Cone had bridged over, employee's were using pry bar to dislodge and clear out cone. Injured was using bar to pry rocks loose and clear a hole. The bar rebounded back and struck him in the palm of left hand.

Mostrando los 18EE was looking for a gas cap from a CAT 769C haul truck that had fallen off. He was standing on the engine compartment next to the operator's cab. He stepped down to look, lost his footing, stumbled backwards and then fell off the truck, approximately 10 feet to the ground. He landed on his neck/back. He worked for about 3 hours after.
Installing a new ware liner in shaker 3 collector discharge chute. Swinging 3lb hammer through opening in door to make sure ware plate was secure. Caught corner opening of door with hammer, causing it to ricochet over and hit right thumb. Thumb was broke - cracked by knuckle and chipped by joint.
Finished changing hydraulic vibrator on pep-shaker. Putting away tools - 1/2 inch break-over bar slipped through hands & caught top of right foot resulting in small puncture wound and swelling.
Temperatures were near or at 100 that day. Had been performing maintenance on the air conditioning on the loader. Then, had loaded 3 trucks. Then became dizzy, had shortness of breath, loss of control of movement and almost blacked out. Due to dehydration. NOTE: Late report due to possible non work related condition.
In process of lifting off top of cone crusher (using a crane), when cone came up, swung out and caught thumb between upper half of crusher and hand railing. Crushe end of thumb, ripped off thumbnail of right hand.

EE WAS USING A BOX CUTTER TO CUT A RUBBER HOSE; HAND SLIPPED AND CUT UPPER RIGHT LEG.
EMPLOYEE WAS TRYING TO FREE THE BLOCKAGE IN THE JEFFREY FEEDER. HE WAS HIT ON THE RIGHT TOE BY A 2-3" ROCK.
INJURED WAS WASHING THE FC CONE CRUSHER. CO-WORKER WAS DRIVING THE LOADER AND BACKED INTO THE MANLIFT IN WHICH HE WAS WORKING. HE WAS PUSHED INTO THE ROCK BOX. HIS RIGHT EYE, RIGHT ARM AND RIG HT LEG WERE INJURED.
EE WAS UNPLUGGING CHUTE WHEN A ROCK FELL AND HITTHE LEFT SIDE OF HIS FACE.
EE WAS POURING CONCRETE AT THE QUARRY. HE HAD RUBBER BOOTS ON BUT SOME CONCRETE GOT ON THE BACK SIDE OF HIS LEGS.
EE WAS USING A GRINDER ON THE SOUTH WALL WHEN A PIECE OF STEEL GOT IN HIS LEFT EYE.
EE WAS BREAKING BOULDERS WITH THE TRACKHOE IN THE OLD PLANT 2 AREA. A ROCK FRAGMENT CAME INTO THE CAB AND HIT HIS LEFT LEG ABOVE THE KNEE CAP.
EE WAS CLEANING UP WITH A SHOVEL UNDER THE C8 CONVEYOR OF THE CRUSHING PLANT. WIND BLEW DUST PARTICLES INTO EMPLOYEE'S FACE WHEN HE TRIED TO CLEAN DEBRIS OUT OF CONVEYOR. DUST PARTICLES FLEW I NTO EMPLOYEE'S LEFT EYE.
WHILE REMOVING DRILL STEEL FROM HOLE, COUPLER FELL OFF THE STEEL, BOUNCED OFF HTE CENTRALIZER AND UP INTO HIS JAW.
