked in a beryllium mill for 31+ years. As of 4/28/09, he was diagnosed with Chronic Beryllium Disease. We are unsure of the degree of impairment at this time.

Employee was removing plates from CHX-1 (carbonate heat exchanger) with a pneumatic impact wrench. The wrench slipped & he tried to catch it with his free hand. A retaining wire on the socket caught his hand causing lacerations to right palm. He was taken to local hospital where he received 8 stitches.

Dehydration

Employee was making his usual rounds, began to climb down the staircase into Thickner Tunnel #8 and fell to the cement below. Fractured L1, L2, L3, Sacrum and skull.

Employee was working on the Metal Lathe in the Mechanics Shop. He tried to remove a metal shaving (filing) from the work area with his gloved hand. The filing cut through his glove and lacerated his right index finger. Required 8 stitches.

Employee entered the lunchroom to get a paper cup, slipped sideways on the wet floor (janitors were mopping at the time), fell forward, hitting her right shoulder on the Pepsi machine, right orbit (area surrounding eyeball) against the soda dispenser(metal). 2 Stitches required by physician to under eye area.

Coming down the stairs in between A & B hydrolyzer, toe caught on cement landing, twisting right knee.

Employee loading a 24' extension ladder into work pick-up, when he felt pain to the left shoulder. Doctor visit on 11-16-06, referred for MRI. MRI done on 11-18-06. No definite determination. Specialist appointment on 11-22-06. Dr. states,"Too early to make recommendation". Wants to see employee again first week of January 2007. No prescription meds. RTW light duty till then.

Employee was assisting maintenance with changing out a 6" stainless steel recycle line on A. Hyd-1. While trying to position a new line for installation, his fingers got pinched between the flange of a 6" X 11.5' stainless steel pipe (approx. 230# pipe) and a channel iron on the stairs.

Subject & co-worker working with vent pipe removal. Vent pipe improperly secured. Subject was struck by moving pipe which then caused him to fall to the floor. (Same level fall)

Subject and his foreman were attempting to move a 75# motor into a tight fitting area, between a support beam and a vent pipe. Subject was pulling on the motor trying to force it through the tight spot while his foreman was pushing. He felt strain to his left shoulder. Will be on work restrictions until approx Sept 7, 2005 after doctor visits.

EE WAS MOVING A BARREL OF PRODUCT WITH THE BARREL LIFTER. WHEN HE PUT THE BARREL ON THE PALLET, HE SAW IT WAS NOT SITTING ON THE PALLET PROPERLY. HE STARTED TO REPOSITION BARREL, AND PLACED HI S LEFT HAND IN BETWEEN THE METAL PART OF THE BARREL LIFTER AND THE RIM OF THE BARREL (IN A PINCH POINT) AMPUTATING THE TIP FROM HIS LEFT RINGFINGER TIP.

EE & OTHER EE WERE PREPPING THE BERYL FURNACE FOR START UP. AS THEY WERE DOING THE THINGS ON THE FURNACE PREP RECORD SHEET, THEY CAME TO THE PART WHERE IT SAYS "CLEAN UNDER THE FURNACE" TO DO THIS JOB, THE FURNACE NEEDS TO BE LIFTED UP & BLOCKED UP. THEN THE OPERATOR GETS DOWN UNDER THEFFURNACE & CLEANS THE RAM MIX & LARGE PIECES OF FRIT THAT HAVE FALLEN DOWN UNDER THE FURNACE. IT

EMPLOYEE NOTICED WHAT FELT LIKE AN EYELASH IN HIS LEFT EYE AT THE END OF HIS SHIFT MONDAY, 12/9/02. FLUSHED EYES WHILE SHOWERING. TUESDAY, 12/20/02 HE COULD STILL FEEL SOMETHING IN HIS EYE, BU T COULD NOT FIND IT. HE, THEN LEFT TOWN FOR WED & THURS. HE REPORTED TO THE DISPENSARY ON FRI MORNING 12/13/02. IRRIGATED LEFT EYE 3 TIMES. TRANSPORTED TO ER. DR REMOVED EMBEDDED METAL SPLINT

EE WAS SHEARING A PIECE OF 1/4 INCH STAINLESS STEEL PLATE FOR THE LAB. AS HE WAS SHEARING OFF A CORNER OF THE PLATE, THE FOOT ON THE SHEAR MACHINE CAME DOWN TO HOLD THE PLATE. HIS RIGHT THUMB WAS CAUGHT BETWEEN THE FOOT AND THE PLATE, AMPUTATING THE TIP OF HIS RIGHT THUMB APPROXIMATELY 1/2-3/4 INCHES.

EE WAS HOLDING THE END OF THE CAR WASH HOSE WHENTHE NOZZLE BLEW OFF. THE HOSE CLAMP ON THE HOSE SLID THROUGH HIS HAND, CUTTING HIS FINGER. LACERATION OF THE PROXIMAL PHALANX/LEFT HAND/POINTER FINGER/PALM SIDE. REQUIRED 4 STITCHES FROM DR.

ICY SURFACE & SNOWING OUTSIDE. EE WAS IN PROCESSOF WALKING TOWARDS THE BACKHOE TO FUEL IT UP. HESLIPPED ON ICE, TRIED TO CATCH HIMSELF, & SLIPPED AGAIN TWISTING HIS RIGHT KNEE. HE SAW DR 12/18 /01 WHO GAVE EE RELEASE TO WORK. KNEE CONTD TO HURT & EE WAS REF'D TO ANOTHER DR. 1/22/02. HE RET'D TO WORK WITH RESTRICTIONS: REF'D TO ANOTHER DR FOR 2ND OPINION 3/14/02 HAD SCOPE DONE &RTW

EE WAS MOVING A 7' DRISCO PIPE THAT WAS STANDINGAGAINST A BUILDING. HE FELT SOMETHING DROP INTO HIS RIGHT EYE. HE WAS WEARING SAFETY GLASSES AT THE TIME. HE CALLE A FIRST RESPONDER TO THE DISP ENSARY TO LOOK AT THE EYE AND HE NOTICED BLOOD IN THE CORNER OF THE EYE AND RECOMMENDED HE SEE ADOCTOR.

RANCHO EMPLOYEE WAS ON THE EL200 TRACKHOE RELOCATING PIPE. INJURED WAS OBSERVING AND HELPING LOEAD THE PIPE FROM THE GROUND. ONE PIECE OF PIPE (JUST THE END OF IT) WAS A LITTLE STUCK IN SOME M UD. WHEN THE TRACKHOE LIFTED THE PIPE, IT BEING STUCK CAUSED THE PIPE TO BOUNCE WHEN IT BECAME UNSTUCK...CAUSING THE OTHER END TO BOUNCE. IT SWUNG & STRUCK HIM IN THE ANKLE.

WHILE WORKING ON A 2" BONDSTRAND DRISCO PIPE, EEHAD PUT IN VICE HE ATTEMPTED TO POUND IT INSIDE A NOZZLE & IT FLIPPED OUT OF NOZZLE & HIT HIM IN THE MOUTH (LOWER LIP) TOOK HIM TO ER & DR PUT 3 STITCHES IN BOTTOM LIP

EE HAS HAD PROGRESSIVE PAIN IN THE RIGHT ELBOW, NUMBENSS IN RING AND LITTLE FINGER. HE FIRST NOTICED IT DURING SUMEMR SHUTDOWN IN SEPT. 99 BUT ASSOCIATED IT WITH AN OLD INJURY HE AHD APPROX 1 0 YRS. AGO. PAIN HAS REMAINED AND GOTTEN WORSE. HE WENT OT A DR. WHO AHS DONE SOME TESTING AND HAS FOUND MILD CARPEL TUNNEL SYNDROME AND BURSITIS IN THE RIGHT ELBOW. ATTEMPTING TO REMEDY WI
