Employee was demoing an out of service AC unit. Employee unbolted and removed the first section of the AC frame, set it on the floor, and reached for an impact. When they reached for the impact, they tripped and fell down to their left knee. Their left knee and shin landed on the piece of demo that was on the floor and cut their shin.

Employee was busting up rocks with a sledgehammer and putting pieces on the belt to clean the tunnel. Reported to manager that they felt a twinge in abdomen after they finished the job. Before starting their next shift, employee was seen by telemedicine. Hernia.

The employee was guiding the belt in along the west side walkway of the 27 conveyor when EE stepped onto some cross bracing where grating was removed and fell approximately 4' to the ground striking EE's face on the conveyor structure cutting EE's left cheek and damaging some teeth.

Employee was pushing a full oil cart through the PM stall next to a loader. The cart shifted to the right and pinched the employee's right middle finger between the cart handle and the rim of the loader.

Individual mis stepped while walking down a small flight of stairs and rolled an ankle resulting in a sprain/strain injury

EMPLOYEE WAS USING A UTILITY TO CUT A CONVEYOR BELT WHEN THE KNIFE SLIPPED AND CUT EE'S LEFT LEG

Employee was lifting a box weighing approximately 35LBS putting it into a pickup and felt a pop in left bicep

61 was filling A fuel truck. They felt their back getting warm and saw that there was a fire. They exited the truck and ran a safe distance. Called the onsite fire department and also had the guard shack call the fire department. There were no injuries, and the truck was a total loss. Fire reported at 7:52 and MSHA was notified at 8:17 am.

A tree blew over into the power lines near the Basin Shops building at approx. 11:27 AM next to the road. Basin employees expelled 4 extinguishers and called Keetac Fire to respond. Keewatin fire department was called and the DNR and Nashwauk fire department. Basin cat was used to assist in cutting access. Fire was extinguished before 3 PM. MSHA was notified at approx. 11:56 am.

Rolling a pin into a skid steer bucket and felt slight pain in EE's lower back.

EE was grinding/welding as part of bull gear repair that was taking place. They stopped grinding to visually check for high spots. When they flipped up their face shield, they felt something enter right eye. it was determined that the EE had a Corneal abrasion as a result of a small piece of grinding debris entering their eye.

Employee was using a jig to install a pin in a loader. The jig has a nut at the end of threaded rod. The jig and pin tipped and the pin slid down the threaded rod and pinched employee's left pinky finger against the nut causing a laceration.

Employee was moving a 17" by 17" piece of broken shell liner (discharge liner) to place lifting sling around for removal. The Liners weight came over center, the injured reached for the liner, grabbed a sharp edge and cut EE's right palm area. The liner cut through the TIG welding glove. Cut was approx 2 inches in length

While walking out to the service truck behind the weld shop by door 8, employee slipped on ice and fell catching self with right hand. Felt soreness in said hand immediately.

Employee was pulling a new hydraulic hose out of the back of EE's service truck that still had plastic on it. EE took EE's gloves off before handling the hose so EE could rip the plastic off of it. When EE pulled the hose EE felt a sharp pain in EE's left index finger. When EE looked EE had about a 1 inch laceration on the end of EE's index finger

Driver was stepping off the haul truck stairs onto the ground & twisted right knee & ankle. This became a recordable incident on 12/3/24 when the employee received restrictions.

EE was re-attaching the skirt board and liner bolts on the conveyor belt in the #1 Crusher, 6th level down. While pulling on a wrench tightening a bolt on the skirting board, EE lost EE's hold on the bolt and slipped forward, extending EE's left leg, and fell backwards. When EE slipped, EE attempted to catch self, but EE's left leg came out from under EE.

The bearing in the 28 belt head pully seized, igniting the belt. The fire was declared extinguished at 12:00pm

The employee was tightening a union and felt pain in their right shoulder. THIS INCIDENT BECAME A RECORDABLE ON SEPTEMBER 3RD WHEN THE EMPLOYEE RECEIVED LIFTING RESTRICTIONS

Employee working on filter head from step ladder. Vacuum pipe alongside ladder came loose from roustabout, rotated and slammed into ladder knocking employee off. Employee landed on EE's elbow. THIS INCIDENT BECAME A RECORDABLE ON SEPTEMBER 12th WHEN THE EMPLOYEE RECEIVED RESTRICTIONS.

Injured right ankle stepping down from bucket after lighting welders welding torch. Did not treat at all in 2024. just recently went to the doctor. no restrictions or lost time

An employee was helping make repairs on a shovel with 2 other mechanics. The injured employee suffered a seizure and was convulsing face down on the steel plated floor. First responders were called and the team was able to get the injured off the shovel. There was no fall incident.

Three Jamar operators were in the process of stowing the jib on a Grove RT765E-2 mobile crane. The injured worker was standing on the crane where the jib extension pins to the side of the boom. As the boom was being retracted into place, the injured workers left hand was pinched between the head end of the jib and its mounting bracket (where the jib pins to the crane).

The injured employee was removing the bolts for the discharge pump on the 3P1042 pump. When removing the last bolt, the pipe moved pinching employee's right hand between pipe and the motor base for the pump.

Employee parked the 4252SS rock spreader truck. When the employee took the last step from the ladder to the ground the employee slipped and fell landing on left leg injuring ankle

MTE was going to walk downstairs from the 2nd floor to the first floor. There is a cement landing at the top of the stairway. As the employee stepped on the landing, EE's right knee gave out. THIS INCIDENT BECAME A RECORDABLE WHEN THE EMPLOYEE STARTED PHYSICAL THERAPY ON 4/8/2024.

Employee was oiling a pneumatic impact and stepped over the bolt removal machine & caught their right boot on a hydraulic line. EE shook their boot to free the line, in the process reached out with their right arm against the mill housing to steady themself. At this point the employee felt/heard a crunching sound and immediately experienced discomfort in the right shoulder area.

Tightening grate casting nuts with an impact wrench, a piece of metal shaving flew off the nut/socket and landed in the employee's eye.

Fire in the Pellet Plant, middle of Kiln Burner Floor. Mutual aid came from local fire dept. The fire was extinguished with an extinguisher and water was sprayed on the gear box to cool down the fire and hotspots. The kiln was restarted at 4:32 am. MSHA was notified at 4:30 am.

Employee had stepped onto the hitch mounted platform to retrieve a shovel from the pickup box. While descending from the platform, the employee lost their balance and fell, landing on their right wrist.

Employee was driving a haul truck after dumping on middle A dump. As the employee was driving down the ramp the truck hit a bump jarring the employee's wrist. The employee stated that it didn't hurt at first but started to swell up over lunch break. The employee became restricted effective 5/12/23.

Employee had collected pellet sample and was working it up. They were lifting the pan and sample (approx. 27 lbs.) to put into the Gilson for screening when they felt a burning/tearing sensation in their right shoulder. Employee reported to plant supervision when the pain increased.

Employee was exiting the break room and pushed on the glass portion of the door and the glass broke. Employee received a one-inch laceration on the palm of the left hand.

Employee was exiting 4259 Haul truck at the end of the shift. Employee caught their heel on the bottom step of the ladder and fell to their knees on the ground. Employee stated that right knee landed on crushed rock which caused a laceration.

Employees finished clearing a rock jam in the #2B Feeder at the #2 primary mill conveyor tunnel. The rock jam had caused some spillage of rocks off the belt and into the walkway. Employee was walking up the tunnel when EE slipped on rocks in the walkway causing EE to fall to the ground and land on EE's right shoulder. Became a recordable on 9/8/22 when physical therapy started.

While aligning a clamp and pipe that was being repaired, the employee had EE's left middle finger in a pinch point between the pipe. The tip of EE's finger was lacerated resulting in an amputation just below the first knuckle of the left middle finger.

An Employee was using a pry bar, attempting to rotate a thrust collar in order to access a retaining bolt located on the back side of the rotor shaft. The pry bar slipped, and the momentum of the individuals pressure, carried EE's forearm into a piece of metal from the fan housing. Resulting in a laceration to the individuals left forearm requiring 19 sutures.

Employee was removing a bolt on door on the 2A Dust Collector - clean side. When the employee went to lift the door off, the employee felt a pop in right forearm. EE contacted foreman and EMT's checked them out and EE was sent to telemedicine and returned with no restrictions. THIS BECAME A RECORDABLE ON 4/14/22 AFTER THE EMPLOYEE HAD A DOCTOR VISIT ON 4/14/22.

Employee was off-loading worn shovel teeth by hand off a service truck. One of the teeth struck another liner on the ground, spun around onto the employee's right ankle, pinning right foot.

The employee was pushing in a MV breaker back into its cabinet in the Keetac substation when they felt pressure in lower right abdomen. There were two other employees helping push the breaker, but the last portion is very hard to do with more than one person.

Employee was removing a sling from a 67" long piece of 10" rubber lined pipe. The pipe was near a spare finisher drum. The employee was pulling on the sling and the pipe started to roll towards the magnetic drum. The magnetic drum pulled the pipe to it, pinching employee's right middle finger between two objects.

Employee was working on changing belts on 68 pump in the screen house. EE bent down to help guide the belt onto the sheave and that is when EE felt a pop behind right knee, EE then grabbed partner so EE didn't fall to the ground.

Employee was assigned to install new drive on the #10B Primary Feeder. Employee reported was pulling and manipulating a 1.5" cable in order to connect the cable into the top of a disconnect. As employee was working with this cable, the employee felt a pop in left hip.

Employee was transporting a tail pulley with bearings down line 6 crude belt tunnel. When guiding the pulley past the feeder the pulley shifted causing the employees finger to be pinched between the pulley bearing and A feeder ladder. Employee sustained a laceration on their right index finger.

the hardnose was tipped forward and supported by the field service truck crane. difficulty getting the hardnose in place so a come along was being used to assist. the cooler became lodged and the cooler was adjusted the hardnose fell back towards the engine briefly pinching the tech between the hardnose and the engine. They broke left collar bone

Employee was operating the 4111 shovel in crude blast. The employee was cleaning and then tracked ahead into the bank. When employee took the first scoop the shovel jarred employee causing neck pain along with a headache. The employee stated must have had a rock under the track that broke causing the sudden jar

While installing a new track pad on a Shovel, employee was attempting to install one of the pins. When EE struck the pin with a sledge hammer, EE nicked the edge of the pin and a piece of shrapnel went through EE's pants and struck EE on the side of knee. The shrapnel did not appear to go under the skin but did cause a small 3/4 inch laceration.

Employee was changing a drill bit on the 4517 drill. While maneuvering around the bit basket on the deck of the drill, the employee bumped their leg on the corner of the bit basket, resulting in a 1" laceration on their right leg

Employee was opening a battery/DEF cover on a fuel truck. The cover was stuck and when the employee pulled on the cover, felt pain go through left shoulder.

Three employees were trapped in the number 2 crusher elevator between floors at approximately 9:20am. MSHA was notified at 9:46 am. Maintenance was able to move the car and free the employees at approximately 10:13 am.

Employee was doing a pre-shift inspection. As EE was putting a wheel chock into the holder, EE slipped on a small wind row of material left behind from grading. EE fell onto right side and reported pain in right shoulder and knee. EE was sent to telemedicine at Minntac for evaluation. This became a recordable incident on 2/25/2020 when employee received restrictions.

Employee was loosening plow bolt on a loader with EE's left hand. EE's right hand contacted a trigger of an impact gun that was resting on the bolt nut causing the bolt to rotate, cutting the employees hand through cut resistant gloves.

The injured employee parked on the South side of the pit fueling station to add coolant to the 4381 grader. The employee was dismounting the grader after filling the coolant, and lost footing, falling backward off the grader, landing on back. The employee hit head and injured back in the fall. The manager was notified and medical was dispatched immediately.

Employee was performing a pre shift inspection on the 4270 Cat haul truck at the pit dry shift change area. The employee finished checking the engine oil level, and turned to finish their walk around inspection when they felt a pop in their right knee

Employee was assisting pond crew with pipe repair. Tailing pipe is located in a ditch with steep side slopes. While returning to the truck, employee had to climb a hill. Employee lost footing on slope, but was able to hold on to a small tree to keep from sliding. Employee complained of knee pain, back pain and shoulder pain. Note: weather was rainy

Employee was moving concrete like cover away from wall to retrieve an item from behind it, the cover shifted and pinched middle finger on right hand between wall and cover resulting in a laceration.

EE was gathering welding lead to weld in another area. EE stumbled and fell backwards from same elevation and struck EE's left elbow against a piece of steel. EE received 3 sutures to EE's left elbow.

Employee was clearing a chunk of slag out from the #3 product feeder with a pneumatic jack hammer. When the chunk split in half and fell, it pushed the jack hammer bit down while pivoting off of the side of the feeder and the body/handle end of the jack hammer pushed upwards. The employee's right index finger was pinched between the jack hammer handle and an overhead water line.

Employee was descending stairs and missed a step and felt pain in the knee. This did not become a recordable until 3/8/2019 when the employee's doctor restricted employee.

The injured employee grabbed a piece of metal liner that was lying in the deadbed. Pulling it from the deadbed, the metal piece slid out of employee's right hand and cut through the employee's leather welding gloves resulting in the laceration. The liner weighed between 18 and 20 lbs and had a razor sharp edge on the one end of the liner

EE was lifting up a steel stair stringer to bolt to new platform. The stair stringer slipped out of EE's hands and came down on EE's right foot fracturing the big toe and index toe. EE was not wearing the required safety toed boots at the time of this injury.

Employee was operating the 4110 shovel digging in the bank when the shovel jerked forward & backward and employee felt a pop on the left side of the neck.

At 8:45AM, 4 MTM's and 1 manager were trapped in an elevator when it stopped between floors. Employees phoned for help. Electricians tried but could not get the elevator to run so MEI Elevator was called out. MEI worked on this elevator the day before. MEI and plant personnel got the elevator to the floor and got the employees out by 10:17AM. MSHA was notified of the incident.

Employee was using 4 ft long steel pry bar to dislodge material in a pan feeder. As ee was striking the material ee struck/caught right little finger between the bar and the side of the feeder opening resulting in a fracture to the proximal metacarpal of little finger

Crew was using Catepiller Pin Pusher tool to install pin in pan feeder drive chain. Injured EE used a 10x1 inch bolt as a spacer to extend the length of the ram to finish driving the pin. As pressure was applied the bolt slipped and ejected striking the EE in the hardhat and forehead resulting in a skull fracture.

Injured EE was working with crew installing new pan feeder at base of bin. Rock debris jammed the drive chain and as EE was clearing that debris a large rock slid down and struck their left little finger resulting in a 12 stitch laceration.

Employee was moving a portable gas heater in the cooler basement and strained their back. Employee was pulling the heater backwards when their foot slipped in some pellets and employee felt a sharp pain in their back. The heater has wheels on one end and hand rail on the other end to be able to maneuver them around.

Two employees were stuck in the #1 Crusher elevator at 12:40pm on 6-04-17. The elevator was stuck between the basement floor and the 1st floor. After an electrician could not get the elevator working again they had to wait for the elevator company to arrive out of Duluth to get the elevator to the basement floor. This occurred at 2:25PM.

Employee was using a brass hammer to remove pins on the 4111 shovel in order to replace the wear package. The next morning they noticed a small spot of blood on their upper chest and reported to their supervisor.

Employee was welding on 4110 Shovel on the left hand side of bucket. Slag fell onto the employee's left metatarsal boot, under the right side flap for laces. The hot slag burned through the top of the boot and sock, causing burns to the top and right side of EE's left foot.

Employee was disconnecting a chain from a pump case, Second employee was grabbing the jib crane hook to move it out of the way as employee readjusted, the hook slipped from hand and swung towards the injured employee contacting injured employee in the mouth which caused a 1/4" cut to the right upper lip.

Close quarters getting in and out of SW Gear. Electrician caught employee's ear on a bolt and tore employee's right ear.

Employee tripped over a cart in a well-lit walkway while trying to find a local disconnect switch. The employee fell over the cart and unto right shoulder.

Employee was hooking up a portable heater when ee felt pain in left groin area. Employee was later diagnosed with a hernia. The employee had surgery to repair the hernia on 1/6/17.

employee was using a wash down hose to clean a slurry tank. The hose ruptured approximately 12 inches from the nozzle. When the failure took place, the employee lost the grip of the hose and was struck above his left eye.

Employee was pulling sprocket off the equipment. When it came loose his foot got caught inside one of the tracks & his momentum forced him to ground & his foot was still in track. He was taken to ER right away by coworker. At the hospital they did X-rays & found out that his ankle was broken a Posterior Malleolar Fracture & needs surgery.

As an electrician opened a 4160V Disconnect, there was an arc causing the employee to receive 1st and 2nd degree burns.

Employee was walking to his personal car and slipped on ice.

Injured was rotating a corner cutting edge on a rubber bull. The injured felt discomfort to his right bicep as the injured applied upward forced on a 2 foot lining bar.

Employee was stuck in elevator starting at 3:10am. Supervisor was called at 3:25am, guard shack at 3:43 am. Employee exited at 4:08 am. Guards cleared scne at 4:16am. Safety was notified at 4:16 am, MSHA called at 4:24 am. Employee experience 38 years/32 weeks. 39 employees on site/0 contractor.

Employee was assisting in the assembly of a 994H loader tilt link using a service truck crane. During install the crane malfunctioned and tension was put onto the lifting sling that was being used to install the link. EE then pushed on the link to rotate it into position and the tension on the strap caused the link to jump causing his hand to become crushed.

Employee was changing wear teeth on 4111 Shovel. After removing the worn tooth, he was carrying it to the service truck when he slipped and felt lower back pain.

In the process of positioning a cabinet, the injured pinched his right ring finger. Employee received 4 sutures.

Technician was replacing the fan drive motor on the engine of machine. When technician turned at the waist to move from the bumper of the machine to a step ladder near machine he felt a strain in his groin.

Employee was inspecting a fire extinguisher when he felt a stinging sensation to his back.

Employee received a laceration to his right middle finger when reaching to obtain a spare probe. Employee's finger came into contact with a piece of channel.

Employee was moving a roller and felt discomfort to his back

Employee felt discomfort to his right knee when boarding the 4334 Skid Steer. Employee had surgery on 2/24/15

Emergency Services Officer was conducting his pre-shift vehicle inspection. While walking near the squad he tripped over a curb. He extended both arms hands out to break the fall. After landing he felt a pain in his left shoulder and left thigh.

Injured was inspecting for cracks in the short rake section of the #2 thickener tank rake arm. He used a retractable lifeline and full body harness to descend a fiberglass extension ladder to access the center of the weir to do the inspection. While on the ladder he lost footing and fell striking adjacent steel structure resulting in contusions to his lower leg and a concussion.

Employee pinched his finger between a diverter plate and a handrail.

Employee was prescribed physical therapy for his shoulder. We were notified of this physical therapy on 3/4/2014.

An employee suffered first and second degree burns to his face, neck and wrists when the employee opened up the inspection hatch of heater, introducing oxygen to the chamber resulting in flames rolling out of the inspection hatch.

Employee got his right thumb pinched between a hinged door and the frame of a dust collector.

Employee was grinding with a Dewalt hand grinder, slipped and cut back side of left hand. Stepping from beam to beam and slipped.

Employee caught his right ear on a bolt.

Improper rigging shackles too large for the 2 ton hook they were in resulting in the top shackle being pushed through safety latch on hook. This resulted in West side of piece falling striking employee's right knee.

A piece of ice/mud struck the employee's back.

Control operator observed large swings in the coal feed system. The system was shut down and restarted approximately 2 minutes later. Upon reactivating the system, the coal explosion suppression system activated. The suppression system detected an increase in pressure and activated working as it is designed. No employees in the area and no injuries reported from incident.

Employee's right hand came in contact with the edge of a block support angle. Employee received 3 sutures.

Limit switch failure. Phoned MSHA @ 8:10 a.m. Trapped personnel were Maintenance Manager, MTM and Laborer. Time of release 8:30 a.m. There were 270 hourly employees, 43 salaried employees and 78 contractors working on site at time of entrapment.

Employee felt lightheaded and dizzy. In the process of sitting down, the employee fainted and struck his head.

Elevator stalled for 211 minutes. MSHA was called on 12/05/2011 @ 8:38 a.m.

Employee's knife slipped while cutting rubber. He poked his thigh with a knife.

Employee received a finger laceration while cutting a piece of stainless steel.

Cut left hand middle knuckle while removing a nylon wire tie with a knife.

Employee raised a ladder on a hydraulic shovel and felt soreness in his back on 03/04/2011. On his next scheduled shift (3-9-2011) he returned to work with restrictions.

Employee was welding in confined space (had welding hood on and appropriate safety glasses with side shields on). Particle entered eye from air current in confined space. Particle entered past employees safety glasses from behind.

Employee was loading empty 55 gallon drums (approximately 30-32 lbs each) into a company delivery vehicle. Employee experienced a strain in his left bicep while lifting a drum. No other conditions contributed to the injury other than the motion of lifting and stretching.

Vehicle was stuck in soft mud on side of trail. Employee dug out wheels and proceeded to pull ferns to reduce the softness of mud. Employee pulled a clump of ferns and caused a laceration on left pinky finger.

An elevator was stalled in the Concentrator for approximately 1 hour 15 minutes.

Employee was welding inside a fan housing while an air mover was ventilating the space he was working in. As other workers walked in front of the air mover, dust was blown inside the space resulting in the employee receiving a foreign body in the eye (dust).

Employee was using a claw hammer to chip away concrete around a door. While swinging the hammer, the employee cut his index finger on a metal strap fastened to the wall.

Employee was installing a motor and wiggled the motor to get it to line up. The employee reported tightness in his back.

While operating a wire feed welder, a small piece of slag hit employee's eye. Employee received prescription eye drops which safety learned was reportable on 4/29/2010.

While lifting a pump with a backhoe, an aluminum hose fitting broke, causing the hose end to swing out and strike the employee in the forehead.

While mopping a floor, employee felt a pop in the left side of her lower back. Note: Employee was permanently transferred 3/1/10. Her transfer was due to seniority at the Co. Safety was not made aware of the transfer until April 20th.

Employee attempted to stabilize a cylinder that shifted while hanging on a motor crane. When he did this, his finger was pinched between the chain and cylinder.

Employee was attempting to loosen some mill balls from a chute with a T-hook. When the balls let loose, they jarred the T-hook, which caused his right hand to strike the side of the chute.

Employee was dismounting a reel truck and slipped and broke his right leg.

Employee received burns on her ankle when hot pellets fell from cooler launder.

Employee was loosening nuts on a pump motor; he caught his little finger between the wrench and the edge of the opening on the riser.

EE's shoulder became sore throughout the day while changing a balling feed screen on 4/21/08. This was initially a no treatment case, but the EE began receiving physical therapy on 6/12/08. *Per Dr's notes, this is an on-going problem for employee. MRI indicate Arthritic changes of the joint. Symptoms & pain were evident 7-8 months prior & not related to activites of 4/21/08.*

An explosive atmosphere in the coal mill caused air ducting to dislodge at the expansion joint.

While employee was tightening a nut on a spring loaded bolt, he held onto another bolt that was located in a pinch point, for leverage. He severed the tip of his left ring finger.

Employee was striking a pin with a hammer in order to dislodge it. While striking the pin, a small piece of the pin sheared off and struck him in the right inner thigh causing a small laceration.

While cleaning a chute, employee fell on her left knee.

Employee fell in void created by bridged over material. Currently on restricted work. A call was made initially to report a serious injury: subsequently the injury was determined not to be serious as defined by MSHA-On 3/9/07 Empoyee became a restricted work case.

While employees were attempting to thaw a frozen drain, a fire erupted.

While removing rubber coupling that connects cooling lines on a haul truck, employee cut his left forearm with a knife.

Elevator stalled between floors.

Walking to break and the employee slipped on icy surface, he lost his balance & fell on his left knee.

While changing self-aligning troughing roller, employee injured his back.

While positioning angle iron that was located on a steel rack to be cut, employee strained his back.

VACUUMING UP MATERIAL, WE HAD TO USE A SLEDGE HAMMER TO BREAK UP FROZEN MATERIAL.

Injured left elbow when welds broke loose on right side of shovel access ladder he was using.

WHILE ATTACHING A NEW RUBBER-LINED PIPE TRANSITION TO A NEW RUBBER-LINED PLUG VALVE, EMPLOYEE'S FINGER WAS PINCHED BETWEEN THE TRANSITION PIPE FLANGE AND THE VALVE FLANGE.
