MTM was lifting Milwaukee battery operated come-along that was in the case from the floor to EE's plant kat
Employee felt soreness in left shoulder after using a 4' bar and 8 lb. sledgehammer. THIS INCIDENT BECAME A RECORDABLE ON 12/8/25 WHEN EMPLOYEE RECEIVED RESTRICTIONS.
While taking apart a connection for conduit, two MTEs were trying to loosen the connection with channel lock type pliers. The channel lock type slipped, unsure if the conduit or tool contacted the injured left side of the chin.
Employee was busting up rocks with a sledgehammer and putting pieces on the belt to clean the tunnel. Reported to manager that they felt a twinge in abdomen after they finished the job. Before starting their next shift, employee was seen by telemedicine. Hernia.
2 MTMS were removing the outboard bearing on a dust collector. To free the bearing from the taper lock, the injured was using a 3lb. hammer while EE's partner held a chisel in a chisel holder against the bearing. During this process a piece of the chisel broke off and struck the injured resulting in a laceration to the left forearm.
EMPLOYEE WAS USING A UTILITY TO CUT A CONVEYOR BELT WHEN THE KNIFE SLIPPED AND CUT EE'S LEFT LEG
EE didn't have a belt to hold up EE's FR work pants, so EE used zip ties through EE's belt loops as a makeshift belt. At the end of EE's shift, the EE grabbed a utility knife to cut the zip ties from EE's belt loops, the knife slipped and the EE cut the palm and thumb of EE's left hand. this resulted in stitches.
Employee was using a jig to install a pin in a loader. The jig has a nut at the end of threaded rod. The jig and pin tipped and the pin slid down the threaded rod and pinched employee's left pinky finger against the nut causing a laceration.
Employee was using a utility knife to cut loose zip-ties. While cutting the zip-ties the knife blade cut across the left thumb causing approximately a 1-inch laceration that needed to be closed with 2 sutures. Employee was cutting towards their opposite hand and not wearing any protective gloves at the time of the incident.
Employee was cutting rubber off the inside of a rubber lined pipe when the knife slipped out of the rubber. The blade cut through their glove, lacerating the employee's left forearm.
Employee was opening a HTHW boiler discharge to expansion tank valve with a 4' aluminum lever bar. Valve position is overhead and employee was pulling down on the lever bar and employee felt a strain in abdomen near prior hernia repair site. Because the "dull ache" has not gone away since, employee wanted to report. This became a recordable when the employee started PT on 11/13/24.
Employee was assigned to clean and repair deenergized medium voltage bus work. It was decided to remove a section of damaged ceramic bus holder. While working from a 14' ladder employee struck ceramic holder with a hammer. The ceramic holder broke in two and both sections fell with one piece striking employee in the finger pinching the left pinky finger against the ladder.
RMS Field service tech was removing a cross threaded bolt from a pump drive shaft, on an 830E-5 haul truck. As EE was repositioning self to get better leverage, the socket slipped off the bolt head and struck EE in the mouth/front teeth causing injury/chipped teeth. No other injuries or impairment
The employee was tightening a union and felt pain in their right shoulder. THIS INCIDENT BECAME A RECORDABLE ON SEPTEMBER 3RD WHEN THE EMPLOYEE RECEIVED LIFTING RESTRICTIONS
An Ironworker was using an adjustable wrench to turn castle nuts on the ends of traveling grate rods for the purpose of installing cotter pins. The wrench they were using slipped off one of the nuts and the palm of left hand struck a cotter pin on a nearby grate rod resulting in a laceration of the EE's palm and requiring sutures to close the wound.
Employee was cutting the tail end of a belt. The employee was cutting toward self. As the employee cut through the belt, the knife made contact with the employee's right thigh.
The employees were installing 4 pieces of packing material using a packing pusher. While attempting to insert the fourth piece, the employee used a pry bar to push the packing in place. The pry bar broke free causing the employee to step back. EE's left ankle became caught in the floor grating causing EE to continue to fall backwards, onto the ground.
MTM was assisting changing out a motor. As the motor was being lowered to the pallet, from a lifting jig, the MTM was using a no touch tool to tip the jig and motor on its side to remove the jig. As the MTM was pushing with no touch tool, the MTM felt a burning sensation and discomfort in the groin area.
Tightening grate casting nuts with an impact wrench, a piece of metal shaving flew off the nut/socket and landed in the employee's eye.
Employee was working on the 3A Fan tightening bearing cap bolts with a 3/4" drive ratchet and was using a pulling motion. Later in the shift the employee felt pain in their right shoulder. ** Additional information - A 1/2 inch adaptor was used with the ratchet to tighten bolts. This adaptor failed while the injured was tightening bolts. This occurred twice.**.
While replacing liners in a pump, the employee was using a 5' pry bar. The pry bar slipped causing the employee's right thumb to be pinch between the lining bar and the pump. This caused a laceration.
Employee was changing out a can on a conveyor belt. The employee was using a chain jack to lift the belt up to get access. The chain jack was hooked to an I-beam on each side. When lifting the belt the jack slipped off and struck the employee just above the right eye.
Employee was unplugging a belt discharge on 002-06 utilizing a "T" handled hook. The hook slipped off a rock, pinching EE's right thumb between the "T" handled hook and the top of the door opening
An Employee was using a pry bar, attempting to rotate a thrust collar in order to access a retaining bolt located on the back side of the rotor shaft. The pry bar slipped, and the momentum of the individuals pressure, carried EE's forearm into a piece of metal from the fan housing. Resulting in a laceration to the individuals left forearm requiring 19 sutures.
Employee was chipping ice/material buildup in the dribble hopper with a pry bar. While chipping the employee's hand made contact on top of dribble hopper causing a laceration to left index finger.
Belt line collapse. MSHA currently investigating.
Employee was using 15" pliers in the left hand and a 15" adjustable wrench in the right hand to tighten a thin 3" nut for a plastic bulk storage tank discharge. When applying downward pressure with the plier, the employee felt pain in the left shoulder. THIS INCIDENT BECAME A RECORDABLE WHEN THE EMPLOYEE HAD SURGERY ON 4-08-22.
While installing a new track pad on a Shovel, employee was attempting to install one of the pins. When EE struck the pin with a sledge hammer, EE nicked the edge of the pin and a piece of shrapnel went through EE's pants and struck EE on the side of knee. The shrapnel did not appear to go under the skin but did cause a small 3/4 inch laceration.
Employee was using a T-handle chuck wrench to tighten one of the jaws on a 4-jaw chuck and felt pain in EE's right shoulder. The T-handle wrench being used weighs 5lbs and measures 20" wide at the handles and 10-1/4" long. This incident became a recordable on July 20, 2021 when we became aware of employee having surgery.
Tractor/Trailer Extreme cold led to landing gear sticking. EE attempted to use pry bar to turn landing gear handle. Pry bar slipped and handle struck EE in left orbital, causing cut/swollen eye, and broken bone.
Employee was taking a pellet sample in the step II Agglom from line 4. While taking the sample a piece of slag hit the scoop knocking it out of employee's hand with the handle striking employee in the chin. Employee was sent to telemedicine and returned to work without restrictions but does state that employee has 2 broken teeth on the upper left side in the back.
Employee was hammering out a guide idler pin with a 4 lb.hammer. Employee missed hitting the pin and hit their right knee.
Employee was removing a floor plate with pry-bar to access the oversized chute. After several attempts of prying and hammering to loosen the plate, the plate popped loose causing the employee to pinch EE's right index finger between the hammer and the pry-bar.
Employee was installing a guard on the west side of a classifier when EE's right ring finger was pinched between the guard and the lip of the classifier tank.
Employee was working on the 278 loader installing control valve. As EE was tightening the base bolts with a 1/2 inch ratchet and 6 inch extension the wrench slipped off the bolt causing EE to smash EE's right pinky finger on the axle housing. At this time employee notified EE's manager
Injured was using 12lb sledgehammer to install concrete anchor bolts in mill sole plate. While swinging hammer ee struck an adjacent coupling extending into the swing path pinching the tip of right index finger between the coupling and the hammer handle resulting in laceration.
Employee was installing the motor for the 206-07-1 pump. As employee was driving in the key for the sheave the hammer missed the key and hit the left thumb. Employee's thumb was pinched between the hammer and motor shaft. Employee was using another piece of square stock to drive the key in.
Employee was using 4 ft long steel pry bar to dislodge material in a pan feeder. As ee was striking the material ee struck/caught right little finger between the bar and the side of the feeder opening resulting in a fracture to the proximal metacarpal of little finger
Employee was holding a wrench in left hand and with right hand hit wrench with hammer. Left pinky finger was caught between beam and wrench cutting the finger.
Employee was pulling with a 36" pipe wrench on a pump shaft. The wrench slipped off and employee fell backward. Employee put out right hand and caught self from hitting the floor. After that, employee felt pain in right tricep.
Employee was in the Mine Annex dry getting ready for the day. The employee grabbed a new pair of gloves. The gloves were secured together with a zip-tie. While holding the knife with their right hand the knife slipped and cut between the thumb and fore finger on the employee's left hand.
Employee was using a brass hammer to remove pins on the 4111 shovel in order to replace the wear package. The next morning they noticed a small spot of blood on their upper chest and reported to their supervisor.
Employee was burning out a keyhole bolt in 071-12 pan feeder on the west side of the discharge chute. The employee stopped to chip out dirt/slag out of the hole using a chipping hammer. After employee struck the hole in the liner the slag/dirt ricocheted behind their prescription safety glasses.
The injured employee was working on clearing the plugged toilet seat on 071-04 pan feeder with a T-handle hook. As the employee dislodged a lone rock from the toilet seat, material above came down and pulled the T-handle hook downward pinching left index finger between the handle and the safety bars causing a laceration and a broken tip of the employee's left index finger.
Employee was hooking up a portable heater when ee felt pain in left groin area. Employee was later diagnosed with a hernia. The employee had surgery to repair the hernia on 1/6/17.
Employee felt discomfort to abdomen when operating a chain jack on 8/31. The discomfort got worse overnight, so the employee reported the incident on 9/1.
Employee was removing a sight glass off of a pump. As EE pushed on a wrench, EE felt discomfort to right shoulder. Was informed of PT on 6/17/16.
Employee was tightening a lip liner bolt with a 15/16" end wrench. As the employee was pushing on the wrench, EE felt discomfort to EE's right shoulder. Was made aware of Physical Therapy on 4/21/2016.
Socket slipped off a bolt which caused the ratchet handle to strike employee's tooth
Employee was changing teeth on the #13 shovel. The employee stated that as he was pounding out the last pin on the tooth, he felt muscle spasms/hurting his right arm.
