Moving a piece of metal with pry bar the metal slipped and caused the bar to flip back striking EE on right hand causing a fracture on small bone between finger knuckle and wrist. Surgery sch. to put pin in bone.

Employee was installing cutting edges on Cat D11R dozer. He turned one of the edges over and it fell on a pry bar and the bar came up striking right lower leg causing lacerations that required 11 stitches.
Employee was helping air conditioner repairman change the coils on a/c unit in 777 rock truck. Repairman tossed a gallon tub of cleaner to employee and it hit the handrail and the lid came off and splashed into employee eyes
EE was climbing ladder to measure for the installation of a fitting in a water tank when bottom of the ladder slipped out and EE fell striking his forehead on a conduit bracket on the wall required 7 stitches.
EE was operating a chain saw cutting trees. A tree kicked back sideways and struck EE on left knee. MRI shown torn ligament. Surgery is being scheduled to repair ligament.
EE came in on 4/19/12 said he had been to Dr. and that the Dr. wanted to do surgery on back as the result of a fall on 3/1/12.EE missed work on 3/12 & 3/26 but did not report it due to an injury. On 3/1/12 EE stepped in a hole and fell backwards but did not miss any work or go to a doctor at that time. EE worked until 4/19/12. W.C.reviewing
Employee was assisting welder lift a piece of metal out of the back of truck. Metal weighed approx. 150 lbs. Felt sharp pain in lower back just as metal got to the ground.
EE was removing cap from air line and failed to completely turn off valve allowing air pressure to be under cap. When cap was removed it struck EE on hand. EE was operating Cat 994 endloader.

When the tree fell, a grapevine that was unseen in the top of the tree pulled a dead tree approx 6-8" in diameter down and struck EE, knocking him to the ground. When EE hit the ground, he fell on a rock that caused a laceration over right eye, requiring 7 stitches.
Employee was laying a 2" cable to pull a tooth onto a 994 bucket with the help of the customer's mechanic. The customer's mechanic let go of the cable without warning, causing the cable to fall across employee's finger, cutting the finger.
EE was climbing up the ladder on Cat 789 Rock Truck, when he bumped his right knee on the steps. By Monday, 7/26/10, knee was swollen and warm. Went to doctor and was admitted to the hospital and had surgery on 7/28/10 to drain infection from knee.
EE was diagnosed with Carpal Tunnel Syndrome in both wrists. Surgery on right hand 6/2/10.
EE was hauling a load of coal up a grade after a rain shower and truck spun out. EE attempted to stop and put truck in reverse and missed getting it into reverse and truck slid backwards, down the hill and hit the berm and turned over on left side.
EE was helping move an electrical cable when the cable suddenly moved after being caught on something and pinned EE's hand between the cable and the end loader bucket. This caused a laceration which required stitches.

Employee was installing a muffler system on CAT 245 Excavator when he strained a muscle in his left shoulder. MRI is scheduled.
EE went to doctor for pain in left knee. X-rays were negative, but when he told doctor that he worked at a mine, the doctor says that his spine is twisted due to work at the mine and that was the cause of the pain. Pain has now moved to the back, according to the EE. Has follow up appointment on 10/6/09 for MRI.
Employee developed carpal tunnel in both wrists due to operating bulldozers over several years. Had surgery on 8/10/09.
Employee was welding on roller frame on Cat D9N dozer when some hot slag metal burnt thru his pant leg and went down into boot and caused burn on top of foot.

After cutting plate he stepped back to be clear and his foot slipped and he fell approx. 4 ft and his right inner thigh hit a slate bar that had been stuck in the ground. the end of the bar contacted right inner thigh causing avulsion.

Mechanic was removing D10 engine. The nose guard had been rolled over and 2 hydraulic lines were still attached and under tension. When 1 line was removed the remaining line could not hold the load and pulled loose striking mechanic on the hand.
Employee was operating D-11 dozer in an area with some large boulders. He got on top of one of the boulders and it shifted and moved. The bulldozer slid about 6 ft to one side and when it stopped, it jerked employee to the side straining a muscle in his neck.
Employee was operating a Caterpillar 777 Rock Truck and he claimed that the endloader operator dropped a rock in the bed of the truck and shook him around and hurt his neck. Doctor has him in physical therapy at present time. Employee stated that he was wearing seat belt.
Mine owner was operating a Caterpiller 245 Excavator and was cleaning the highwall when a rock rolled down and struck the cab. Owner had laceration to left hand and two broken fingers. Returned to work the same day.
EE was climbing out of Ingensol Rand Drill, when the step broke off where it has been welded. EE fell about 4' down and landed on his feet. His back started hurting later on and he went to the doctor on 2/5/07. He was put off work for 1 week for rest.
Employee notified company on 12/14/06 that he injured his back on 12/5/06 climbing on or off a D9L bulldozer. He went to doctor and had an MRI done. Test show ruptured disk in lower back. Doctor is saying surgery is necessary.
Employee was helping cut and weld on loader frame on 966 F endloader when a small grease fire started in a hole on the frame. Another ee attempted to use an air hose to blow put the fire and blew hot grease on ee causing minor burns to face and neck
Employee was climbing up in the bed of his welding truck when he hit his knee on bumper. Went to ER on 9/5/06. ER doctor said employee had some strained and torn ligaments in left foot. Reported to company on 9/5/06 when employee went to doctor.
Employee was doing a routine check on caterpillar 785 rock truck and as he exiting from underneath the truck he hit his left shoulder on a pull hook under the bumper and then hit the side of his head on the bympen. He wearing hand hat.
Employee was repairing a Mack 600 fuel truck. When he stepped off the truck he sprained his ankle when he stepped on a rock.
Employee was side cutting a road down the side of mountain when a large rock rolled out of the cut and hit the ripper of the back of the bulldozer causing it to jerk sideways. Employee complained of pain in his back. X-rays & MRIs were negative. Has appointment with another doctor on 8/24/06.

Employee was going down access road to leave the mine when a coal truck, a Mack 800, crossed over to the wrong side of the road and struck employee's vehicle.

Employee was working on the emergency brake on a Caterpillar TC-30 Forklift and strained a muscle in his neck. He reported it on 2/15/2006 and went to the doctor on 2/15/2006 and a follow up visit on 2/17/2006 when the doctor put him off duty till further notice.
