Employee was taping transducer wires to PVC under the table when he was struck in the mouth by a PVC lifter that was knocked off the table. Employee suffered three fractured teeth.

EE was changing cutting edges when one fell on his foot. He was seen at the ER and diagnosed with a fractured big toe. Released to restricted duty that the site was unable to accommodate. Lost time.
Helper was closing the rotation guard on the drill when his left finger tip was pinched between the frame
Tripping out rods when threads caught strap which wrapped around rod catching employee's hand between the rod and strap.
Employee was opening a can of welding rod when the sharp edge of the pop top of the can cut his right palm at the base of the thumb. Stitches were required and the employee was placed on sedentary duty. Referred to an orthopedist.
Employee was installing a fan extension and the guard slipped and cut the back of his left hand. Seen at the ER, stitches and put on light duty for 10 days. No light duty available, employee put off work for 10 calendar days, resulting in 5 lost work days.
IW stated that her arm was sore on 07/21/11. On 07/27/11 she was using tape making signs and her arm was sore again. Seen at ER on 07/27/11, diagnosed with strain and released to sedentary duty for one day. Investigated by workers' comp and site, determined work related on 08/15/11.
Employee was checking to see if core was inside of core tube with his hands. While his hands were at the end of the core tube, the tube was being lowered at the same time and his fingers became pinched between the tube and the floor.
Employee helping to replace brake drum on shovel, rolled new drum into house, lifted it, and it tipped and smashed the tip of the right ring finger between drum and equipment. Received 7 stitches and released to full duty.
Employee was removing a pin on the rear axle of a piece of equipment when the pin came out and dropped on his left thumb. He was taken to the hospital where the thumb was surgically repaired and he was released to sedentary duty. He was subsequently placed off work.

Emp bent over, opened a 45 pound metal cover for gas meter and felt pain in lower back upon pulling and turning to move cover. Investigation completed and determined work related on 12/21/2010.
Emp climbing in and out of hopper chipping frozen mud. His right hip was hurting, but no accident. He saw doctor on 12/15 and on 12/20 investigation completed and claim accepted by workers' comp. Emp having electrical stim treatments at chiropractor, making this medical treatment.
Employee's hand slipped while putting inner tube back into the hole. This caused the employee to pinch his fingers between the rod and the latch head. The weight of the tube caused a laceration of the left ring finger and the left pinky finger of the employee.
Employee was washing under a conveyor belt with process water that has a high P.H. level. The injury arose from contact with the process water and a reaction between his skin and rubber gloves.
While unloading walkway extension the support leg folded up on the employee's left ring finger resulting in laceration and loss of fingernail.
Employee was lifting a box out of the back of his work truck, and felt a 'twinge' in the groin. He continued to work and at about 8 am he lifted another box and felt pain. He was seen at the ER, diagnosed with a hernia and referred to a surgeon. He will have hernia repair surgery on 11/02/2010.
Lifting casing with jib boom when casing became hooked on threads of other casing allowing C hook to come out and casing to fall back to truck hitting employee's foot
Employee was climbing the ladder of a motor grader when he felt a pop in his right shoulder followed by a burning sensation. He was seen at the ER and referred to an orthopedist. He was diagnosed with a right rotator cuff strain, released to restricted duty and referred to physical therapy.
While checking the teeth on a bucket, the employee put his left hand on a tooth to lean down and check underneath. The metal was extremely hot and burned the palm of his left hand. Employee was seen at NNRH where his hand was bandaged and he was released to light duty for two shifts.
While mounting the lube truck, employee felt a sharp pain and grinding in his right elbow. Taken to NNRH where he was given medication and a full duty release. Could not return to work the next day due to pain. He went to his personal physician on 07/09/10 was put on Restricted Duty. No RD in his area so it is a Lost Time.
Employee was installing a motor on the batch plant mixer when his left thumb got caught behind the motor. Thumb was de-gloved from knuckle to the tip. He was taken to NNRH where the tip of the thumb was reattached.
Employee felt what she thought was a rock in her boot. When she took it off at the end of the shift there was a chemical burn from shotcrete that had gotten into her boot. She was later seen at Reno ER when the burn got infected on 4/08/10. She was given a full duty release.
Employee was trying to catch a bolt but when they started to loosen the bolt the piece of metal that was welded on the bolt caught his finger and pinched it.
Employee got his finger caught in the dipper trip cable drum. It cut his left pinky and so the employee was taken to the hospital where he received stitches.
Helper lifting empty tube into rod string felt a "pop" in groin area.
EE reported back pain on 2/4/10 due to repeatedly bending over while coiling up heat trace cable. He did not want to see dr at that time. On 2/9/10 he reported he had reinjured his back while lifting his personal tools from back of site truck. He still did not want to see a dr. On 2/10/10 he notified his supervisor he was not coming in due to back pain. He was then taken to a dr.
Hose fitting became liberated striking employee in mouth fracturing tooth
The injured employee was lifting some warehouse items onto a pallet and felt some pain in his knee area. He did not think much about it at the time and did not report it to anyone. There was no accident notification at all, we did not know about this occurrence until 4 days later.
Employee was loading a flat tire into the back of a service truck when the tire shifted and the employee heard his left wrist pop. Was taken to the ER and given a full duty release. At a follow up on the 16th it was discovered that he had a fracture and was released to light duty. No light duty so he became a Lost Time on the 16th.
Employee was fabricating cable reel frame when a piece of steel channel tipped over and fell on his left foot. Employee was taken to the ER and was given a full duty release. On a follow up with a specialist on 10/05/09 he was given a restricted duty release. There is no restricted duty in his area so it has been classified as a lost time.
On 07/10/09 emp. was rolling up cable when he felt a hard pop in his right shoulder. He was taken to the ER and was looked over and given a full duty release. He continued to be in pain so he was sent back in for a check up and was then scheduled for a MRI. On 09/09/09 it was discovered during the MRI that he had a torn rotator cuff. He became a Lost Time on this date.
Surgery for Left Inguinal Hernia, employee taken off work.
Employee was cleaning up drainage system. As employee was picking up grading employee placed hand between grading and another piece of grading. Employee set the grading down on his finger.
Employee was stacking rebar on the bank. One of a handfull of bars flipped up and cut his ear.
Miner was washing out transmixer drum and as material was draining out the back it caused the water to splash up underneath the miner's safety glasses.
Turning over blade cylinders for D10 unit 421 to install piping and brackets to unit. Felt real sore after work and pain worsened over night. During the next shift he let the job foreman know about it. The following day he told his company supervisor and was taken to the Doctors. He received first aid treatment and returned to work on his regular shift with no restrictions.
Employee was pulling chain over rusty pipe. Wind blew rusty debris under bottom of EE's safety glasses and into his eye.
Crew was loading 2 pieces of casing onto a rack on a pickup using a forklift. The forklift operator tipped the forks to level them and the pipes slid off the forks and pinched the helper's left middle and ring finger. Resulting injury was a contusion to the fingers requiring 2 days of modified duty.
Driller was breaking off the sub on the caseing, there was a rod in the chuck about 11/2 ft away from the sub. Helper(injuried) went to retreive the sub, as driller released the footclam the rod came down on helpers hand. Helper was struck in the back of the left hand, causing laceration and fracture to little finger.
