Employee returned to work his next scheduled working shift however on a follow up Dr. appointment on 04/24/09 the Dr. requested he be off until his next Dr. appointment.
Employee was roof bolting on the right side of the Fletcher Roof Bolter in the No. 2 Entry. Employee started to drill the first hole in the second to last row in that place. ATRS and safety post was set. Rock came loose from the right rib, striking the employee on the lower back. (Rock size was approx. 5'x3'x5" thick)
Employee was roof bolting when he observed a loose piece of rock. As he attempted to take the rock down, a piece broke off, landing on the employee's right ankle.

The working supervisor was observing the Continuous Miner Operator in the No. 2 entry, when a piece of rock dislodged between the roof bolt and rib line, striking employee on the head forcing his hard hat off thus causing a head laceration and jamming neck. Employee required staples to close laceration.
While employee was walking by the #1 bridge a piece of rock fell off striking employee's left foot.
WHILE EMPLOYEE WAS LOADING ROCK DUSTER WHEN A PIECE OF ROCK DISLODGED FROM ROOF LACERATING SMALL FINGER ON RIGHT HAND. EE RECEIVED SUTURES AND WAS PLACED ON RESTRICTIVE DUTY THUS MAKING THIS REPORT A MEDICAL.
EE WAS OPERATING THE #2 MBC. AS HE WAS TRAMMING INBY IN THE #3 ENTRY, A PIECE OF ROCK(APPROX. 18" X 18" X 8" THICK) DISLODGED FROM BETWEEN ROOF BOLTS STRIKING THE OUTSIDE PART OF EMPLOYEE'S LE FT FOOT.
