Mostrando 50 de 55One employee was holding a 1/2" inch rod that was pointed on one end. The victim was holding a rubber water hose that was plugged up with sand and running the hose on and off the rod, trying to unplug it. The hose slipped off the rod as the victim pushed causing the rod to puncture hand.
EE was attempting to nail two 2x4 boards together in a right angle. EE had a smaller 2x4 standing on end with the longer 2x4 on top. Was holding them together with left hand. Brought the automatic nailer up and then back down on the top of the board. EE missed the center. The nail grazed the edge of the board, deflected and penetrated EE's hand. A doctor removed it.
Manually pushing a piece of rock on a manual roller conveyor. Left middle finger was below the roller and stone rolled up with his finger between stone and the roller on the roller conveyor, mashing the tip and nail of his left middle finger.
MSHA inspector had been in plant and had EE wearing noise and dust sample collecting equipment for the day. The hoses from these devices got tangled up and he was trying to straighten them out. He straightened his arm out and hand went into radial saw. Fatty tissue between thumb and 1st finger was cut, also his thumb. Wearing 2 devices caused the entanglement.
Mill was setting the blades to make slabs out of a block of stone. Employee had tightened the blade by hitting the key. He put the pin in place to prevent the key from flying out. He then started hitting the blade to move it over. As he did this the pin fell out and the key flew up and out. The key hit the employee under the left bicep.
Pulling a piece of stone across a roller conveyor and he had 2nd & 3rd fingers between bottom of stone and roller conveyor on left hand. Pulled 2nd & 3rd nail loose front fingers. no broken bones or stitches on the fingers, just dressing and bandage for 2 days.
Employee was getting off the fork truck in the garage and stepped down onto the concrete floor and tripped over a water pump and scraped left arm from five inches above elbow to three inches below elbow. Cracked bone around ankle area. Tripping over the pump or the concrete floor caused the injury.
EE was welding a 12"X1"X3/4" piece of metal onto a mill gang cart. He was holding the piece in his R hand and welding with his L hand. He stated he heard it pop and then the welding wire bounced up and penetrated his welding glove and R thumb. He applied 1st aid and continued to work. On 4/3/11 it was swollen/sore, went to ER, did 1st aid. On 4/4/11 got worse so he was admitted.
EE was leaving the work place after quitting time in his personal auto. Another private auto was moving on the same road and they hit head on, causing the auto accident.
The machine finished splitting a 12" x 12" x 4" piece of stone weighing about 50#. He was taking the stone off the rollers to palletize. In one motion he picked the stone up, turned & threw it on the pallet. As the stone landed his finger was crushed between the stone and the stone already on the pallet.
Gang Five had finished cutting the block into slabs and the cart was pulled out of the building. EE was prying the roughback out so that he could put the chain around it, when the roughback broke and a piece fell toward EE, hitting him on the right forearm.
EE was tightening a turnbuckle with a bar, as he pushed on the bar, the nut on the turnbuckle broke. EE fell forward and his hands landed on the concrete floor.
The radial arm saw has 12' rollers on each side of it. Employee put a 10' 2"x4" on the rollers on the left side of the saw. With his left hand he grabbed the 2"x4" from the end closest to the running saw and tried to pull it into the saw. His hand slipped off and hit the blade on the running saw. He received multiple lacerations to two fingers on his left hand.
Employee stepped up on lift truck to operate and felt something pull and sting in his left ankle or foot. Left ankle and foot sprain.
EE WAS REPLACING A SEAT IN A 988B CAT LOADER. HE HAD REMOVED THE BOLTS AND WAS TRYING TO REMOVE THE SEAT FROM WITHIN THE CAB. AS HE DID SO, HE FELT PAIN IN HIS BACK
Walking down stone steps and went to step down on next step and missed the step and twisted or sprained left knee.
EE WAS USING A BAR TO LOOSEN A TURNBUCKLE ON GANG #3 IN THE MILL. THE BAR SLIPPED AND HE FELL. HE CONTINUED TO WORK WITHOUT RESTRICTIONS UNTIL 7/1/05 WHEN HE MISSED WORK. ON 7/5/05 I RECEIVED A FAX PUTTING HIM ON LIGHT DUTY
EMPLOYEE WAS USING CUTTING TORCHES TO CUT A 48" X 16" X 3/4" PIECE OF METAL OUT SO THAT IT COULD BE REPLACED. HE DID THIS FOR MOST OF THE DAY. AT THE END OF HIS SHIFT HIS EYES HURT & HE COULD NOT SEE WELL. HE WAS WEARING A FACE SHIELD WITH #12 LENS ALONG WITH TINTED SAFETY GLASSES.
EE had just finished drilling a 34" vertical hole with a track drill. He was removing the second circular drill stock when he lost control of it. As it fell he grabbed it with his right arm but the momentum of the drill stock falling caused his elbow to jerk.
THE LOADER HAD JUST SET A LOAD OF SLABS IN A CART. THE LOADER THEN RELEASED THE PRESSURE OFF THE SLABS BY LOOSENING THE CABLE BALES. THE MEN THEN ADVANCED TO REMOVE THE BALES FROM AROUND THE SLABS. THERE WAS A 40 LB LOOSE PIECE OF STONE ON TOP OF THE SLABS & AS HE REMOVED THE BALES IT HIT THE LOOSE PIECE & FELL ABOUT FIVE FEET HITTING HIS FOOT RIGHT ABOVE THE STEEL TOE.
EE was stepping off a portable platform elevated 12" off the ground. He was wearing the extra baggy pants and as he stepped off his right pant leg caught on the platform support causing him to lose his balance fall. as his pant leg ripped up the side he bounced off the step with his right hip and then landed on the floor.
STACKING STONE ON A PALLET AND LACERATED THE MIDDLE FINGER ON LEFT HAND BETWEEN TWO PIECES OF STONE.
Metal plug flew up from the ground after being hit by a sledge hammer and struck ee in the forehead causing a laceration on forehead.
HITTING A BLOCK OF STONE WITH BOTH HANDS ON A 5 LB SLEDGE HAMMER AND THE RIGHT 5TH FINGER (PINKY) SOMEHOW CAUGHT BETWEEN THE STONE AND HAMMER BREAKING THE LITTLE FINGER ON RIGHT HAND.
EE WAS WALKING ON ABOUT 2" OF MUD ON A SANDSTONE BASE. HE WAS CARRYING A ROUND STEEL PIN 3" X 24" IN HIS LEFT HAND. HE STEPPED ON A FLAT PIECE OF SANDSTONE THAT HAD A LAYER OF WET CLAY ON IT. AS HE DID THIS HE SLIPPED AND FELL.
EE JUST FINISHED DRILLING HOLES WITH A TRACK DRILL AND WAS CREASING THE HOLES TO SCORE BOTH SIDES. HE RAISED THE DRILL OUT AND GRABBED IT WITH HIS LEFT HAND. HIS GLOVES WERE LONGER THAN HIS FI NGERS SO WHEN HE GRABBED THE CREASER THE GLOVE WAS BETWEEN THE CREASER AND THE PALM OF HIS HAND. WHEN THE CREASER ROTATED, IT TIGHTENED THE GLOVE WITH HIS HAND IN IT.
TIGHTENING CHAIN AROUND BLOCK OF STONE AND LITTLE FINGER ON LEFT HAND GOT BETWEEN THE BLOCK OF STONE AND THE CHAIN AS THE CHAIN WAS BEING TIGHTENED BY THE CRANE, BREAKING THE LITTLE FINGER AND LACERATING THE FINGER.
CABLE RUNNING THRU A SHEAVE AND SOME WAY FINGERS GOT BETWEEN THE SHEAVE AND SIDE BRACES. LEFT AND RIGHT FINGERS SMASHED AND SOME AMPUTATION ON LEFT AND RIGHT FINGERS.
A PIECE OF SLAB END WEIGHING ABOUT 75# WAS SITTING ON A SET OF ROLERS ABOUT 42" HIGH. EE PICKED UP THE PIECE AND PUT IT ON A PALLET. AS HE BENT OVER, HE FELT PAIN IN HIS BACK. HE CONTINUED TO WORK THE REST OF THE DAY. THE NEXT DAY EE REPORTED THE ACCIDENT AND REPORTED OFF WORK.
EE WAS REMOVING STONE FROM A SPLITTING MACHINE. AS HE PICKED UP A PIECE WEIGHING ABOUT 75#, THE PIECE BROKE AND ONE PIECE FELL AND HIT HIS FOOT ABOVE THE STEEL TOE.
EE WAS HOLDING A COPING TOOL AGAINST A PIECE OF STONE WHILE CO-WORKER HIT THE COPING TOOL WITH A SLEDGE HAMMER TO SCORE THE STONE. CO-WORKER HIT THE EDGE OF THE COPING TOOL AND A SMALL PIECE O F METAL CHIPPED OFF THE SLEDGE HAMMER. THE PIECE HIT EE IN THE LEFT FOREARM AND BECAME EMBEDDED.
REINJURY OR PAIN IN LOWER BACK REGION FROM PREVIOUS INJURY 12/27/01.

EE WAS HITTING THE BOTTOM OF A KEY THAT WAS HOLDING A BLADE TIGHT IN A GANG SAW SO THAT HE COULDLOOSEN THE KEY. AS HE WAS DOING THIS HE REACHED WITH HIS LT. HAND TO GRAB THE KEY, WHEN HIS HAND WAS ABOUT 5" FROM THE KEY, THE KEY UNEXPECTEDLY POPPED LOOSE AND FLEW UP AND HIS HIS HAND.
LIFTING A PIECE OF STONE TO STACK ON A PALLETT AND SPRAINED LOWER BACK.
WALKING IN STONE YARD AROUND A PICK-UP TRUCK AND FELL ONTO GRAVELS OR A PIECE OF STONE AND LACERATED RIGHT FOREARM WHEN FALLING. NOTHING CAUSED HIM TO FALL OR TRIP. HE CAN'T EXPLAIN WHY HE JUS T FELL.
STONE BROKE WHILE STACKING AND 3RD FINGER ON RIGHT HAND WAS HIT BU A BROKEN PIECE OF STONE.
2 EE'S WERE SPLITTING 16" HIGH ASHLAR & STACKINGTHEM ON A PALLET. WHEN ONE WENT TO THE RESTROOM INJ EE HELPED ON THE MACHINE. 10/17/01 EE REP'D THAT HIS SHOULDER HURT. HE CON'T WITH HIS REGULA R JOB AS FOREMAN UNTIL 4/19/02 WHEN HE STARTED A MEDICAL LEAVE OF ABSENCE
THE BLADES ON GANG FIVE HAD BECOME WORN OUT FOR SAWING STONE SLABS. THE EES HAD REPLACED THE BLADES AND WERE HAMMERING THE KEYS IN TO TIGHTEN THE BLADES. WHILE THEY DID THIS EE'S ELBOW STARTED TO HURT. HE CONTINUED TO WORK UNTIL 1900.
BILATERAL CARPAL TUNNEL RELEASES AND POSITIVE TINEL S SURGERY WAS PERFORMED TODAY ONT HE LEFT WRIST AND A LATER DATE FOR THE RIGHT WRIST. HE USED A HAMMER ON HIS JOB EVERYDAY.
HIT RIGHT ELBOW ON DOOR LATCH AND CAUSED LATERAL EPICONDYLITIS.
CUT LEFT SIDE OF HIS HEAD ON A PIECE OF ANGLE IRON WHILE UNLOADING RACKS.
EE WAS WORKING IN THE MINE PIT NEXT TO A 40' STRAIGHT VERTICAL HIGHWALL WHEN A SMALL ROCK LESS THAN AN INCH THICK FELL AND HIT HIM ON THE RIGHT SHOULDER. HE DID NOT SEEK MEDICAL ATTENTION OR L OSE WORK. HE WENT TO DOCTOR THEN WENT ON LIGHT DUTY.
WALKING IN THE AREA WHERE FINISHED BUILDING STONE IS STORED FOR MOVEMENT TO ANOTHER LOCATION AND FELL OFF THE END OF FLOOR TO THE GROUND BELOW. FELL 3 FEET, SPRAINING BACK AND LEFT KNEE.
STANDING ON WIRE SAW POST AND HIS RIGHT KNEE BRUSHED AGAINST THE SHEAVE LACERATING THE RIGHT KNEE.
DRIVING STEEL KEYS WITH HAMMER WHEN A FOREIGN BODY WENT INTO RIGHT EYE.
BENT OVER TO REMOVE A ROCK FROM ATOP A WATER VALVE COVER SPRAINING MY BACK.
EMPLOYEE WAS CLIMBING DOWN A SIX FOOT LADDER. HE TURNED AS HE WAS STEPPING OFF THE LAST STEP. HIS LEFT FOOT SLIPPED OFF OF THE LAST RUNG. THIS LEG WAS IN A TWISTED MOTION AS HIS FOOT HIT THE GROUND.
