EE was being loaded by shovel when shovel bucket contacted the back canopy of production truck causing jostling to the EE.
EE was holding a positioning device while partner was using pneumatic jackhammer to drive out mill bolts on mill. The bolt came loose causing the jackhammer to move forward. Resulted in pinching the EE's finger between the jackhammer & knockout sleeve (positioning tool) causing a laceration to left index finger and stitches.
Employee was welding/grinding inside a duct and felt irritation in their right eye. They flushed with eye wash and got some relief. They stated they weren't sure if it was the result of arc flash or dust in the eye. The next day it was still bothering them and supervisor could see a particulate in the eye.
Employee preparing to cut beams with plasma cutter. As employee was setting and preparing to test the magnetic guide, the trigger activated with employee's left middle finger in front of the tip.
Employee was using a one inch impact gun to remove nuts holding the bucket segments on a loader. The bolt spun and cut the employee's palm on their left hand.
Employee was walking a PC 490 excavator north of the hydro discharge pump house. The excavator's right track slid off the edge of the embankment causing the excavator to roll over onto its side.
Employee was water blasting in a scrubber when debris fell from the ceiling hitting employee in the hand/gun, knocking gun out of employee's hands. This created a potential water injection injury, still yet to be confirmed.
Employee was repairing worn spots on a D-11 dozer blade. While building up the blade, a piece of molten metal fell onto the top of employee's left boot and burned through the leather. The diagnoses was a full thickness burn to the left foot.
Employee did not report an incident to the Company during or after scheduled shift on December 28, 2017. Rather, EE left a message for the Company on December 30, 2017 stating that EE reported to urgent care that day for a sore back. EE informed the Company representative that EE developed a sore back after operating a rental D10 dozer.
Employee utilized dual eye protection while grinding overhead on the 51 shovel project. At shift end, the employee felt something in right eye but did not see anything. The employee reported to the eye doctor the next morning. A small piece of metal was removed and the employee received prescription eye drops.
Employee oiling a 4.5 inch air grinder with a cut-off wheel. While setting the grinder on a work bench, the wheel spun and made contact with the employee's knuckle on right index finger. The grinder cut through EE's weld glove and caused a laceration. The employee received three stitches.
EE was sandblasting on elevated platform wearing personal fall arrest gear and PPE. EE sandblasted one of the nylon straps causing the platform to tilt. EE slid down platform, but fall gear stopped fall. EE lost control of the blast hose, and it remained operational. The freefloating hose then turned toward EE. EE sustained sandlblast injuries to face, hand & leg.
Employee was installing a belly pan with an impact wrench. The socket came loose and struck the employee in the mouth resulting in a cracked tooth.
Employee was sawing a u-clamp with a chopsaw. The clamp slipped and struck employee's left thumb.
Employee was bending a piece of 1 1/4" round bar. While bending the steel, the round bar slipped and the hot steel burned his right wrist.
Employee noticed a pain developing in their lower back. The employee believes the pain developed over his last two shifts while performing frost ripping with a D11 dozer in the pond. The employee was sent into the clinic on February 25, 2015 and received work restrictions.
Employee was performing grinding work. He was wearing safety glasses and a grinding shield. Employee reported the next day that he had a particle enter his eye on the prior shift. The employee requested medical treatment.
McLellan machine boom came into contact with employee's right hand resulting in right wrist contusion and splint.
Employee working inside Mill 7. Hydraulic boom moved unexpectedly and struck employee in chest. Employee was diagnosed with a chest wall contusion.
On January 22, 2014, Employee was performing shovel cut clean-up work with a Komatsu dozer. Two days later, the employee informed his supervisor that his back felt like it was on fire and he had occasional throbbing pain in his back while operating the dozer on January 22nd.
Maintenance Mechanic was changing out the blade of a valve for the Decapus. The employee released an air actuated valve cylinder. Employee held a plate in position under the cylinder and when the pressure released, it pinched his second finger on his left hand. The finger tip required ten (10) sub dermal and surface stitches. Employee referred for additional consultation.
Mechanic was walking in designated walk area in front of HAV unit blowing toward the walkway. The employee left eye was irritated with dust, he used eyewash bottle to try to remove material. Still bothering him and was sent to clinic
Mechanic was changing a new hoist cable on the 61 shovel. He was operating the remote control pendanto for the tugger cable and was watching the cable and his left finger got caught in the snatch block.
EE is a new shovel operator and was moving the shovel to another area. He was standing up and using the joy sticks to operate the shovel. He felt had a back spasm and was removed from the shovel. He was been with the company since March of 2011. He mentioned he had a snowmobile accident a few years ago and has had problems with his back before.
Employee's foot was pinched in Traverse Drive Unit in Crushing Dept. Further investigation pending
EE came into the reporting area after the shift and stated her back was sore after operating the shovel for 12 hrs. The first day taken off the shovels were 02/18/2011. The repetitive motion of the shovel and the personal behavior of the employee has been put on restrictions.
EE was attempting to install a long flanged pipe with a 1/2 impact. The impact did not work, so the EE used a right angle impact to finish the job. As the EE tighten the bolts the impact swung away from his body and jerking his arm. Surgery on 08/23/2011
EE was removing shim bracket on a drum finisher and was using hoisting sling around the drive unit with overhead crane. The sling slipped from the shaft collar and slid causing the drum to drop about 3 inches. When the drum slipped the EE index finger was pinched.
Welder twisted right hand when using 1/2 electric drill with 9/16 bit and jammed in the hole that he was drilling.
EE was cutting a piece of flat bar on the power band saw. When the cut was done, he took his right glove to turn the power off and then released the vise and grabbed the bar to remove. With his glove off, he cut his right index finger on the bar.
EE was cleaning sideframes on the shovel and claimed he burnt himself on right leg. Under further investigation another welder was told from him that his knee was sore before the shift occurred and we believe it was alleged.
EE was using a rivet buster to remove coupling bolts on the low speed gear reducer. The two bolts that wouldn't come out were cut off and the rivet buster was used. A piece of metal flew up and hit him in the forehead.
Employee was arc airing bolts to remove worn liners. The employee felt something enter the right eye. Employee attempted to flush eye, attempt was unsuccessful. Sought medical attention, no particle found in eye, evidence of a minor burn and prescription antibiotic was administered.
Employee is production truck driver and was being loaded by the 50 shovel when a frost chunk fell off the end of the dipper and landed on the canopy of the truck. She reported the incident on Feb 7 and a report only and after the incident she felt a strain in neck and back. She continued working until Feb 18 and was taken off the trucks.
Employee was operating the shovel and was digging out hard and the action caused a sore neck.
Employee was removing wear liners from a shovel dipper. He was using a jack hammer and piece of steel struck him in the right cheek.
Employee was welding under a 14" pipe and during the welding process, a piece of molten metal fell from the weld and lodged against the front of this leg. The next day was treated for the burn and was treated for cellulitis
Employee was using a high pressure nozzle cleaning a truck and fell over from the hose.
Employee was operating a D-11 pushing filter cake and was backing up and was jarred. He was gripping the controls and strained his bicep. After further review and investigations from the safety committe he worked his regular shifts and had 5 days off before seeing a doctor.
Employee stated that this was a reoccurrance of a back injury from previous incident. Under review and futher investigations of the safety committee and found more details of the incident. She was transfered to Shovel operator this week.
Was air arcing a clamp on the bottom side of the FMS 405-2 finisher tails pan, felt something in his eye.
EMPLOYEE WAS LINING #6 GRINDING MILL. WHILE REMOVING A LINER BOLT WITH A RIVET BUSTER, A METAL PARTICLE ENTERED HIS RIGHT EYE UNDER HIS SAFETY GLASSES. EMPLOYEE SUFFERED A CORNEAL ABRASION. HE WAS GIVEN A PRESCRIPTION MEDICATION TO PREVENT INFECTION.

Employee was being loaded at the #62 electric shovel, a BE-295. A rock rolled off the truck canopy and struck and shattered the mirror. A piece of glass entered his right eye. The truck window was open and he was wearing safety glasses. Prescription meds.
When lifting a chop saw off a table, the safety pin on the arm of the chop saw may not have been fully engaged and the arm opened and closed on his hand cutting the top on his middle finger.
WELDER WAS WORKING WITH A TEAM OF WELDERS TO REPAIR THE DISCHARGE END OF A GRINDING MILL. HIS EYES BEGAN TO BOTHER HIM AT THE END OF THE SHIFT. HE WENT TO THE ER AND WAS DIAGNOSED WITH A FLASH BURN
Working on 61 dipper rebuild as Employee was welding a block on the inside of the dipper a hot spark hit him in the left eye and struck his eye lid. He was wearing safety glasses and a welding shield.
EMPLOYEE WAS OPERATING A HIGH PSI LANCE AND LOST HIS GRIP AND CONTROL OF LANCE (APPROXIMATELY 20 - 30' IN LENGTH) AND RECEIVED A CUT TO HIS LEFT THIGH.
EE WAS OPERATING A 240 TN HAULAGE TRUCK. WHILE BEING LOADED AT THE SHOVEL, THE DIPPER BRUSHED THE REAR CORNER OF THE TRUCK JARRING THE DRIVER. THIS REPORT IS BEING FILED BECAUSE OF CITATION #6159918. WE BELIEVE THIS IS NOT REPORTABLE
WHILE WELDING ON A TRUCK EXHAUST HAD TO KNEE ON THE FRAME. A PIECE OF HOT METAL ROLLED DOWN AND BURNED HIS KNEE.
