6/9/2019 temporary laborer was reaching over to lift an assay lab sample bag when they felt a pain in the back/shoulders. On 6/13/2019 they sought further medical treatment and was then placed on light duty by the physician
Employee was lifting materials out of service truck when employee felt a strain in lower back
While employee was washing glass bottles, a bottle slipped out of their hand breaking and causing a laceration to their left wrist.
EE was in the process of transporting piping for leach operations when they caught their left 2nd finger between pipe and back of loader. Initial clinic visit revealed no fracture and EE returned to full duties. On a follow-up visit on 5/1/19 the PA prescribed 2 physical therapy sessions when EE reported finger felt stiff.
Ee was sliding filter plates across a beam and pulled a muscle. Ultimately had surgery on 11/07/19 making this lost time.
Employee struck the back of left hand on a truck door causing an acute displaced facture in left hand.
IP reported on 3/20 that IP had been experiencing discomfort in lower back for the last several weeks while performing routine tasks in the lab. IP was seen by provider, prescribed physical therapy and given a list of restrictions. Restrictions will not be accommodated.
Employee was clearing a lime screw feeder to prepare it for a maintenance inspection. The employee identified the feeder was plugged and was attempting to clear the blockage. Dry lime burped out of the feeder (swirl box) while the employee was standing in close proximity. The employee sustained chemical burns to the face, neck, chest, abdomen and eyes.
IP was in the process of closing the service cage at the 4450 station. In that process, IP's left ring finger came into contact between the handle and latch. The injury resulted in a laceration which required sutures to close. IP is currently on restrictions.
Performing filter press trouble shooting and was bending down to walk under structure when at same time pry bar caught grating and then contacted nose and upper lip resulting in lacerations requiring sutures to close.
IP noticed air and water headers come down off the rib in a muck bay. IP decided to move the water line first by bending it back on itself and stuck the header into the ground. As the IP was in the process of retrieving the air line, the water line came dislodged and the valve turned on. The water line struck the IP's right forearm resulting in a fracture.
Employee was helping move a piece of 8" pipe when EE's pinky finger was pinched between the end of the pipe and a valve body. Employee received a laceration that took 7 sutures to close.
IP was mounting dozer and was pulling self up by the hand hold when EE felt a "pop" in EE's right elbow. MRI revealed partial thickness tear of radial bicep tendon. Cannot accommodate restrictions with regular tasks resulting in a lost day injury.
While the driller was cycling a core tube down, standing on the helper deck a 5' rod in the rod rack started to slide to the other end of the rod rack. The driller turned around and tried to stop the sliding rod when 2 other rods slid against the first one pinning the driller hand between the rod and rod rock causing lacerations to the left fingers.
Employee opens valve and gets debris in both eyes
EE was cleaning hydraulic fittings underneath a haul truck when ee lost balance and placed ee right hand out to catch self. In the process ee hand got caught between a hydraulic line and the truck frame. This resulted in a wrist and shoulder strain. When evaluated the EE was not released to return to work.
EE was removing engine doors from a CAT 16M Grader. Handed door down to another employee and felt pain in back.
While inserting a shim into a gap between a drill and a wrench, the employee pinched EE's thumb.
While employee was replacing a lock ring, it slipped and pinched left pinky finger.
EE was working on placing concrete and never noticed that EE's pants at the Knee level became saturated with concrete, resulting in concrete burns to EE's Left and Right Knees.
An employee was checking for air flow in the HVAC system of a CAT grader. EE reached into the fan housing while the fan was running and contacted it with EE's right thumb.
Driver just finished unloading lime and went to disconnect the product hose. The hose was not under pressure, and the driver was pulling on the hose to get it unhooked. Once the hose disconnected the coupler hit the driver in the upper jaw below the nose displacing driver's 2 front teeth.
In the process of lifting a sling the IE felt a "pop" in the stomach. During medical evaluation the IE was diagnosed with a hernia around the belly button. It has yet to be determined if surgery will be required. The IE was placed on light duty restrictions.
Two employees were lifting an estimated 100lb mini-power zone from the back of a flatbed truck. One of the employees hand slipped from its holding position, causing the edge of the mini-power zone to strike EE's right thigh. This resulted in a laceration to the employees right thigh which required 16 sutures.
Emp was assisting to put CAT988 loader cutting edges back on the forks that slid off and onto the ground. Emp was using right hand to push one of the cutting edges to straighten it out so the forklift could pick it up and felt 3 pops in right elbow. This event became restricted duty on 2/12/2018.
Technician was removing impellas from assembly, supported with lifting straps and forklift, while positioning for removal, weight shifted causing extremely sharp impellar fin to contact hand with leather work glove on cutting left ring finger
There was a sharp wire on the back side of a hose. Operator went to adjust the hose and did not identify the hazard resulting in the wire piercing the glove and cutting and tearing the skin.
While operating bolter UDR 053 at the face in the 4450-604 the operator went to egress from the bolter to change a bit. When descending the operator tripped and grabbed the handle of the door to catch the fall. The result was a dislocated right shoulder.
Employee stepped to gain access to a drill ladder and felt a pop in the right knee. Employee sought medical treatment and was given restrictions.
While preparing to change out a grease hose reel on a fuel/lube truck, the reel fell striking a lubricating technician's right hand pinky finger.
While an employee was lifting a coolant surge tank, weighing 120 lbs, a portion of the rigging broke, causing the load to fall three inches and pinching left thumb. This resulted in an injury that required 6 stiches.
While handling a rubber ceramic liner, employee pinched employee's finger between the liner and the chute wall. As a result the employee received a laceration to employee's left ring finger requiring sutures to close.
Employee was standing on scaffolding inside a tank. EE was removing a sand-blasting hose to hand it to another EE who was standing on a platform above. The hose caught on the scaffolding plank EE was standing on causing it to slide off one side of the piping it was positioned on. EE's left arm got caught on a scaffolding cross brace.
Employee was unloading 10lb sample bags from a flat bed trailer one a time. Employee reached to grab one and started to pull and lift not realizing their were three bags tied together. employee felt a strain and pop to their wrist.
Instrumentation Tech was removing leak detector pump #23 at Underdrainage #3 when the employee experienced issues removing leak detector from pipe - getting stuck and slipping. The employee then put on latex/vinyl gloves when attempting to remove it again the pump slipped and lacerated the right index finger at the joint closest to the palm.
Employee was getting on a 988 surface loader using three points of contact. While stepping up onto the bottom step, the employee felt a pop in the left shoulder. The employee sought medical treatment and was unable to return to work.
IE was moving an empty garbage bin into place and felt a pain in abdomen. Was diagnosed with a unilateral inguinal hernia and released to restricted duties.
IE was in the process of putting a sling away when it twisted. Felt pain in shoulder which had been surgically repaired in the past. As a precaution IE requested to be seen at ER. No diagnosis given to date, however, employee placed on restrictions.
EE was helping another laborer move a negative air machine from one vessel to another. The EE aggravated a pinched nerve from a previous injury. The EE requested to be laid-off. On 3/21/2017, we were made aware that EE went to a chiropractor for treatment.
Driller went to inspect drill rod removed from drill string that was cut and reaching finger inside pin end, cut left index finger.
Injured Employee (IE) was cleaning up spillage around Rock Breaker 1 at the end of the shift. Upon closing the barrier gate to the grizzly the IE caught their right ring finger between the gate and a deflector plate. X-ray revealed a fracture to the tip of the finger and sutures required to close laceration.
The employee felt a pop in right shoulder while lifting personal backpack up on to the boarding platform of a haul truck. This injury resulted in a torn tendon in employee's right shoulder. This injury became reportable on 2/28/17
Driver pinched finger under chain while securing a load on trailer. Helper was on one side of truck securing binder while driver was on the opposite side straightening a chain.
Tech got finger caught between a belly pan and bolt. Pinching the left index finger. The tech in an attempt to remove the nut from the bolt pressed on the washer with their fingers to bind the bolt, while removing the nut form the bolt. Causing damage to the index finger just above the first knuckle.
While at work pouring concrete for shaft sink, EE was holding the end of a 6" x 12 foot hose with both hands, as concrete came to the end of the hose, the hose jerked and lifted EE causing feet to come off the ground and when ee came down it caused EE to strike right forearm on a metal hopper edge.
Employee was struck in the forehead with a 1/4 inch chain. Employee was wearing a hard hat.
Technician was repairing air pipe in mill. Due to weakness in shoulder, when residual air from line came out, arm dislocated from the socket. This is the 2nd time this has happened in 2 months. Technician was taken to hospital to have arm reduced into place.

Our driver was unloading Hydrochloric Acid when EE went up trailer ladder (approx. 4-5 feet) to disconnect air supply hose. EE disconnected the hose, vapor and some product came out and EE dropped the hose to the ground. Product continued to leak out of the hose and EE hurried down the ladder to stop the leak. EE missed the last rung and fell to the ground, fracturing right heel.
While tripping pipe onto the Rolagon, employee was on the Rolagon sliding the 10 foot RC rods into place. When the employee reached down and pulled on the pipe ee felt a pull / strain in right shoulder.
