Kemmerer Mine Coal

Employee was cleaning coal spill with a shovel. The employee noticed lower back pain at home after shift. Employee thought it would heal over the long weekend. On 12/3/2025 the employee sought medical attention and then reported the injury to Supervision on 12/3/2025. Employee will start losing time 12/8/2025.

Employee got dizzy and sat down. Employee then passed out and fell to the floor. They received a laceration to the left eye brow. The laceration needed 3 sutures to close it.

Employee was coming down a ladder when they slipped and fell to the ground. Employee was diagnosed with a fractured tailbone.

Employee was removing boom shims from a 6060 Shovel. The shim moved unexpectedly and pinched the employee's finger between it and a bolt. The employee received 3 stitches in the left ring finger.

Employee was diagnosed with carpel tunnel in the right hand. The physician stated the injury is from operating the controls on a dozer. Employee required surgery.

While walking to the steam bay to retrieve a truck EE slipped on some ice and trying to catch themself grabbed the side of a dumpster and experienced pain in the shoulder, neck and chest.

Employee stated they had a sore neck from a truck bed coming down onto the frame unusually hard. The employee saw an Ortho who ordered an MRI and the Ortho will not let the employee return to work.

Employee exited their haul truck to look it over. When doing so they started to slip on the uneven ground. They twisted in an odd way to prevent themselves from falling. In doing so they strained muscles in their lower back causing muscle spasms.

Employee was standing at the bottom of a ladder looking up asking another person a question when the employee fainted falling backwards and hit their head on the floor which resulted in a laceration on the back of their head and causing unconsciousness.

While exiting a D10 Bulldozer after a day of training on this equipment, the employee missed the first step and fell to the ground on EE's right side.

While the Employee was hooking up a strap to lift a piece of the equipment while in the process the strap came loose and the hook on the strap struck the employee in the teeth causing one to split and chipping another.

Employee was cleaning up EE's work area in an outdoor location when EE slipped and fell in the mud. Employee experienced pain in the groin area. Initial diagnosis is a pulled muscle. Employee was placed on light duty.

While assisting to lift a Highwall Drill drill bit into the back of a vehicle the employee felt a pull in lower back. Employee did not think much about it and the pain grew worse with movement. After work on Thursday the pain grew worse and employee did not return on Friday. Went to doctor and was restricted until 10/17/23 when employee returned to work.

A smoker was observed in the Twin Creek Pit 2 highwall that is a non active area of the mine. Access to the highwall is difficult so the smoker cannot be extinguished within the 30 minutes allowed. The Kemmerer Mine is monitoring the smoker while making a plan to extinguish.

Employee was doing EE's inspection on the 34 Shovel house when the Employee tripped and fell causing injury to EE's torso.

EE slipped while getting out of personal vehicle and caught self on the way down wrenching EE's back. Pain gradually gotten worse.

Employee slipped on ice in the pit. The employee tried to break the fall and in doing so landed on right elbow. This caused a small fracture in the elbow area requiring surgery.

Employee was doing impoundment inspections and fell off of the snow machine EE was riding. This caused a broken right index finger.

This injury is an Occupational Illness, repetitive stress injury that was diagnosed on December 30, 2022 as being caused by pressing on the fuel pedal. *Previous treatment was ineffective. Another procedure was done on 8/29/24 with missed workdays starting 9/3/24.

Miner was loosening a Final Drive Drain Bolt. The socket slipped and the miners finger was pinched between the rim and the wrench. This resulted in a broken left ring finger.

Employee was lifting samples of coal at the tipple from the sampler. They felt a pop in their back. The employee finished their work and then let supervisor know of the incident.

The employee was lifting a pallet off of a rack when the pallet broke and caused pain in the left shoulder. The employee was checked at the local hospital and returned to duty. The employee followed up with an Orthopedic at a later date and was told has a torn Labrum. The employee continued to work until surgery on 5/26/2022.

Miner was walking from a port-a-potty to the haul truck, when EE stepped on a rock and EE's Left knee popped.

Employee was cutting into a wire harness with a cutting tool when it slipped and cut the top of EE's right thumb.

The employee brushed against a service truck leaf spring that was stored in the Warehouse. The leaf spring began to fall and the employee reached out to grab it. In doing so the leaf spring pinched employee's right middle finger between it and a desk. The pinch resulted in a laceration that needed 7 stitches to close.

While working in the afternoon with temperature near 90 degrees, an electrician working on power line and elevated in a man bucket lift, experienced heat exposure.

Employee was walking in a muddy condition. The employee felt pain in the right knee. The diagnosis is a strained MCL.

The employee arrived at Elkol Tipple for lunch when it was noticed that EE had trouble concentrating and responding to other employees. The employees gave first aid and called our MRT team. The employee was assessed and taken to South Lincoln Medical Center where EE was treated for CO poisoning.

Employee experienced lower back pain during the shift. The pain was significant enough that EE went to the doctor who prescribed medications that prevented the individual from returning back to EE's next scheduled shift.

The employee claims EE was descending the stairway of a 793 Haul Truck in the maintenance shop. When EE made the last step to the concrete EE felt a burning sensation in EE's left foot. The initial diagnosis is a partial tear of the left Achilles tendon. The employee is awaiting a case number from Workers Comp in order to schedule EE's MRI to verify the tear.

While walking from vehicle to the blend building employee slipped on ice resulting from melt and landed on hip and shoulder.

Employee was hooking up air line to start a truck and when applying pressure had a "pop" in their shoulder.

The center cutting edge on a scraper was being adjusted to the next set of retaining holes. While rolling the edge over to clean the back of the edge it shifted pinching the middle finger on the left hand causing a laceration.

While retrieving an oil supply hose from a service island the hose became hung up on the outside of the reel causing the employee to stop short straining the right shoulder.

While standing on a ladder breaking loose bolts for the air dryer on a 793C haul truck. The employee strained the left shoulder during the process.

A strap was being used to check the pivot bearing on a grader. The strap broke and struck the employee in the left hand. Ligaments in the left hand were weakened and the employee was released on light duty.

The employee was exiting a pickup and tripped and fell out onto the ground. The foot was tangled in wiring fed through the passenger door. After medical review it was determined that the right ankle was fractured and that EE would need to wear a boot for three (3) weeks.

Employee reports numbness and pain in both right and left hand/wrist while completing daily work assignments and from years of repetitive motion.

Employee was operating a Cat 793D haul truck and was being loaded by a L1400 Letourneau loader. The loader put the third bucket in the truck when a rock rolled to the right side of the truck box and caused the truck to rock which caused the employee pain in the neck area. Employee was seen at the local clinic and released to work. The employee missed their next scheduled shift.

EE notified supervisor at end of shift their lt. eye was irritated. EE claimed that during safety meeting at start of shift something got into eye & over the shift caused the irritation. Supv. offered to have EE seen by mine rescue & get eye flushed. EE refused treatment at mine; however, sought med. attn. after shift. There was nothing in eye to remove but cornea was scratched.

Employee was working on the explosive crew and was carrying a 35 pound box of boosters across the pattern and felt a sharp pain in their right knee. The employee has managed the injury but required surgery on 1-29-2019 which is also the first missed shift for this employee.

Employee was using a hand press to remove a u-joint from a light vehicle driveline when the driveline shifted and contacted the employee under the chin. The laceration required 3 stitches to close.

As employee was climbing the boarding ladder on to 59 haul truck they contacted their knee with the boarding ladder attachment. Employee later called the supervisor reporting that their knee was injured and their neck was sore.

Employee was repairing exhaust on a D10T dozer and strained their right wrist. Physician released employee to work. Physician prescribed a support brace on 11-22-2017 and the wrist required surgery on 1-16-2018.

While servicing a hydraulic shovel the employee walked into an sun visor lacerating left ear.

Employee was removing a carrier plate off of a D11 dozer and had their left thumb pinched between the plate and the swing frame. The employee pulled their thumb free and noticed it was bleeding. Local medical center doctor advised employee to come in after the shift. The wound took 2 stitches to close.

Employee was removing a wheel chock from the bumper of a haul truck and felt a pop in their right shoulder. Employee was seen at local medical center and diagnosed with non-surgical tears in the rotator cuff muscles.

Employee reported to line-out meeting at beginning of the shift and was attending a safety presentation that was to be 1 1/2 hours in duration. 30 minutes into the training, the employee was not feeling well and was going to go get some fresh air. The employee stood up and fainted and dropped to the floor. This resulted in a non-displaced hairline fracture of upper jaw.

Employee was assigned duties as a shop mechanic. Employee was installing a tire on a haul truck and claims that they injured their back while performing above mentioned duties. The Mine Operator is contesting that the claimed injury occurred on the mine site.

Employee is a tipple laborer and was diagnosed with carpal tunnel illness which required surgery on 4/10/2017. I was not notified of employee absence until 5/1/2017.

Employee was evaluating a stuck vehicle and was walking to the rear of the vehicle and tripped on a windrow of rocks. While stumbling forward, the employee planted their right foot and felt a pop in their right knee.

Operator was boarding a dozer and slipped and fell to the ground.

While servicing porta-potty's with a contractor the employee strained a shoulder muscle while loading a porta-potty onto the rear transport tray.

An employee was kneeling down testing the voltage on a switch, when employee stood up, employee felt a pain go down back. MRT was dispatched and employee was taken to the hospital for evaluation.

While working on a dozer, a sledge hammer slid off the final drive of the dozer and smashed the employee's finger between the hammer and the dozer swing frame. The injury required 4 stitches.

While moving chock blocks and pulling on a hose reel, an employee had a sharp abdominal pain. Employee was taken to the hospital for observation. It was determined the employee had a hernia that will require surgery.

Employee complained of back pain after sitting in a meeting. Employee took next scheduled shift off due to the back pain. Workers Comp accepted the claim on 3/22/2016. At this time the incident is under investigation and the company has objected to the claim.

While pushing and pulling on a dumpster, the employee aggravated a pre existing medical back issue.

The fuel truck was going up a ramp. The lights of a passing haul truck made it hard for the operator to see. The fuel truck hit the berm on the edge of the haul road and tipped the rear of the fuel truck onto its left side. The operator cab portion stayed upright. The operator was transported via ambulance to South Lincoln Medical Center for injuries he received from the incident.

The employee was leaving Mine Medical on her way to the warehouse. The employee was stepping off of the last step of four steps when her knee gave out. She tried catching herself unsuccessfully and fell to the ground contacting her face onto the ground. This caused a chipped tooth, facial abrasions and contusion on her left knee.

Employee contracted carpal tunnel from years as a welder. The first surgery was done on 7/17/15.

While rappelling during a Mine Rescue Training, the employees left knee twisted when they contacted the ground. The employee did not seek any medical attention for several weeks. The ACL was torn and required surgery on 9/16/15. Work activities were not restricted until that date.

An employee was working on a haul truck when dirt and rust debris fell making contact with their face. Some of the debris went between the safety glasses and face getting into the employees eye. The employee was transported to the hospital where a pysician was able to remove the material and an antibiotic was prescribed for the eye.

An employee felt a sharp pain in his right knee while walking up the stairs of a shovel. The employee did not miss any work or have any restrictions until 6/17/15 when he underwent surgery to repair his knee.

An employee reported on 3/23/15 that his shoulder had been hurting him for a couple months. On 3/27/15 he saw a physician. An MRI was taken. It was determined that years of repetitive motion had worn out the shoulder and it would require surgery.

While EE was putting cutting edges on a grader the cutting edge fell off the magnet at approx. 3 feet high and struck the EE on the left foot. The cutting edge landed on the back side of the EEs steel toe boot causing a fracture in his left big toe and required stitches to his left big toe.

Employee was moving a metal plate and felt burning in his groin area. Initial treatment was ice and ibuprofen. Employee said he felt ok. Days later the employee had a doctor examination and it was determined a hernia that needed fixed. Surgery was done on 12/12/14.

An employee stepped in a parking ditch causing his knee to pop and causing pain. The employee was seen by an orthopedic specialist and an MRI was done on 10/17/14. This was the soonest he could be seen. He was released back to work on restricted duty on 10/17/14.

An employee was pulling on a rail switch when they felt a burning sensation in their back. The employee woke up the next morning with numbness extending down the left leg. After the investigation and an MRI, it was determined that the employee had an acute herniation of the L3 and L4 vertebrae. This information was made available to this office on 10/14/14.

Employee was boarding their truck and struck their left knee on the deck above the ladder. No treatment was given until 10/29/14 when surgery was done for a torn meniscus.

Employee reported that he was diagnosed with Carpal Tunnel in his right wrist. No surgery date has been set at this time.

An employee was experiencing shoulder pain in his right shoulder but wasn't sure if he hurt it at work. He reported this pain on 5/8/14. A surgery was scheduled on 8/6/14. After the surgery, the surgeon said there was no evidence of any one injury. The pain was a result of years of repetitive motion.

An employee was descending a platform ladder and slipped on the third step down causing him to twist his ankle. The employee resumed work without informing his supervisor of the incident. The employee had the ankle checked the next day and X-rays revealed that it was broken.

As employees were arriving for shift the smell of fire was noticed in the shop area. Employees then investigated to discover smoke on the hill north of the maintenance shop. A grass fire was identified and reported to the shift supervisor who then took action to notify the local fire district to extinguish the fire. No injuries or damage to structure occurred.

An employee lifted a chock block to place it back on a fuel truck. This action caused him pain in his right arm. No medical treatment other than ibuprofen was given. The employee stated that the pain never went away and it was looked at by a doctor on 8/5/14. At this time the employee was given work restrictions and surgery was scheduled. Surgery was done on 8/13/14.

The employee stepped onto the deck of an overburden drill that was wet due to rain and slipped. This caused him to twist his knee. The knee required surgery 20 days later.

While attempting to close the hopper door on a train car, one of the employees feet slipped on the snowy ground causing his little finger to get caught in the hoppers locking mechanism. This caused the bone in the tip of the finger to be broken and caused a laceration requiring two stitches.

The employee was bent over pounding stakes for a drill pattern. The employee felt pain in his lower back that radiated down his right leg. The employee was transported to the local clinic and the medical provider prescribed a prescription (muscle relaxer) which caused the employee to miss his next shift.

The employee was using a hydraulic ram to remove the rear wheels on Lowboy. As the ram pressured up, it slipped and smashed the employees left thumb between the ram and the inside of the wheel. The employee was transported to the hospital where three stitches were required to close a laceration on the thumb.

The employee was working on a booster pump. While pulling on a six inch pipe trying to get it into place, the employee felt a pop and tearing sensation in his left shoulder. This shoulder had been injured previously in 2010 that required surgery.

A bead flange from a haul truck tire fell from tire being installed landing on the left foot of employee breaking the employee's foot in multiple places. Employee required surgery.

Employee was removing an apron cylinder from a scraper with the overhead crane. The cylinder became stuck, employee wiggled the cylinder loose. When the cylinder came loose his right middle finger was pinched between cylinder and machine. Employee was taken to the Medical Center, finger was x-rayed and showed finger was broken.

The employee was walking on the deck of a haul truck with both of her hands full when she tripped and fell. When the employee fell, her head came into contact with the coolant reservoir tank causing a slight fracture to the employee's forehead.

The employee was lifting some belt material when he felt a pain in his right arm. The employee continued to work at full capacity until the employee was placed on modified duty on 8/6/2012.

The employee was driving a post with a post driver when the driver slipped off the post and contacted the employee on his hard hat.

Employee got his arm caught in the door of a pick up and fell while trying to stop it from rolling. This caused his right radius and ulna to be fractured.

Employee struck a pin hammer with another hammer causing a metal projectile to contact his chin.

Employee cut left index finger while changing a blade on a razor knife. The cut required seven stitches.

An employees haul truck rolled backwards making contact with a highwall at an angle that caused the truck to tip over onto its right side.

Worker was installing boxes and conduit when a gust of wind blew dust and particulate matter into his eyes. Went to doctor next day for exam. No metal in eyes. Flushed with water. Prescribed antibiotic drops as a precaution. Released to full duty.

Employee slipped on frozen ground falling on his right shoulder after cleaning his tail lights on his overburden truck.

The EE complained of sore back 9/27/11 & declined medical care. On 10/3/11 he was taken to clinic for pain and clinic diagnosed inflamed muscles & released to full duty. MRI was completed on 10/6/11 and physician transferred EE to original physician in Idaho due to pre-existing injury. 10/9 EE went to Idaho to see his dr and is still on full duty.

Electrician was injured when a pry bar he was using to place an electric panel slipped and cut his left thigh just above the knee. The injured worker was transported by a supervisor to the nearby medical center where the wound was cleaned and sutured. Returned to work the same day with no restrictions.

Employee was assisting a maintenance field mechanic installing the Y-link on a coal haul truck. While maneuvering the Y-link, the employees finger was caught between a wood block and the Y-link.

A worker looked up inside a truck dump hopper under construction and got some debris in his left eye. He was wearing safety glasses. The wind was gusting at the time, and it was later determined that the debris in his eye was dust. He complained of discomfort and irritation. He was prescribed an antibiotic ointment as a precaution against infection.

An employee was using a small band saw in the shop to cut off the end of a pipe nipple. Employee was using his gloved hand to saw off the end of the nipple when his hand slipped and went into the blade cutting through the glove and into the end of his right thumb.

Employee was stepping on an overburden truck when pain was felt in knee.

Employee slipped and fell on ice between shop and steambay dislocating knee cap.

Employee alleges that on January 10, 2011 he fell inside an engine compartment on a Cat 793. As a result from catching himself he injured his left shoulder. This incident was reported to the mine on 4/20/11.

Employee slipped and fell on icy concrete in the steam bay. As a result there was green stick fracture to right fibula.

Employee was operating a fuel truck when the operator drove through a drainage ditch going across a haul road. The employee fractured two lower vertebrae's as a result of the impact.

Employee was using a pneumatic chisel to remove bits from a pick roll, when a small piece of metal broke loose from the chisel and struck the employees left arm.

Employee was lifting a 5 gallon bucket with 4 gallons of water in the bucket. As the employee lifted the bucket, they felt a sharp pain in their lower back. As a result the employee re-injured his back from an off the job injury that resulted in back surgery earlier this year.

Employee was assisting another employee on 384 drill. Employee took a step and stumbled, as the employee attempted to catch himself, his foot caught the hydraulic ram, causing the employee to fall from an elevated height of 67" landing on his head.

Employee was using an 8 pound sledge hammer to strike a breaker bar. The hammer glanced off and pinched employees right index finger between the hammer handle and the breaker bar.

Employee was working on a water truck and was using the tailgate to access the rear axle box. Employee was climbing from the ground into the back of their pickup when the employee felt their right knee pop causing pain to the employee. Employee had surgery on 8/20/10 to repair a torn meniscus and returned to work on 8/23/10 with work restrictions.

Employee alleges that he has carpal tunnel in both wrist from operating equipment.

Employee was walking back to his fuel truck, when he slipped and fell on his back.

Employee alleges that over 34 years of being a mechanic he has carpal tunnel in both hands. Currently the employee has not elected to have surgery.

Employee was on the ground putting chokers on a large rock. Employee tripped and his right shoulder stuck the ground causing a burning, tingling, numbness to the arm and shoulder.

Employee was tapping mounting holes so new bolts would thread easier when mounting a new starter. The employees head was directly under the mounting holes when a piece of foreign material fell in his right eye. The employee used eye drops and thought he flushed it out. The next day the mechanic went to a doctor and a piece of metal was removed from eye.

Employee was using a pocket knife to remove a zip tie when it slipped cutting his left thumb. Employee received 4 stitches as a result of the laceration.

Employee was removing a broken trip cable on a P&H 4100 shovel. As he attempted to spin the cable drum to continue spooling out the cable, he pinched his finger between the drum and guard.

Employee had surgery on right hand due to carpal tunnel. He is an electrician by classification and alleges that doing his job he acquired the symptoms for carpal tunnel.

Employee stepped off a ladder and felt pain on his left side of rib cage.

Employee was walking and said his knee started to hurt.

When removing the S-cam on brakes, the employee struck it with a hammer and a piece of metal went into his left eye.

Employee was swinging a sledge hammer and missed what he was swinging at. Employee felt like something pulled and reported next day.

Employee was in the process of removing two hydraulic lines on a hydraulic cylinder. After removing one line without problems, he was in the process of removing the second line when pressurized hydraulic oil sprayed him in the face.

While installing the saddle block at the load zone on a belt, the saddle fell over coming into contact with the EE's legs. Intial diagnosis was a first aid, however after continuing pain, the employee had an MRI done which showed a fractured left tibia.

Employee was complaining of pain by his stomach on his left side. Employee stated that he had pain all day but thought he could come to work. After taking employee to the hospital, he was diagnosed with a ruptured appendix and was scheduled for an emergency appendectomy that evening.

Employee was air arching underneath a rubber tire dozer, however flashes reflected underneath welding hood causing flash burn to left eye. Employee reported incident to supervisor on May 8, 2009 at 10:00 am.

While attempting to weld a sign to a post, the sign fell on the employees left index finger which was between the sign and welding table. The left index figer was fractured as a result of this incident.

Employee was lifting a scraper for belt when he strained his right shoulder.

Employee was carrying two boxes containing a gear and another with bolts. When going through the door to the shop where he was delivering the parts he tripped on the door threshold. The employ ee caught his balance but felt a burning sensation in his stomach. After visiting the doctor it was found to be a hernia in the stomach area.

Employee was attempting to dislodge a tape measure that became stuck when taking measurements on a shovel bucket. When the tape did not come loose the employee grabbed the tape and pulled cutting his right index finger.

Employee was hoisting a valve into place with an overhead crane & sling. The valve broke striking the employee in the lower jaw causing a laceration that had to be stitched.

Employee was walking when she stepped on a dust covered sheet of ice. The employee fell on her backside and left arm. The employee went to the doctor and was released on restricted duty.

An employee was getting in position to help guide a truck out of the shop. While stepping backwards he slipped on some ice and fell to the ground.

While waiting for the employees shift to start the employee started cleaning up her reporting area. While using a shovel to move garbage, she felt a sharp pain in her hip. The employee went to the doctor and was not released to return to work because of the hip injury.

Employee was preparing to drill a hole in the overburden. When applying down pressure, the pressure regulator apparently did not work and created full down pressure on the drill steel. This action lifted the drill off its jacks causing the drill to tip over.

Employee was directing air flow onto himself from the air vents and inadvertently stuck his finger into the blower fan.

The employee alleges while lifting a 4" water hose to attach to a valve on a truck he pulled a muscle in his arm. The employee continued to work with no restrictions from the doctor until his surgery on August 15th, 2008.

While servicing a haul truck the employee stepped on uneven ground twisting his knee. The employee continued work for 4 months before irritation of the knee set in and had it examined. The physician found a tear in the meniscus and it was determined suregery was needed which was performed on November 11, 2008.

Employee was attempting to tip a standing full 55 gallon drum of oil in order for a forklift to get under the barrel to remove it from the back of a truck. The employee did not report anything unusual at this time, but did report the following morning he had incurred pain in torso area, allegedly from moving the barrel on the previous day.

Two employees engaged in horse play and sprayed cleaning agent into another employee's eyes. There was irritation to the eye but no damage according to the doctor.

Employee alleges his work duties through the years has caused Carpal Tunnel Syndrome in his hand requiring surgery to remedy the situation.

Employee was preparing to climb onto a dozer when his foot slipped. Employee held onto the hand rails causing him to strain muscles in his arm.

Employee's hand became caught between door and door frame. Employee opened door to get hand out. Employee went to doctor when returning home from work for exam. Laceration took 6 stitches to sew up.

Employee was boarding #057 haul truck. His hand slipped from handrail and fell to the ground. He fell on his left shoulder, dislocating it. He gloves were wet and muddy due to rain and just dismounted, another truck contributing to the incident.

Employee alleges continuous operation of heavy equipment has caused pain to his lower back.

Apprentice was stripping 4/O THHN wire with razor knife - knife slipped and cut thumb.

Employee was in the process of closing an inspection door of the crusher at Sorensen tipple. When closing door it fell off the hinge and fell on his foot.

Employee was climbing down from a 5' scaffold using the scaffold rungs as steps. Loosing his footing, he fell approximately 3.5' to the ground.

EE alleges that welding for 20 years has caused carpal tunnel syndrome. Surgery required.

EE was changing a ripper tooth off a dozer. He hit the ripper tooth with a hammer, which knocked off a small piece of metal from the tooth, that hit the EE in the chin, requiring 2 stitches

EE was removing magnets from a tube going to a transmission screen. EE was pounding the tube down in order to remove magnets. While pulling a magnet from the tube a sharp edge was formed and the EE cut finger on the edge of the tube.

Two employees were placing a 1'x9'x3/8" piece of metal on side of a truck for welding. The injured employee was using a pry bar to adjust the piece of metal. He had his one hand behind the metal piece when the pry bar slipped. The large piece of metal moved and wedged employee's finger between two pieces of metal.

EE moved a chain that was dangling in his way on to conveyor while doing his work the chain fell from the belt striking him in the mouth. The ee's right canine tooth was chipped and then repaired the next day.

Employee was using a 3/4" ratchet when the socket slipped off the nut. Employee finger got caught between handle of ratchet and metal he was working on.

EMPLOYEE ALLEGES THAT THE NUMBNESS, TINGLING AND PAIN INCURRED WAS CAUSED FROM THE REPETITIVE WORK DONE OVER THE YEARS. CARPAL TUNNEL SYNDROME.

Employee was walking around dozer when his foot sank in soft dirt causing ankle to roll. Employee worked two days before going to doctor and missing work.

On August 8, 2005, employee had surgery on his right elbow because of a condition know as "lateral epicondylitis" (tennis elbow). He did not considere this condition as work-related. Today (9/19/05) we submitted his claim to the Wyoming Workers Comp Division for medical review. If the state determines it to be nonwork-related, we will notify you to remove this case from our record.

The employee was lifting a piece of 54" wide belting, approximately 10' long, into the bed of a pickup. While lifting the belt, he felt a sharp pain in his lower stomach area. He reported the incident but did not ask for medical assistance. The employee did not miss any work until August 17, 2005, when it was determined by his doctor he would need surgery to repair the hernia.

EMPLOYEE WAS ATTEMPTING TO CLEAN OUT A CHUTE ON A COAL SAMPLER SYSTEM. THE SECONDARY CUTTER OF THE SAMPLER MADE A CUT WITH THE EE'S HAND IN THE CUTTER. HIS HAND WAS WEDGED BETWEEN THE CUTTER & CHUTE OF SAMPLING SYSTEM. EE DID NOT USE PROPER "LOCK OUT" PROCEDURE.

EE was operating a dozer 0-10 pushing and ripping overburden. after his shift was over he noticed his back being sore, which the ee alleges was caused from the roughness of the dozer and type of work he had done. the following day he went to the dr and received a cortizone shot to relieve the pan. the dr did not restrict the ee from any work.

Employee was operating cable peeler picking up trail cable behind a shovel. Employee had the reel too high and backed up an incline causing the reeler to tip on its side.

Employee was getting out of mantrip and bumped his knee on the seat in front of him. He went to the doctor when his shift ended and fluid was drained from the knee. Knee had been injured approximately 6 to 8 weeks previous to this work incident, according to the employee.

EE WALKED OUT OF BATH HOUSE, SLIPPED ON SOME ICE ON WALKWAY INJURING LEFT HIP

EE WAS HELPING TO PUT TRAIL CABLE IN A D RING CABLE HOLDER. ANOTHER EE WAS TAPING TRAIL CABLE BEHIND A SHOVEL TO THE HOLDER. D RING SLING SLIPPED OUT OF EE HAND AND HIT EE IN THE MOUTH, CHIPPING A FRONT TOOTH. EE HAD TOOTH REPAIRED. EE NOTIFIED MANAGEMENT ON 02/25/05 THAT TOOTH WAS GOING TO BE REPAIRED

EE WAS HELPING TO MOVE ENERGIZED TRAIL CABLE BEHIND SHOVEL AND STEPPED ON A ROCK CAUSING EE TO SPRAIN LEFT ANKLE. EE DID NOT USE ANY TYPE OF ILLUMINATION TO ASSIST EE TO LOCATE ANY DANGERS

EE alleges continuous operation of equipment has caused the need for carpal tunnel surgery on both wrists of the EE.

Employee alleges his normal work routine while on the job contributed to his carpal tunnel illness.

Employee alleges continuous operating of equipment over several years caused numbness in hands and wrists resulting in the need of carpal tunnel surgery.

EMPLOYEE ALLEGES THAT YEARS OF REPETITIVE STRESS AND JARRING WHILE OPERATING HEAVY EQUIPMENT RESULTED IN A DEGENERATIVE CONDITION THAT CAUSES SEVERE PAIN IN HIS LOWER BACK. ON AUGUST 23, 2004 HE APPLIED FOR COMPENSATION IN THIS CASE. WE WERE NOTIFIED ON SEPTEMBER 27TH THAT AN AWARD OF COMPENSATION HAS BEEN MADE.

EMPLOYEE SAYS THAT CARPAL TUNNEL SURGERY IS REQUIRED AS A RESULT OF REPEATED TRAUMA CAUSED BY OPERATING HEAVY EQUIPMENT OVER THE YEARS.

EMPLOYEE WAS STANDING ON A TANK TO INSTALL A HEATER ON THE ROOF OF A BUILDING. WHEN PULLING THE HEATER UP ON TOP OF THE TANK IT TIPPED AND EMPLOYEE GRABBED IT WITH HIS RIGHT HAND. WITH HIS LEFT HAND HE ATTEMPTED TO PUSH DOWN ON HIGHER END TO LEVEL IT OUT. EMPLOYEE FELT PAIN IN GROIN AREA.

EE ALLEGES THAT HIS DRIVING HEAVY TRUCKS AT MINE CAUSED HIS CARPAL TUNNEL ILLNESS.

EMPLOYEE REPORTED DISCOMFORT/PAIN IN HIS KNEE. MEDICAL EVALUATION AND TREATMENT INDICATE THAT YEARS OF REPETIVE MOTION (STOOPING, WALKING, BENDING, KNEELING, ETC) COMBINED WITH AN ARTHRITIC CO NDITION OF THE KNEE LED TO PROGRESSIVE WEAKENING OF THE MENISCUS. EMPLOYEE UNDERWENT ORTHOSCOPIC SURGERY. ALTHOUGH THIS CASE COULD BE A PERSONAL HEALTH ISSUE & NOW WORK RELATED WC AWARD COMP

CARPAL TUNNEL SURGERY, EE ALLEGES ILLNESS OCCURRED FROM OPERATING EQUIPMENT FOR 20 YEARS.

EE ALLEGES THAT DRIVING A TRUCK AT THE MINE HAS CAUSED CARPAL TUNNEL SYNDROME IN HIS WRISTS

EE WAS CUTTING A PIECE OF CARDBOARD FROM A BOX. INSTEAD OF PUTTING THE KNIFE AWAY IN A PROPER MANNER, HE STUCK KNIFE INTO THE BENCH WHERE HE WAS SITTING. THE FOLDING KNIFE SHUT CUTTING EMPLOYE E'S THUMB CAUSING HIM TO BE TAKEN TO HOSPITAL FOR STITCHES.

EE WAS LOWERING AUGER ON EXPLOSIVES TRUCK. HE PUT ARM UNDER THE AUGER TO GAIN ACCESS TO AUGER CONTROL. WHEN LOWERING AUGER IT CAME DOWN ON EE'S ELBOW AND BROKE HIS ARM.

EMPLOYEE WAS WASHING A DOZER IN THE STEAMBAY. A THIN FILM OF ICE DEVELOPED ON TOP OF DOZER DUE TO THE WASHING. EMPLOYEE WENT TO TOP OF DOZER TO CONTINUE CLEANING AND SLIPPED ON ICE. HE EXTENDE D HIS KNEE CAUSING A STRAIN ON IT.

EE HAD PULLED GREASE HOSE FROM REEL IN SHOP TO EQUIPMENT (122 DOZER) HE WAS WORKING ON. HE WENT BACK TO REEL TO PULL MORE HOSE OUT AND TRIPPED OVER HOSE.

REPETITIVE STRESS INJURY (CARPAL TUNNEL SYNDROME) RESULTING FROM HAND/ WRIST MOTION OVER TIME WHILE OPERATING EQUIPMENT.

EMPLOYEE WAS DRIVING FUEL TRUCK WHEN HE HIT AN ICY PATCH OF ROAD CAUSING THE TRUCK TO GO INTO A SKID. WHILE TRUCK WAS SLIDING IT HIT A DRY SECTION OF ROAD AND THE FUEL TRUCK TURNED OVER ON ITS SIDE.

EE WAS CUTTING WOOD ON A TABLE SAW WHICH WERE 2" X 2" IN SIZE. HE THEN STARTED CUTTING SMALLER PIECES OF WOOD & DID NOT ADJUST HEIGHT OF BLADE, LEAVING BLADE HIGHER THAN NEEDED. WHEN PUSHING S MALLER WOOD THROUGH THE CUT FINGER CAME IN CONTACT WITH BLADE CUTTING TIP OF LEFT INDEX FINGER.

EE WAS SERVICING EQUIPMENT AS HE WALKED AROUND SERVICE TRUCK HE STUMBLED AND FELL TO THE GROUND. HE SUFFERED A CUT ABOVE HIS RIGHT EYE. STITCHES WERE REQUIRED.

EE WAS WORKING TO REMOVE BOLTS FROM A DOZER WHEN HE EXPERIENCED KNEE STRAIN.

EE WAS ATTEMPTING TO LOAD WATER TRUCK. WYHEN HE TRIED TO OPEN VALVE TO LET WATER FLOW IT WOULD NOT OPEN. EE THEN JUMPED UP TO GRAB ROPE ON VALVE TO PUT ALL OF HIS WEIGHT ON ROPT, ATTEMPTING TO FREE STUCK VALVE. WHEN VALVE CAME LOOSE, EE HIT GROUND (LANDING ON HIS FEE) AND FELT SHARP PAIN IN HIS BACK.

WAS USING LARGE GUZZLER HOSE TO VACUUM UP SPILLED COAL. HIS FOOT SLIPPED, PUTTING STRAIN ON HIS ABDOMEN, CAUSING PAIN AND SWELLING. HE CONTINUEDWORKING FOR SEVERAL WEEKS BUT HAS NOW GONE IN FO R SURGERY AND WILL BE OFF SEVERAL WEEKS. DR SAIDHE HAD A HERNIA.

TWO EMPLOYEES WERE CAR POOLING TO WORK IN A PERSONAL PICKUP. THEY ARRIVED AT THE PUBLIC PARKING LOT AND ONE GOT OUT. THE OTHER TURNED THE CAR SHARPLY AND KNOCKED HIM DOWN. THE RIGHT REAR TIRE PICKUP KNOCKED HIM DOWN & RIGHT REAR TIRE RAN OVER HIS ANKLE & LOWER LEG CAUSING SEVERE BRUISING

EE WAS UNHOOKING COMPRESSOR FROM TRUCK. AFTER DISCONNECTING COMPRESSOR IT ROLLED FORWARD AND EE CAUGHT HAND BETWEEN TRUCK AND HITCH ON COMPRESSOR.

EMPLOYEE WAS STEPPING UP TO GET ON A SMALL STOOL (15"HIGH) WITH HIS RIGHT LEG FIRST. WHEN HE STEPPED UP THE LEFT KNEE GAVE OUT FROM UNDER HIM. HIS LEFT KNEE HAD BEEN HURTING HIM AT LEAST A WEE K PREVIOUS TO THIS INCIDENT.

EE SAYS HE SUFFERS FROM CARPAL TUNNEL SYNDROME IN THE RIGHT WRIST AND HAND. ORIGINAL DATE ON REPORT IS 5-25-00. HE CONTINUED WORKING UNTIL LAST WEEK WHEN HE UNDERWENT SURGERY ON 6-19-01. HE SA YS CAUSE IS REPATITIVE HAND/WRIST MOTION OVER TIME.

EMPLOYEE WAS MOUNTING A DOZER, SLIPPED ON DECK &STRUCK ELBOW ON HANDRAIL.

USING A RAZOR KNIFE TO CUT A NYLON STRAP WHEN KNIFE SLIPPED AND CUT EE'S THUMB. SEVERAL STITCHES WERE REQUIRED. EE RETURNED TO WORK SAME DAY.

CO-WORKER WAS USING A HAMMER DURING A REPAIR OPERATION WHEN A SMALL METAL FRAGMENT FLEW INTO EMPLO9YEE'S EYE AND BECAME IMBEDDED. HE WAS WEARING SAFETY GLASSES AT THE TIME. FRAGMENT WAS REMO VED BY DR. NO IMPAIRMENT.

DISMOUNTING WATER TRUCK, HIS HAND SLIPPED OFF HANDRAIL AS HE WAS PASSING THE BOTTOM STEP. HE LANDED HARD ON HIS RIGHT FOOT, STRAINING HIS KNEE. HE CONTINUED TO WORK BUT NOW HAS TAKEN TIME OFF TO HAVE SURGERY ON THE KNEE.

PERFORMING MAINTENANCE WORK IN SHOP ON EUCLID WATER TRUCK WHEN PARTICLE OF DIRT GOT INTO HIS EYE. NO IMPAIRMENT TO VISAION. DR. WAS ABLE TO REMOVE THE DIRT.

MAKING UP TIRE/RIM ASSEMBLY FOR HAULAGE TRUCK. WHILE WORKING TO POSITION THE LOCK RING, THE LOCK RING SLIPPED AND PINCHED EE'S FINGER.

EMPLOYEE SUFFERS FROM DEGENERATIVE CONDITION OF THE LEFT KNEE WHICH CAUSES HIM PAIN FROM TIME TO TIME, ESPECIALLY WHEN HE STANDS UP FROM A KNEELING OR SQUATING POSITION. HE EXPERIENCED SUCH AN EPISODE OF PAIN WHILE ON THE JOB ON 2-16-00 WHEN HE STOOD UP FROM A KNEELING POSITION AS HE WAS TIGHTENING THE BOLTS ON THE FAN SHROUD OF O DOZER.

REMOVING COAL FROM BOTTOM OF 1-UD PIT.DURING THIS PROCESS THE FLOOR ABOVE BEGAN GRADUALLY TO SETTLE AND MOVE. AS A RESULT THE HAUL ROAD BEGAN TO CRACK AND SETTLE ALONG WITH THE FLOOR. FLOOR MO VEMENT STABILIZED AND STOPPED AFTER ABOUT 24 HOURS. MOVEMENT MEASURED INCHES PER HOUR AND TOTALED ABOUT 24 HOURS AND TOTALED 4' TO 15' DISPLACEMENT ALONG RAODWAY FOR ABOUT 200 YARDS.
