The Electrician was in the process of pushing a fish tape (tool used to pull conductors) into a raceway where it became difficult to push through the raceway and used his needle nose pliers to assist him and while pushing the fish tape the needle nose pliers slipped of the fish tape and struck himself on the lip and chipped his tooth causing an open wound on the upper lip.

The electrician was stripping conductor in preparation of pulling the conductor and making terminations at the control panel. The electrical moved backwards while stripping the conductor and bumped into another electrician. The knife jumped out of the grove it was in and into his hand. The blade penetrated both a leather glove and a Kevlar glove.
Employee struck a pin hammer with another hammer causing a metal projectile to contact his chin.
Employee cut left index finger while changing a blade on a razor knife. The cut required seven stitches.
Electrician was injured when a pry bar he was using to place an electric panel slipped and cut his left thigh just above the knee. The injured worker was transported by a supervisor to the nearby medical center where the wound was cleaned and sutured. Returned to work the same day with no restrictions.
Employee was using a pry bar to remove concrete forms. The chisel end between the form and concrete slipped and employee's right hand struck the rib. The point end of the bar drove through his glove and hand between the thumb and index finger. Medical treatment. No days missed.
Employee was cutting bands off bundle of wire mesh, when one of the bands sprung up and struck employee in the face. Medical treatment necessary, no days missed, no restricted work.
Employee was punching out a brass bushing on a drill machine when he missed the punch and smashed his left index finger between the hammer and the punch causing a blood blister and swelling to the finger.
A groundman at the Dragline #3 was assisting in the resocketing of the hoist ropes. As the EE swung a #20 sledgehammer, he missed square impact and glanced off the hoist wedge and hit right side steel-toed boot. The EE refused onsite treatment and was transported to a medical facility for evaluation.
Employee was using an 8 pound sledge hammer to strike a breaker bar. The hammer glanced off and pinched employees right index finger between the hammer handle and the breaker bar.
Employee was using a pocket knife to remove a zip tie when it slipped cutting his left thumb. Employee received 4 stitches as a result of the laceration.
When removing the S-cam on brakes, the employee struck it with a hammer and a piece of metal went into his left eye.
Two employees were leveling the rakes on #5 Thickener. They had a wrench on one of the adjusting bolts for the rake. Employee 1 was pulling and employee 2 was pushing on the wrench. Employee 1 felt a sharp pain to the lower back that traveled to the lower abdominal area. * Started losing time on 8/25/09
Employee was swinging a sledge hammer and missed what he was swinging at. Employee felt like something pulled and reported next day.
Employee stated that he was assisting another employee with a wrecked jeep when the jack they were using, came out from under mantrip and struck the employee on the left elbow. EE FLWD was 7/13/09.
Employee was raking on Flat Creek Main beltline when he felt a burning in his left shoulder. Employee's first lost work day was 9/25/09 due to surgery.
Employee stated that he was moving a pump with a metal bar when he stepped on some loose coal and lost his footing. The pump shifted and caused the bar to jerk his arm. Employee's FLWD did not occur until April 15, 2009.
Employee was using a com-a-long to raise a pipe in order to tighten support chain. A clevis that was supporting the com-a-long spread apart, allowing the com-a-long and pipe to drop, pinching employee's finger between pipe and support chain.
Employee stated that he was attempting to cut a rubber hose when the knife slipped and he cut his left index finger.
Welder was knocking on a knocker wrench with a sledge hammer, when the nut loosen it caused him to over swing the hammer causing him to hit against a protruding bolt with his right index finger. Welder sustained a laceration and fracture to the tip of right index finger.
Employee stated that he was repairing a splice, when he attempted to knock a clip off of a pin and his hammer glanced off of the piece of belting and struck his finger.
Employee stated that he was cutting a metal band on a roll of belt at Flat Creek No. 2 belt header when a loose end of the band struck him in the face.
The employee was attempting to cut holes in his glove cuff to facilitate hanging them on a belt hook using his personal jack knife (leatherman tool). The leatherman did not have a locking blade device and the knife closed on his finger causing a laceration which took four stitches to close.
Employee was helping splice West Main belt and changed the blade in his cutting knife, which he thought he had closed the blade in the knife. While he was working on the belt splice his right hand came in contact with the exposed blade cutting the side of his hand where his pinky finger is.
Employee stated that he was attempting to remove bolts from ranging arms. He used a wrench as a cheater bar. The wrench slipped & struck him in the nose. He received 11 stitches.
Apprentice was stripping 4/O THHN wire with razor knife - knife slipped and cut thumb.
Employee stated that while loosening a plug on gear box, he hit his head on side of gear box causing a laceration to the left ear.
Employee stated the weld broke that was used to pull slack out of belt causing the eye and chain to strike him in the head. This resulted in a laceration and a chipped tooth.
Employee stated that he was attempting to cut a plastic tie when the knife slipped, cutting his wrist. The laceration was closed with sutures.
EE was changing a ripper tooth off a dozer. He hit the ripper tooth with a hammer, which knocked off a small piece of metal from the tooth, that hit the EE in the chin, requiring 2 stitches
EE stated that he was cutting a piece of belt. His left hand slipped off of the knife and cut his left index finger. The laceration required 8 sutures.
Two employees were placing a 1'x9'x3/8" piece of metal on side of a truck for welding. The injured employee was using a pry bar to adjust the piece of metal. He had his one hand behind the metal piece when the pry bar slipped. The large piece of metal moved and wedged employee's finger between two pieces of metal.
Employee was using a 3/4" ratchet when the socket slipped off the nut. Employee finger got caught between handle of ratchet and metal he was working on.
EMPLOYEE STATED THAT HE WAS HELPING JACK A ROCK DUST MACHINE BACK ON THE TRACK WITH TWO OTHER EMPLOYEES WHEN HE FELT A SHARP PAIN IN HIS LEFT ELBOW. HE WAS DIAGNOSED WITH A STRAINED LEFT ELBOW. FIRST LOST WORKDAY DID NOT OCCUR TILL 8/22/05.
EMPLOYEE WAS ATTEMPTING TO CLEAN OUT A CHUTE ON A COAL SAMPLER SYSTEM. THE SECONDARY CUTTER OF THE SAMPLER MADE A CUT WITH THE EE'S HAND IN THE CUTTER. HIS HAND WAS WEDGED BETWEEN THE CUTTER & CHUTE OF SAMPLING SYSTEM. EE DID NOT USE PROPER "LOCK OUT" PROCEDURE.
EE states that he "was helping another employee install the plow on the stageloader using 'com-a-longs' and a rock bar." The "com-a-long" chain slipped on the plow resulting in the EE's finger being mashed between the stageloader and chain. EE's first lost work day was 2/24/05.
EE was "breaking" with a hammer to clear the walkway. A piece of the rock slipped in the ee's hand causing a laceration to the right index finger. No lost work days occured as a result of this accident.
The employee was working in the service bay, approximately 10 feet from where a mechanic was trying to remove a toothcap from the bucket of a loader. A piece of metal struck the employee in the left bicep. He was taken to RMCH emergency room and the metal was removed and the wound was sutured.
EE STATED HE WAS CUTTING 2" PLASTIC PIPE W/POCKET KNIFE WHEN THE PIPE SLIPPED RESULTING IN LACERATIONS TO HIS FINGERS.
WHILE CUTTING A PIECE OF ELECTRICAL TAPE, THE UTILITY KNIFE SLIPPED & LACERATED HIS LT HAND, MIDDLE FINGER, DISTAL END. LACERATION WAS 1"LONG & REQUIRED SUTURES TO CLOSE. THE TAPE BEING REPAIR ED IS USED BY SHOOTERS TO MEASURE HOLE DEPTHS LOADING OF HOLES FOR BLASTING. HE WAS HOLDING THE ELECTRICAL TAPE IN LT. HAND BETWEEN THUMB & THE PALM OF HAND.
EE WAS CUTTING A PIECE OF CARDBOARD FROM A BOX. INSTEAD OF PUTTING THE KNIFE AWAY IN A PROPER MANNER, HE STUCK KNIFE INTO THE BENCH WHERE HE WAS SITTING. THE FOLDING KNIFE SHUT CUTTING EMPLOYE E'S THUMB CAUSING HIM TO BE TAKEN TO HOSPITAL FOR STITCHES.
EE WAS ASSEMBLING BIT HAMMERS FOR CRUSHER AND WHILE STRIKING ROLL PIN WITH HAMMER A PIECE OF METAL FLEW OFF AND EMBEDDED IN HIS LEFT HAND.
EE STATED HE WAS SHOVELING OUT A HOLE IN THE L MAIN BLEEDER ENTRY TO SET 3" PUMP WHEN HIS SHOVEL STRUCK A LEDGE IN THE MINE FLOOR AND JAMMED HIS LEFT SHOULDER. FIRST LOST WORK DAY DID NOT OCCU R UNTIL 2-25-03.
EE USING 16# SLEDGE HAMMER TO REMOVE WEDGES FROM DRAG SOCKET. WHEN STRUCK WEDGE A METAL SHARD FLEW OFF STRIKING LEFT CHEEK.
EE WAS REMOVING DIESEL PARTICULATE FILTER WITH CHANNEL LOCK PLIERS WHEN THEY SLIPPED CAUSING HIM TO JAM HIS FINGER INTO PARTICULATE FILTER HOUSING. THIS RESULTED IN A SMALL LACERATION TO HIS LT. INDEX FINGER.
EE WAS HELPING MOUNT TIRE ON A RAM CAR, USING A ROCK BAR TO HELP POSITION THE TIRE WHEN HE FELT A PAIN IN HIS RIGHT SHOULDER AND BACK. HIS FIRST LOST DAY DUE TO SURGERY WAS 4/3/03. O HAVE SURGERY TO REPAIR THE TEAR.
EE WAS USING A ROCK BAR TO PRY A HYDRAULIC JACK WHEN THE BAR SLIPPED, CAUSING EE TO FALL AND CAUGHT HIS HAND BETWEEN THE BAR AND COAL RIB. THIS RESULTED IN A LACERATION TO HIS RIGHT INDEX FING ER.
EE WAS HELPING ANOTHER EE DISMANTLE AN OVERCAST WHICH HAD BEEN TAKEN OUT OF SERVICE. THEY WERE REMOVING THE LAST SIDE SECTIONS OF TING WITH SLEDGEHAMMERS WHEN THE OVERCAST SUPPORT BEAMS SHIFTE D AND FELL, STRIKING EE ON THE SHOULDERS CAUSING HIM TO FALL TO THE MINE FLOOR. THIS RESULTED IN CONTUSIONS TO HIS SHOULDERS AND LACERATIONS TO BOTH LIPS.
