Ee was walking to clear plugged chute. EE reports rolling EE's ankle on an unseen object. EE reports hearing a loud pop in ankle. Seen at ER, xrays negative for fracture, given ace wrap and crutches, released to restrictions. Site cannot offer temporary modified duty. EE released w/restrictions, site is offering modified duty. EE has no missed time.

While having a Safety meeting, a newly employed miner fainted and fell to the floor of the office. The miner was taken to Battle Mountain hospital where EE was released. The physician could not identify why the employee fainted. The employee was instructed to see EE's primary care Physician.

EE was assisting another EE lifting a cathode. The prybar dropped and they went to catch it and felt pain in shoulder. Seen at clinic, released with restrictions, and referred to ortho. Diagnosed with possible rotator cuff tear and referred for MRI, continued on restrictions.

Ee was cleaning a jammed piece of copper from the stripping machine. Wearing gloves and using finger to try to remove the jammed piece and felt a cut. Took off the glove in the restroom and left index finger was cut. Seen at clinic, diagnosed with a laceration that was closed with 4 sutures. Released to work.

EE was pulling a hydropak out of the walkway in the tool room. They felt pain in their right groin area while pulling. CT scan negative for hernia, ee diagnosed with muscle strain and released to modified duty site can provide.

Ee was lifting a blue drum that ee reports was half full. Ee felt pain in right bicep while lifting. MRI on 11/15 was positive for a tear making this reportable. Claim accepted by workers' comp on 11/18. Referred to orthopedic surgery.

EE hooked a 4 way chain to component with crane. When they put tension on the chain, it spun and hit them in the mouth. Broken front tooth. Seen at clinic, referred to a dentist, and released to full duty.

Ee was using a pry bar to move a pin. The bar slipped and smashed/pinched employee's finger between the bar and a ladder. Seen by doctor, diagnosed with a fracture to the right long finger, and released with restrictions site can accommodate.

Ee was working on machinery in the SXEW plant. Ee reports a foreign object getting behind ee's safety glasses into right eye. Seen at clinic, diagnosed with right eye burn. Given erythromycin drops and released with restrictions for one shift to work indoors. Seen on 7/5, released to full duty.

Ee was putting filters away when a filter press fell, striking the ee on the back. Ee reported head, neck, shoulder and right hand pain. Seen at ER and clinic, released with restrictions site can accommodate.

Ee was changing a drill bit. Ee lifted the drill bit out of the bit basket and felt pain in lower back. Seen at ER, diagnosed with a strain and spasm and lumbago. Released to restricted duty that site can accommodate.

Hydroblast cut and injection of water in the left hand between thumb and pointer finger

Strap broke and shackle recoiled back.

Ee was lowering a man basket on Sh017 for fueling. Ee had to push hard on the release pin to get the basket to move. The basket dropped down, and the ee reached out to grab the handrail. Ee contacted the man basket safety gate bracket which was sharp, and lacerated right hand.

EE was doing a post-shift inspection and slipped on ice in the chock ditch, falling and landing on their right side. Seen at ER, diagnosed with right rib fractures and right elbow contusion. Put off work pending follow-up on 1/22/24.

EE reports was lying on the ground, air arcing on a piece of equipment. When EE went to change positions, EE felt pain. Seen at ER and clinic. Suspected non-occupational condition ruled out by ultrasound. EE on restricted duty.

EE was walking in the process area when EE's boot caught on the grating and EE fell, striking their right knee and left hand. Seen at Golden Valley. Lacerations to right knee and left hand were sutured and EE was released to restrictions effective 11/21 that site can accommodate.

EE was moving a crate. When they went to set it on the flatbed truck, the crate slipped and it fell on their finger. Seen at clinic, diagnosed with a tuft fracture and referred to orthopedic. Released with restrictions site can accommodate.

EE changing wear iron on loader. In the process of hammering the wear iron off the loader, a piece of shrapnel dislodged and struck the EE in the left leg. Seen at ER, referred to surgeon. Shrapnel surgically removed, EE put off work pending post-op appointment.

Work Shift started on 6/01/2023. Incident occurred on 06/02/2023. Employee was using an angle grinder to buff rubber on floor of a flotation cell. The angle grinder kicked back resulting in a laceration to right knee.

Ee found on floor of mill locker room, blood on floor. Ee reports passing out. Ee was not coherent, unaware of where ee was, or who ee's coworkers were who found ee. Transported to ER, diagnosed with a non-occupational condition. This incident was immediately reported as a courtesy. No evidence of work-related event/condition/injury.

Ee was walking down a small set of stairs, felt sharp pain in upper back. Prior to walking down stairs, EE reports having collected sample and assisting the 5 inch bull feeder. Leaving reclaim and walking down stairs when felt pain. Investigated and accepted by workers' comp on 3/27, reportable for restricted duty.

Ee was working on retention screen rebuild. EE removed a beam and felt a pinch on EE's right hand. EE went to the lunch room to check EE's finger and noticed a large cut. Seen at clinic, laceration was closed with 10 stitches. Released to restricted duty site can accommodate.

Ee was replacing the "E" cyclone when the wire/cable sling broke, striking the employee's right hand. Seen at a medical center, xrays negative for fracture, diagnosed with a contusion to the right hand, and released with restrictions the site can accommodate.

Mine conditions did not contribute to this situation, and no impairment was sustained from this incident.

The employee was using a core press, the ram was bent under pressure. When the employee retracted the ram, the ram launched the tube upward striking the employee in the hand resulting in a laceration & forehead (eyebrow).

Ee was pulling a welding machine, lost grip on the handle, and felt shoulder pop. Pain did not resolve, MRI done 9/29/22, compared to MRI done on 10/12/18, and new tear determined to be work related on 11/3/22. This claim is now reportable for restricted duty effective 11/4/22. Surgery is required.

Individual experienced pain in the abdomen at end of shift on 6/7/2002. Next day pain was still present. On 6/8/2022 reported to their supervisor about the pain and thinks an injury occurred towards the end of shift when handling sonic core samples (30 inch, approximately 54-60 lbs). This resulted in an a diagnoses of inguinal hernia. Pending follow-up with specialist.

EE was trying to step down from the tailgate to the bumper of a pickup when they slipped and fell to the ground. Twisted a knee. Experiencing ongoing discomfort and requested to be seen. Seen on 4/4, diagnosed with an MCL sprain, referred to physical therapy, and released to full duty.

EE was lifting a 12/24 jump pack on to a charging shelf. EE felt a pop in right shoulder. EE reported immediately but did not seek treatment. On 2/25 EE sought treatment. Denied by work comp pending investigation. MRI done and torn rotator cuff identified. Determined compensable by workers' comp on 3/9/21.

Ee tripped getting off piece of equipment. Ee fell forward and caught self with left arm. Reported on date of injury and requested to see doctor when pain did not resolve. Seen at a medical center, xrays showed midclavicular fracture. Referred to orthopedic surgeon for consultation. Released to full duty.

Ee stepped off the bus at Phoenix mine site parking lot. EE's ankle turned and EE felt immediate pain. Seen at Golden Valley, xrays being reviewed. Ee released to restricted duty and referred for an orthopedic consult.

EE was helping extract a sheared off lead anode from a EW cell. EE was pulling anode from below floor level with a hook when they felt their left shoulder pop and then had pain and numbness in fingertips. Seen at medical and diagnosed with a shoulder strain. Released to restricted duty that site can accommodate.

EE lifted a bucket of bolts that weighed 138 pounds. When EE turned, EE felt a sudden pain in right side of groin. Seen at Golden Health, diagnosed with a hernia, referred for surgical consult. Site accommodating restrictions pending surgical consult.

Ee was lifting a lid to a dumpster while trying to throw a full bag of garbage into the dumpster. Ee felt pop in right shoulder. Did not feel pain until that night at midnight. Reported to supervisor, seen at Golden Health, diagnosed with a sprain, released to light duty that site can accommodate.

EE was working in CV003 chute. Lime dust fell and got on EE's neck and back and also in and around eyes. EE washed eyes out and reported to WorkCare. Self-care seemed to be working, but on 2/15 requested an appointment with an eye doctor. Was seen on 2/16 and diagnosed with allergic conjunctivitis. Eye drops prescribed and EE released to full duty effective 2/17.

Ee was removing bolts with impact and felt something go into right eye. Ee was wearing safety glasses. An EMT flushed ee's eye and the ee returned to work. Ee requested to be seen by eye doctor due to ongoing irritation. Seen on 10/12, a rust ring was cleaned from the eye and ee was prescribed antibiotics. Ee is released to full duty.

Ee cleaning knelson bowl. Ee used left arm to turn the bowl and felt pain in left shoulder. Diagnosed with shoulder strain, released with restrictions. Ee had submitted resignation effective 8/25/2020 prior to the injury date. Last day of work 8/24/2020.

EE was opening window of loader and caught right pinky finger in the latch. Seen at ER, diagnosed with fracture to tip of finger and fingernail avulsion. Sutures required. EE released to modified duty.

Walking by bay 1 while someone was hammering on a bucket. Something flew off and hit employee in the face. Thought it had hit a tooth, then noticed the bleeding. After EMT cleaned up, noticed it was through and through. Sent into town for X-rays. X-rays showed metal still in lip it was removed.

Ee was stepping onto a high step, foot caught, ee fell forward onto concrete slab. Injured right arm. Notified of need for surgery related to fall on July 9, resulting in this report. Ee will be scheduled for surgery.

Hydraulic line on haul truck broke while ee dumping load. The loaded bed fell quickly, jostling the ee in the cab of the haul truck. EE had immediate pain in the neck and back. Seen at Golden Valley and diagnosed with neck muscle and lumbar strain. Released to restricted duty.

EE stepped over a hose, the hose blew apart and whipped around, hitting them on the back of the lower right foot, hyperextending right knee. EE reported on the date of injury, but did not seek treatment until 01/03/20. EE was put on modified duty and an MRI was ordered.

Employee was using a 2.5 pound hammer to loosen up prill in the silo. While swinging the hammer the head caught on the silo handle causing employee's arm to stop suddenly. Employee felt a pop in left shoulder.

Ee was unplugging a sump pump in the lime slaker area. Air broke the plug free and lime dust and debris was blown under the employees safety glasses into eyes. Seen at the ER and diagnosed with chemical conjunctivitis. Ee prescribed medication for the eyes. Released to full duty.

Employee was working in the old float area of the mill scaling a tank. While using a small pry bar the employee was striking and prying the scale away from the side of a tank. The employee contacted a sharp piece of bracing that was partially hidden in the scale on the side of the tank and employee's right forearm contacted the sharp metal causing a laceration.

While removing a liner in the mill with an overhead crane the liner swung free hitting the employee on the back of the head causing a laceration to the scalp.

Employee report pain in their back when the bed of the haul truck came down after dumping a load of rock to the crusher.

While employee was washing glass bottles, a bottle slipped out of their hand breaking and causing a laceration to their left wrist.

Employee struck the back of left hand on a truck door causing an acute displaced facture in left hand.

Operator fell while dismounting equipment and fractured right ankle.

While inserting a shim into a gap between a drill and a wrench, the employee pinched EE's thumb.

While employee was replacing a lock ring, it slipped and pinched left pinky finger.

Employee felt pain in lower back after bend over to chock tires.

Employee felt a pop in left shoulder when turning a pipe wrench.

While descending stairs employee felt a pop in left knee. Employee has a strained muscle in the back of knee.

Employee was struck in the forehead with a 1/4 inch chain. Employee was wearing a hard hat.

Employee was struck in the face with a 3 pound hammer head that came off its handle while another employee was driving a shaft through the planetary gear.

Employee's right wrist was cut by a copper plate that penetrated EE's work glove.

Shovel operator felt a pain in back after stepping on a piece of gravel.

While climbing a ladder the employee felt a pull in EE's groin.

Employee unloading 1200 lb bags of Aluminum from trailer. It had tipped over in truck which made it hard to unload. Employee used a pallet jack to move bags. The employee reported hurting shoulder, on way home was sent to clinic, then Occupational Doctor following day received full release. Went back on 03/14/2016 MRI and Therapy scheduled. Became lost time on 3/15/2016

Employee stepped on a rock after descending the stairway of his haul truck.

While two mechanics were installing a 4" Victaulic clamp onto a 4" elbow, the mechanic at the bottom of the ladder was hit by the falling clamp when it slip out of the hand of the mechanic on the ladder. The clamp bounced off the ladder and hit the mechanic in the mouth. He was taking to the hospital for stitches.

While employee was removing an air hose there was a release of unexpected lime slurry which hit the employee under his safety glasses in the right eye.

While an employee was removing a small airline hose, the fitting on the pressurized airline struck his forehead causing a laceration.

While descending stairs, employee rolled his right ankle on a rock after stepping off the last step onto the ground.

Employee strained his back while shoveling.

AMS was contracted to install new water supply piping. In the process of moving a section of pipe with a forklift one of the forks became stuck underneath a small metal lip. The forklift operator attempted to tilt the forks to load the pipe unaware that a fork was caught. When the fork released it caused the pipe to elevate off the forks and strike the worker in the chin.

Sprained left knee while stepping down from the jumper frame.

Mechanics were separating a gear box from an agitator shaft flange. The Victaulic coupler holding the shaft failed causing the shaft to slip through the hugger approximately 3.5 feet. During the agitator's fall an impact gun resting on a mechanic's leg was forced into the top of the mechanic's knee causing a severe fracture to his lower leg.

Employee was descending stairs while talking to a fellow employee and missed the bottom two steps falling on her knees.

While swinging a 3 pound hammer employee sprained his left hand. Employee's hand did not come in contact with anything.

Employee was going to move a small box weighing about 5 pounds from the floor into on of the shipping boxes and when she bent over and started to move the box she felt a pop in her left shoulder and felt pain.

Employee was replacing a loose liner with two other employees when it shifted and pinched his fingers between the liner and the assembly.

Employee felt debris in left eye while riding in a truck.

Rolled ankle while walking.

While boarding haul truck employee felt pain in right shoulder.

While carrying tools employee turned and slipped in the mud, causing him to fall and twist his back.

The employee was removing the wear package on a CAT 994 Loader bucket. The employee was attempting to loosen a lip protector striking it with a mild steel dead blow hammer. A small fragment of metal from the edge of the lip protector stuck the employee in the back of his left lower thigh. The employee was placed on light duty to assist healing.

Employee leaving building walking down the stairs caught his boot on the lower step causing him to trip and impact his right knee on the grating below.

While dismounting a dozer, employee slipped and bruised left knee.

Employee felt pain in right shoulder during the course of his shift and reported it to his supervisor. Physician placed employee on restricted duty.

While manually handling a rock, employee pinched his right index finger between the rock and structural steel. Although the employee was wearing impact gloves, the contact resulted in open fracture of the tuft of distal phalynx, which required 8 sutures.

While enlarging a hole with a burr bit, the bit and grinder kicked out and struck the left forearm, causing a puncture wound.

While holding onto a cable tray in a man basket, the employee's hand was pinched between the basket and cable tray, causing a laceration to the left hand.

The employee was walking and the wind blew some debris into the left eye causing a corneal abrasion.

Employee was descending right side ladder from boom truck. He slipped from step area as he was stepping down to the ground. Employee stumbled as he turned around and contacted this head on the outrigger cylinder housing, causing a laceration that required 4 sutures.

Employee was attempting to loosen a hose fitting. When the fitting broke loose the employees hand went foreword and struck a sharp edge. This caused his left small finger to be cut in the middle of the finger nail. Employee received 2 stitches and the remaining fingernail was removed. Employee was wearing gloves at the time of the incident.

Operator noticed smoke/flames coming from his haul truck. He activated the fire suppression system and attempted to leave the truck. There were some flames near as he exited the cab of the truck so he attempted to remove the fire extinguisher but changed his mind and exited the truck without the extinguisher. He got some burns on his arms and a small spot on the side of his face.

Employee was sorting steel grinding balls when her hand felt like it was cramping. She let it relax but still could not pick up balls. She states her hand starting hurting, her fingers started to tingle, and her hand and wrist swelled up.

Cutting a jug in the warehouse and states the knife slipped and cut his left wrist.

Haul truck XXR-611 was descending the Reona pad loaded. XXR-611 breached the left side berm on the second 90 degree corner. The truck continued to moved across the flat and came to rest against the berm of another haul road.

B-32 blade cat 16 h caught on fire while it was parked on the readyline. This happened between shifts. Extensive fire damage to the cab and engine compartment. No personnel were injured in this event.

Mechanic slipped while checking maintenance issue with haul truck engine. He caught himself with his left arm causing a strain to his shoulder. Employee didn't seek medical attention at time of injury but later requested to be seen by doctor and was diagnosed on 3-21-12 with a torn rotator cuff.

The employee was moving pieces of steel and pinched the end of the left ringer between the steel causing a fracture to the tip of the left ring finger.

While using a swing hammer, mechanic pinched his right pinky finger between the tool and the head bolt on the mantle. Tissue injury required three stitches.

Employee was removing a nut from bolt with a 3\4 impact gun while removing cutting edges on LD138. He was holding the bolt head with his left hand. When the bolt became unseated it started to spin and cut his left hand. Transported to medical facility and employee received sutures to the hand.

Employee was brushing off pants, a knife that had the blade open was in his pants pocket and the employee ran his right hand over the blade, causing a laceration to the right hand.

Employee was trapped by a rock for approximatley three hours, MSHA was notified of the entrapment immediately. Medical treatment rendered was limited to pain management and observation for a contusion. Employee was removing rock from jaw crusher that wouldn't crush, as he stood on the rock, it rolled onto his right ankle pinning him.

Mechanic was using a pry bar to remove old screen panel parts from a banana gravity screen. He had the bar underneath a piece of rubber and was prying when the bar slipped from his hand and struck him in the mouth. The employee received a split lip (minor first aid) and the employee's two top, center teeth were chipped.

While on a transducer well the small casing (2 1/2") got hung in the hole. A pipe wrench was placed above the driller's hand to help turn the casing past the hang-up and when the casing fell through the driller's hand was in a pinch point between the pipe wrench and the table of the drill.

While placing a bushing into the table on an angle hole the bushing slipped down and the employee's middle and ring fingers went between the bushing and the table. The driller activated the hydraulic cylinder to close the table and pinched the employee's fingers amputating the tips.

Employee was removing sample bags from sample bin and felt a pop in right shoulder.

While using a burning bar to remove the first liner in the last row of the concave, slag fell through to the surge pocket. Lube lines below the concave and pinion were covered with belting for protection. Slag fell directly onto the protective belting and ignited the rubber. Damage was minimal due to placement of the protective belting over lube hoses.

Crew was tripping out pipe when the jib boom arm broke and fell fracturing employee's left femur and ribs.

Operator completed a walk-around inspection on the dozer then moved crew van to parking area. While walking back to the dozer the operator tripped and fell to the ground and on his right hand palm causing a cut on the heal of his hand. Employee was taken to a medical facility and received two sutures for cut.

While employee was removing bolts from a panel, the panel sprung open and struck the employee in the nose, resulting in a minor laceration on the bridge of the nose which required two stitches.

Employee was assisting in the removal of a grader scrapping edge. Employee was picking up bolts after they had been knocked out by co-worker. When the last bolt was knocked out the employee went to pick it up and the unbolted edge fell onto his hand causing injury.

Employee was using a different methodology to install rubber liners in ball mill due to the fact that the liners would not fit using usual methods. A porta power was utilized to spread the liners on the shell of the mill to install the last liner. The porta power released unexpectedly and struck the employee above the eye.

Employee was installing pin on strut and was using short 4' step ladder. While descending ladder and standing sideways on lower step, he spun to left and with his right foot stepped off ladder and onto a 20 lb. sledge hammer that was standing with handle in vertical position and slightly to the left of the ladder. The employee had been using the hammer and placed it on the ground.

Maintenance tech had a piece of metal 1/2" X 6" X 72" clamped to the side of the low boy trailer. The metal was hanging over about 4" and was clamped at each end. He was grinding a 45 degree angle to the metal with a large grinder, during the grinding process with some pressure applied, the metal slipped and fell with one end landing on tech's right foot.

Supervisor was draining the contact cell con transfer tank. While he was pulling, closing the drain valve, he felt a pop in his right shoulder and then noticed an abnormal bulge in his right bicep.

Employee was threading casing onto sub and put fingers in end of casing to guide it. As casing rotated, three of his fingers on right hand were cut.

Employee was using a grinder with a wire wheel cleaning rust off of a lube truck. A wire broke off and lodged in the left knee. Employee was taken to a medical facility to have the metal removed and received stitches. He was returned back to work at full duty.

While stripping the jacket from an electrical cable, knife slipped and cut thumb, requiring four sutures.

Welder was Air-arcing on a 793D Haul Truck bed standing on the ground outside the bed when he noticed his shirt was on fire. Another welder in the area was alerted by the shouting and helped put the fire out. Burns to employee's back. He was taken to medical facility and released to work.

Employee felt a burning sensation in his left eye when he got on the bus to go home at the end of shift and thought he might have gotten something in it.

Operator was closing the right side cab window on LD249 (994F). Operator pulled window by the handle but didn't ensure that his fingers were clear of hinge area. Operator closed the window on right pinky finger.

While operating HT 959, employee was being loaded by SH102 in the F-1 6260 bench. During the loading process, a large rock fell from the hydraulic shovel bucket, striking the inside of the haul truck bed. This jarred the truck driver in the cab resulting in a sprain.

While welder was using a soft hammer to loosen an upper adaptor cover from the tree lock on a shovel bucket, a piece of iron splintered off of the cover and lodged in his leg.

Operator of SH017 was holding pin removong tool for mechanic, pin was struck with soft faced hammer, metal sliver came off pin removal tool and embedded into left hand of operator. Employee was taken to medical facility and sliver of metal was removed from employees hand.

Employee while exiting the pit van slipped on the snow and fell on his back, resulting in contusion of sacrum and coccyx.

Employee stopped to get samples at the grind sample area , she entered through the walk-through door, it was dark to her as her eyes & glasses hadn't adjusted to the lighting difference. There was a 3 " x 20' pipe laying in the walkway that she tripped over . As she tried to catch her balance, she landed on her left hand causing a laceration on her left palm.

EE crested ramp from stockpile in 777 haul truck and saw water truck watering ramp. Began catching up to water truck, tried to slow down, lost control and spun out striking base of stockpile with 777.

EE attempted to tighten a loose radiator cap with his left foot. Cap came off spraying hot coolant onto lower leg causing 2nd degree burns. Burn later became infected requiring reporting to MSHA.

Removing steering cylinder pin and pin popped out of boss. Employee moved to avoid pin but was struck in the head. He was taken to a medical facility and received stitches.

Employee was pulling up the hoist ladder for shovel 112. While pulling the ladder into the raised position, he placed his right thumb between the top of the ladder and the ladder stop smashing his thumb. He was taken to a medical facility and received stitches. X-rays also indicated a broken bone in the tip of the thumb.

Employee dumped a load at the tailings dam. When he pushed the dump handle down and sat up straight, pain shot down from his lower back into his left leg.

EE was walking to pickup @ night tripped in ditch & fell.He did not want medical attention @ time.Days later he complained of continuing pain and was taken for medical eval and released to work.@ next follow up he complained of continuing pain and was scheduled for MRI.MRI review EE placed on ristricted duty eff. 5-7-08.

While training Employee stepped into a grader ladder being used for a scenerio. When he placed his left foot into the ladder rung and shifted his weight he heard a pop and felt pain in his foot.

Employee was exiting ER 4 when his right leg cramped up and his left leg gave out causing him to fall on the stairs. He was carrying tools in both hands while descending the stairs.

The Employee was removing shoring for concrete and smashed his Right Thumb between the shoring.

The Employee was putting up scaffolding and caught his thumb between the scaffolding smashing the Left Thumb.

Employee walked out of the process shop area to start a pick up. He slipped on some snow covered ice falling to the ground and injuring his leg. He was taken to a medical facility for evaluation.

Employee was getting ready to remove and replace packing on the #1 reclaim pump at tails when he slipped on some ice in the work area and fell onto a bolt for the pipe banding and bruised his torso on the left side. He was evaluated at the hosiptal and released in restricted capacity.

Employee states he was mixing mud next to the hopper, when the other helper jumped into the water truck and began to drive away with a hose still attached (helper forgot to unhook hose) to the pump. The pump began to drag and came towards employee, who jumped out of the way and landed on the ground and felt pain in his knee.

While removing a pilot hose under the cab of shovel, the wrench that he was using slipped off the hose fitting of the hose he was changing allowing his index finger on his right hand to come in contact with a bolt head cutting his knuckle. Employee was taken to a medical facility and received stitches. He was released to full duty.

Employee states he tried to free up a drill spindle with a wrench and as soon as he tried, the rod dropped due to slack in the mainline smashing his finger between the pipe wrench and the foot clamp handle.

Employee felt his knee pop while trying to position a Duff Norton Jack under a loader.

Mechanic was using a pocket knife to remove a zip tie from a mirror bracket. While kneeling down the mechanic pulled the knife towards himself and the knife slipped causing a small laceration to the right thigh. Employee was taken to a medical facility, recieved sutures, and was released to full duty.

An air hose on an air gun was pulled tight when JLG that air gun was on moved. Employee tried to knock the air hose loose when the air gun attached to the air hose moved catching his finger in a pinchpoint.

While attempting to slide the liner to attach a lifting stap on the liner, employee felt his lower back pop. He was taken to a medical facility and placed on lift duty.

The employee was using a pry bar to remove a rock from in front of the wheel on a pallet jack. The load shifted on the pallet jack, which put pressure on the bar and smashed the employee's left index finger.

Injured employee walked away from half empty coffee cup. After working for a short time employee returned to his coffee and drank a couple of "gulps". Employee noticed "chemical" taste in coffee and began to vomit. It was discovered that a co-worker had put a liquid chemical de-icer into the coffee causing a possible poisoning situation.

He has been complaining that his wrist hurts. He fell while pulling liner out of ditch.-while walking tripped and fell.

Employee was opening radiator cap on equipment to check coolant level. Coolant sprayed out from under the cap and burned his fingers.

Employee stepped on a rock while picking up a stake on the drill pattern causing his ankle to turn to the side.

Employee was hanging up the chock block on his haul truck and missed the hook on the bumper. The chock block fell and pulled his arm down and strain/sprained his elbow.

Employee was loading a barrel of oil onto a lube truck with a co-worker who was operating a forklift. While using the forklift to lift the drum with a barrel clamp, the employee's finger got caught between the clamp and the barrel lid. He was taken to a medical facility for evaluation.

Employee states while lifting bag of mud, he experienced pain in lower abdominal area. Did not desire treatment at the time. Symptoms did not improve.

While cutting foam product employee states the box cutter he was using "slipped" causing a laceration to rt. palm.

While rolling welding cable off an electrical reel a large piece of mud came off the top section of the reel striking the employee on the right side of the face and head. The impact of the mud knocked off the employee's hard hat and safety glasses and mud entered employee's right eye resulting in a minor abrasion.

Employee felt a pop in his knee while descending down a ladder.

The wind caught the door of a track drill while the employee was exiting the equipment and caught his left "little" finger in the door of drill. He was wearing leather gloves at the time. Employee was taken to a medical facility, received 5 stiches, and returned to work.

Employee was checking oil level on rockbreaker, when he felt something in his eye. He was taken to a medical facility, given medication for the eye, and released to full duty.

Employee stepped off the drill, started walking around to put up drill stakes, stepped on a rock, and twisted his right ankle.

Employee was walking by a counter in the warehouse carrying a package of chop saw blades when his left ring finger hit the counter ledge, lacerating the knuckle. He received stitches and was released to full duty.

Employee states while removing hydraulic hose from shovel, the hose fell onto finger and pinched finger between hose and metal on shovel. Injury resulted in laceration/open fracture of right little finger.

Worker was walking alongside the backfill area for the overflow channel, when he stepped on a soft spot losing his balance and twisting his right ankle. The worker reported the injury to the Project Supt on 10/4/06 and chose to seek treatment the same day. The physician diagnosed a 2nd degree sprain, w/o restrictions. A 2nd opinion on 10/6/06 resulted in restrictions.

Moved wrong with a sample bag in my hands.

Employee was bent over at the waist using an impact to remove bolts on the cutting edges on a rubber tire dozer.

Worker was exiting the tractor portion of a "belly dump" and in the process stepped on a rock, twisting his right ankle. The worker reported the injury to the Project Supt. on 8/28/06, but chose not to seek treatment. On 8/31/06 employee stated he wanted to seek treatment and physician placed a splint on the injured area, but did not restrict workers duty.

Fire lasting longer than 30 mins. after discovery burned grass, HDPE pipe, and power poles near CIC. No injuries, no environmental release, no cyanide issues & no outside resources needed. Fire was caused by arcing on a jumper insulator on a line pole in an area where vegetation is dry. 3 water trucks responded ( 1 put out fire, 1 on hot spots, and 1 on stand by).

Worker performing finish grade activities in order to prepare subgrade for liner crew. In the process of removing a rock from the prepared subgrade, the worker "strained" his back during the act of lifting (bending over at the waist). Worker reported the injury to the Project Supt. om 8/18/06 and chose to seek treatment the same day.

He was carrying 6" plastic pipe on his shoulder, when the wind caught them, and he went to adjust his footing and stepped on a rock and rolled his ankle.

Employee fell and cut his thumb while exiting a doorway on the shovel. He was taken to a medical facility and received stitches. Employee was released to full duty.

Employee was cutting 3/4 chamfer with skill saw, saw bound and kicked back cutting ee's right knee.

Stepping upon concrete pad leg slipped in mud.

Employee was struck on the shoulder by a 4" metal ball that fell from a conveyor belt magnet discharge chute.

Filling sample bags bent over to shovel and back started to get sharp pains. It was hard to walk or sit.

EMPLOYEE WAS INFORMED BY CONTRL RM OF A PLUGGED CHUTE ALARM. EMPLOYEE WAS REMOVING CHUTE COVER. WATER CONTACTED DRY LIME CAUSING A BURP THAT COVERED HIM WITH LIME ON FACE AND BODY.

Employee was laying on the grating outside of the chute and reaching into the chute for conveyor #6 belt assisting co-worker. Employee used his left arm on the scraper mount to support and stabalize himself while he lifted the scarper blade with his right arm. While he was doing this he felt a pain in his left forearm.

Employee states while loading bags of drilling mud onto mud tank, he experienced popping in shoulder.

While cutting a bolt with a torch, a piece of slag ran down sleave of coveralls causing a quarter size 3rd degree burn on right forearm.

While fabricating machine guards for mill, employee pinched finger between two guards breaking the tip of the bone on the right middle finger.

Crew was tripping out of the hole when breakout wrench slipped off of pipe and kicked back. The wrench struck employee in left thumb and fractured the bone.

Employee was stripping a 3/0 XHHW wire, when the blade slipped off of the wire, allegedly cutting through a leather glove and cutting his left index finger above the knuckle. The laceration required 5-sutures to close. Employee was wearing leather gloves instead of a cut-resistant material such as Kevlar (as required).

Employee was walking and slipped on a patch of ice, landing on the right hip.

Employee tightened an anchor bolt and the nut got tight. The employee pushed harder on the wrench to snug the nut down. There was a welding BB in the threads of the bolt and when the BB broke loose the employee's hand struck the tank.

Employee was lining up a pipe support to weld it when he felt a pop int he right shoulder.

Employee was organizing laydown/storage area for demobilization of project, including stacking lumber forms and discarding unusable pieces of wooden formwork. The employee stepped on a nail that was protruding from a 2x4 piece of lumber that penetrated his foot.

EE WAS LOOKING UPWARD WHILE FITTING PIPE WHEN A 1/2" COUPLER FELL STRIKING THE FRONT TOOTH

Employee was attempting to exit cab of lube truck, facing out. He had ahold of door handle and handrail on exhaust stack, his foot slipped and caught the step on top of fuel tank. He hit his right hip on a concrete wall corner. He was taken to a medical facility and placed on light duty.

Employee was pushing 10' drukk rods on to bed of truck. Experienced pain in upper left arm.

Climbing down a wooden job ladder. Brushed arm against rail and picked up a sliver.

Using an air hammer to help removal of keeper pins on teeth for lip protector on a loader. Pin punch became wedged so employee tried to use air hammer on other side. When he pulled the trigger, his left little finger became wedged between the hammer and the next tooth.

EMPLOYEE WAS TIGHTENING A FILTER HOUSING WITH A CRESCENT WRENCH WHEN IT SLIPPED. HE BROKE AND CUT HIS FINGER.

Ee was cutting with utility knife when he slipped and cut his knee.

Employee was assembling shoring. He removed a form from upright staged bundle. Bundle slid forward striking employee and pushing him into a deep hole. He landed on his feet and buttocks fracturing his pelvis.

LIFTING 4 X 8 X 3/4" SHEET OF PLYWOOD. AT 11:30 AM FELT ILL AND FOUND BULGE IN ABDOMEN. DIAGNOSED AS HERNIA ON 5/11/05.

Employee altercation resulting in cut above eye on one of the the employees involved.

While climbing down the ladder of track drill (equipment #TD656), with one foot on the ground, and still hanging onto the handrail, the employee felt a pain in his right elbow. He was taken to a medical facility and placed on light duty.

Employee felt a twinge in his right shoulder while pulling a sample through the deck of the drill. Employee went home after shift, but the pain became worse, so he called his supervisor. He was then taken to a medical facility and placed on light duty.

He picked up box of boosters, turned and walked to truck, then complained of pain in lower back. At the time of the incident, he did not wish to receive medical attention. The following day, the employee wanted to seek medical attention. He was taken to the doctor and placed on restricted duty.

WHILE PLACING CONCRETE, EE WAS KNOCKED OFF OF THE FORM WALL BY PUMP TRUCK HOSE, CAUSING SPRAINED KNEE.
