Employee was cranking a cable winch to pull the dust stacker conveyor sideways. The cable guide bar on the winch failed. The broken bar flew up striking the employee on the right side of EE's face causing a laceration, fracture and right eye injury.

Mostrando los 16Employee was cleaning up spillage, pushing a wheelbarrow. The wheelbarrow started to tip over. Employee tried to prevent the tip over by grabbing one handle with both hands. The handle went between EE's fingers causing a fracture of the middle finger on EE's right hand.
Employee was operating a Porta Power hydraulic jack. The employee's right hand little finger got caught in the pinch point at the base of the jack handle.
Employee slipped on lower step while dismounting end dump, fell and injured left hip and knee.

EE was using pressure water hose to wash out the sand screws. When the water pump was turned on the hose jerked causing the EE to fall. EE struck his left arm on the plant causing a laceration just above the elbow.
Employee was changing screens in the stone sizing plant. Employee was pulling on the screen to remove it from the screen deck and he pulled a tendon in his right arm.
Employee was lifting an electric motor from it's mount using a block and tackle. The motor was caught on the base. The employee twisted the motor to free it and the motor sprang up striking the employee's left arm.
EMPLOYEE HAS SILICOSIS AS SHOWN ON CHEST X-RAY. EE IS ASYMPTOMATIC AND HAS NORMAL PHYSICAL EXAM.
EMPLOYEE FELL TO HIS KNEES WHEN A CATWALK PLANK BROKE.
EE was walking and carrying a 20 lb propane bottle when he slipped on snow and ice. EE fell and broke right ankle.
EMPLOYEE WAS WALKING FROM LOCKER ROOM TO WELDING SHOP. EMPLOYEE SLIPPED AND FELL. BROKEN LEFT SHOULDER.
EE WAS MOVING SPARE RETURN ROLLER FOR CONVEYOR. A SECOND ROLLER FELL AND STRUCK EMPLOYEE ON RIGHT HAND.
EMPLOYEE WAS USING A HAND GRINDER TO CLEAN UP STEEL FOR WELDING. A PIECE OF METAL WENT INTO RIGHT EYE.
