Mill 6 Metal/Non-Metal

EE completed putting pump PP131 online. They walked by pump and the spool piece around the intake started spraying hot slurry. Hit them on right arm and right side. EE reported injury, seen by on-site EMS, went back to work. EE went to clinic on 8/23, diagnosed with cellulitis. On 8/26 follow-up put on restrictions. Site offering modified duty. Became reportable on 8/23.

Ee was working to dislodge mill balls from the discharge chute when one fell on EE's finger. Seen at clinic, diagnosed with a tuft fracture, left distal ring finger. Released to full duty and referred for orthopedic consultation.

Ee was moving forklift and placed EE's hand in a pinch point between the fork and the assembly. Reported and did not seek treatment. On 4/28 EE requested to be seen at the clinic. Seen and diagnosed with a fracture to the tip of the left index finger. Released with restrictions that do not preclude full duty.

EE was tightening a bolt when the wrench slipped and smashed right ring finger. EE was seen at the ER, diagnosed with a tuft fracture. Seen at Health the following day and released to return to work.

EE started climbing up the ladders and scaffolding platforms to help at the top of the ball mill discharge. EE got to the 4th scaffolding Deck and turned body away from the ladder and towards the deck. EE proceeded to step off and heard a loud pop from EE's left knee as EE stepped to the deck.

During the demolition and removal of 30" HDPE piping, IE utilized a Sawzall hand tool to cut piping. The Sawzall blade broke and became lodged in the pipe. IE was not aware that the broken blade was protruding from the pipe and rose from EE's position. As EE rose, EE's thigh above EE's left knee came into contact with the broken blade, causing a laceration.

EE was opening door on a tank. Door fell off the hinge, landing on EE's foot. EE was seen at the clinic, diagnosed with a fracture to right foot. Released with restrictions and referred to ortho.

Ee was walking outside at Mill 6 by the shack for RTO (oxidizer). Slipped on ice and fell. Seen at clinic, diagnosed with fracture, released with restrictions site can accommodate.

Ee reached around another ee to help hold discharge gate, and ee felt a pop and burning sensation in right shoulder. Sought treatment on 11/20, initial diagnosis was pain and arthropathy. MRI on 12/4, on 12/5 diagnosed with a tear in bicep. Put on restrictions, making this reportable.

The elevator attendant in Mill 6 was operating the elevator and had dropped a crew off on the 7th floor and was coming back down when due to loss of power the elevator stopped between the 5th and 4th floor. This elevator is well ventilated, we had clear communications, and no time did the operator feel distressed. No injuries or property damage associated to this event.

Ee was cleaning metal shavings from the lathe machine. While pulling the shavings by hand, some got struck and cut through ee's leather gloves and ee received a laceration to the tip of the right thumb. Seen at ER, sutures required, and ee released to full duty.

While preparing to replace patch on acid storage tank, a rapid energy release out of the vent line caused the line to burst, causing unexpected release of acid to spray through the deaerator on top of the tank. During the action of grinding the sparks were going towards the overflow pipe which caused an ignition due to the high amounts of hydrogen.

Employee loaded hat channel into bed of truck which stuck out approximately 3'. The employee walked into the channel during EE's lunch break while talking on EE's phone

Ee was pulling a metal hose. The hose caught on something and the ee's hand slid along the hose and left pointer finger was cut. Seen at a health clinic, sutures required to close the wound and ee released to restricted duty the site could accommodate.

Employee was removing a steel flexible guzzler hose from a scaffold. When they pulled on the hose it came apart exposing sharp edges that cut through the leather glove lacerating a finger. The employee returned to work under modified duty, but then quit and went back home to California afterwards.

Ee reports feeling pain in the shoulder when pulling self into mill. Diagnosed with a strained shoulder and released to full duty. Subsequently released with restrictions on follow-up.

Ee was putting a baghouse cage into its holder and the cage fell, pinching right middle finger between cage and holder. Ee seen at ER, diagnosed with fractured fingertip and laceration requiring stitches. Released to restricted duty.

Employee opens valve and gets debris in both eyes

A process operator was digging a ditch with a pick axe and inadvertently struck self on top of right foot.

An employee was using a mill bar to clean out a chute. As the employee was bringing the bar back, EE struck EE's hand on a cross member bar, causing a laceration to EE's right hand. The employee received six sutures to close the wound.

Contract elevator operator let employees off at level 8 and as EE began descending, the operator felt a jolt. The elevator came to a rest and was between levels 7 and 8. Employee was unable to exit the elevator as it was stuck between levels. Contract elevator company was called in and able to move the elevator, according to their SOP's, for safe exit by the employee.

Employee slipped on ice and dislocated EE's left thumb.

EE was completing inspections and got foreign debris in left eye. EE rubbed eye suffering a corneal abrasion and was prescribed antibiotics.

Employee was using an air lance to unplug the bottom of a tank that had acid solution in it. Received second degree burns.

The employee fell five feet from scaffold during erection. The deceleration device on his personal fall protection was deployed.

Employee reported a swollen right knee near the end of his shift on 5/10/2011. He had been performing electrical work and had to work on his knees (with knee pads) for periods of time. He was diagnosed with bursitis.

Driver unloaded product hose to start unloading crushed lime, he was connecting 4 inch cam lock fitting to silo and his back started hurting. He was able to stand up with help holding on to silo leg. Mine EMT checked him out and sent to ER. The e.r. doctor said it was muscle spasms and gave him a muscle relaxer and 24hrs bed rest with a follow up visit on the 24th.

While using a utility knife cutting a 2 inch plug out of a piece of rubber mat the employee's right hand slipped cutting his left index finger which required stitches.

While removing a piece of scale from the quench tank, the employee's left thumb was caught between the scale material and the tank, fracturing his left thumb.

While descaling a tank, the process tech slipped and fell, hitting his chin, requiring sutures.
